Commons ChamberI will come on to the underspends, because that is one of the key dividing lines between the two sides of the House. After we came out of the EU, I secured an agreement with the Treasury that all the underspends from the new schemes would remain in the Department for Environment, Food and Rural Affairs budget, for farmers and for DEFRA. At the end of this month, in the Budget, we will see whether the Secretary of State has secured the same terms for any underspends.
We know that the previous Government made a whole raft of commitments that there was no way of paying for, so there is no credibility to the suggestion that the right hon. Gentleman secured something from the discredited Treasury that he was under. The key question is: if that money was so desperately needed, why was it not being spent?
As we came out of the European Union, new schemes were set up, including the SFI, to support nature and farming. They represented a shift from the EU scheme, under which 50% of the money went to 10% of landowners. We were able to design new schemes. We listened to farmers, and that is why I announced at the farming conference an average increase of 10% in payments, and 50 more choices to better reflect the variety of farms, including upland farms. We responded, but the point is that underspends all remained within the DEFRA budget. The key question, which I am sure the Secretary of State will come to, is whether he will give a similar commitment to the House that any underspends will remain in DEFRA, given that we have just faced the wettest winter in 150 years, and given that in September, 10 counties had the worst rain on record.

Commons ChamberI am grateful for your generosity, Madam Deputy Speaker. We are just grateful that at least some Conservative Members have turned up.
On 20 October over 500 homes in Chesterfield, like hundreds across the UK, were flooded, leading to the tragic death of Mrs Gilbert on Tapton Terrace. Less than a month later, with the impeccable timing that only this accident-prone Government are capable of, the National Audit Office announced that the Government had cut by 40% the number of homes that will be protected from floods by 2027. Will the Secretary of State at least promise the House that he will never again say to a flood victim that the Government are doing all they can?
I am sure the whole House extends our condolences on the sad loss of Mrs Gilbert. Our investment in flood asset maintenance is up by £220 million. As I said, the previous six-year investment was £2.6 billion. Between 2021 and 2027, we are now investing £5.2 billion. Yes, there have been pressures on the programme through inflation and covid, as the National Audit Office report shows, but what is not in doubt is the increased funding that this Government are putting into flood protection—the £5.2 billion indicates that—alongside the increased funding for flood maintenance.

Commons ChamberOne of the things that my hon. Friend agrees with is that more decisions should be devolved rather than every decision being made in Westminster. Part of the reason for integrated care boards is so that they can look at where best to allocate their funds locally. He raises an extremely important point. He is right that around a quarter of delayed discharges are on the social care side—a fifth actually, in the NHS; there are a number of factors within that, which we will need to disaggregate.
On my hon. Friend’s point about local capacity, the Government are allocating the funding to his local ICB. I am sure he will have a conversation with his ICB on where the spare capacity can be best identified and rolled out at pace.
Last week, I met Hal Spencer, the chief executive of Chesterfield Royal Hospital; the pressures that he and his staff had faced as the hospital went into a critical incident over Christmas were etched all over his face. He spoke about the pressures on A&E registrars, ambulance drivers and nurses and about coming face to face with people who had been waiting 24 hours in a corridor on a trolley or who had been waiting many hours for an ambulance to turn up.
Is not the reality that this is a system-wide failure 13 years in the making? Did the right hon. Member for Gainsborough (Sir Edward Leigh) not hit the nail on the head in saying that Labour has a long-term plan for our NHS and this Government do not?
On the hon. Gentleman’s first point, this is absolutely a system-wide challenge. That is why the use of innovations such as virtual wards in demand management upstream, in the care home or on the home, is important, just as discharge—getting patients to leave hospital who are fit to do so—is important. The focus has often been on ambulances being delayed at A&E or on the significant and real pressures in emergency departments themselves, but the challenge is much wider. That is what the funding in the autumn statement recognised.
In response to his second point about this being a longer-term issue in England specifically, I would just point him to the examples in Wales and the pressures in Scotland. This surge in flu combined with covid and the pandemic legacy that we have seen in England have created so much pressure over the festive period, and it is something with which many other health systems around the globe have also been grappling.

Friend is a strong champion for this issue; when I was Chief Secretary to the Treasury, I remember him lobbying me about how a bridge from one bit of the hospital estate to another could provide additional capacity to meet the pressures his trust has faced. That is partly why we have been working intensively with the trusts that have the most severe cases of ambulance delays, looking through the work of the taskforce at best practice and what works best in those settings, and ensuring that the trust chief execs have the right level of support. It is important to recognise that the problem does not always manifest where it is caused. Quite often, challenges on the social care side, or further upstream in the conveyancing rate, put pressure on an emergency department and on the trust.
The Secretary of State is absolutely right about the abject failure in care that his Government have overseen over the past 12 years, but his statement did not refer so much to the pressures in A&E. It seems entirely wrong to me that if someone walks into an A&E department they are its responsibility, but if they turn up in an ambulance they are expected to sit in it for hours on end until the A&E is willing to take responsibility. Will the Secretary of State say more about dealing with the issue so that A&E departments realise that however someone arrives—whether they walk through the door or arrive in an ambulance—they should be the responsibility of the A&E, and the ambulance should be out fetching other people in the area?
The hon. Member makes a very fair point. Within the question he raises is the unmet need where an ambulance does not reach a patient in the community, as opposed to the known risk once the patient is within the hospital trust’s purview. On capacity in A&E, as I touched on in my statement, we put in £450 million at the 2020 spending review to upgrade A&E facilities at 120 trusts.
With respect to the hon. Member’s specific point, he may be aware of the letter that the NHS medical director Professor Stephen Powis and the chief nurse Ruth May sent at the time of the heatwave about where risk sits within hospitals. The taskforce has been doing further work on pre-cohorting, post-cohorting and observation bays so that we can better free up that ambulance capacity and get it back on the road.

Commons ChamberOf course, decisions on quantitative easing are for the Bank of England, which is independent. The last time I looked, I think the initial response to the global financial crisis was approximately £75 billion, and there has been about a twelvefold increase in QE since then, so I understand my hon. Friend’s underlying point. Ultimately, what my right hon. Friend the Chancellor has been focused on is the plan for jobs and supporting the economic recovery. We can see from the output data that the economy grew by 2.3% in April, as I said earlier, and GDP data has come out of the Office for Budget Responsibility forecast.
However, as my hon. Friend, who takes a deep interest in the matter, well knows, the picture remains challenging. There were 1.9%—or half a million—fewer employees in May than in February 2020, and 3.4 million people are still on furlough. It is a challenging picture, but I think that the plan for jobs is working, and the data suggests that.
The impact of these restrictions on Britain’s pubs has been very tough indeed, but it has been even worse for nightclubs that have been unable to open at all. It seems entirely wrong to me that, from 1 July, a nightclub that is unable to open will be paying a 33% business rate bill and seeing an increase in its furlough contributions. Given that the Government’s failure has forced them to extend how long the nightclubs are closed for, will the Chief Secretary confirm that he will consider whether nightclubs should no longer be expected to pay that 33% on their business rates?
The hon. Gentleman raises a perfectly legitimate point about how acutely that sector in particular has been affected, as I think everyone in government recognises, but I do not think it fair to say that the Government have not announced any measures that reflect those challenges. Indeed, on commercial rent, he will have heard in my statement today’s specific announcement that applies to the sector. There are also other things, such as the furlough going long, the restart grant and a number of things within the comprehensive package, that are obviously of benefit to nightclubs.

My hon. Friend raises an issue that unites the House. The huge value of the work done by the hospice movement was recognised as part of the package of measures put in place by my right hon. Friend the Chancellor, with £750 million of support for the charity sector and with the hospice movement being specifically identified. I am happy to continue working with my hon. Friend as we work together, and we recognise the importance of that sector.
Many small business owners have been forced to raid their personal savings to keep their businesses afloat over the past eight months, but that is not a bottomless pit. Many small business owners are not wealthy people, and the Chief Secretary to the Treasury must understand that if they go under, the whole company goes with them. The Treasury has now had eight months to get this scheme working. Why is it still excluding 3 million people? Does he not recognise how perilous the situation is for many of those businesses?
I drew attention earlier to the fact that more than £13 billion had been allocated to the self-employed income support scheme and through the income support grant. That indicates the support that the Treasury has given. The hon. Gentleman draws out, as the Chair of the Treasury Committee did, the specific issues around company directors. I have set out to the House the difficulty of clarifying precisely what is earned income as opposed to dividend income, but it is worth drawing the House’s attention to the fact that more than £13 billion of support has been allocated.

Commons ChamberFirst, the hon. Gentleman is right to recognise the importance of today, the anniversary of a national tragedy that unites us all. As I said to him last time, I am keen to work with him constructively, as I know he is, to take that work forward. Later today, I have a call with the Finance Ministers in the Scottish, Welsh and Northern Ireland Administrations, so I will be able to talk further about that. As a fellow Unionist, he will know that one of the advantages we have had throughout this pandemic is the broad shoulders we have been able to provide as a United Kingdom to the various business support and job support measures. What is announced for England is subject to the usual Barnett process, and I will discuss that. One of the concerns of Members across the House is about decisions taken in Wales that have an economic impact. It is important that these decisions are co-ordinated through the Joint Biosecurity Centre, in order that we have a consistent, scientific approach. That is a key issue that a number of Members have concerns about.
The Minister suggested a moment ago that the Mayor of Manchester had not been constructive; he praised how constructive the Mayors who had come to an agreement with him were. The Mayor of Manchester made a request for £90 million and was willing to be negotiated all the way down to £65 million, which sounds incredibly constructive to me. In the spirit of trying to bring everyone back together, would it not be better for him to recognise that the Mayor of Manchester is doing what he thinks is right—as are other Mayors and council leaders in other areas, whether or not they agree with the position that the Prime Minister and the Government come up with—and to say that everyone in this is attempting to be constructive?
I was making a factual point that picked up on the opening remarks of the hon. Member for Ashton-under-Lyne. The Mayor of Manchester, in his discussions with Government, expressly said that Manchester should be treated differently from other areas. The hon. Lady said that the Mayor of Manchester “spoke for Great Britain”. That does a disservice to other local leaders, who have also spoken for their areas and have worked constructively with Government. I do not think it is the case that the Mayor of Manchester, unlike the Mayor of Liverpool, speaks for Great Britain in the way that the hon. Lady suggested.
The Mayor of Manchester’s position is not deliverable operationally, because local authorities do not have access to welfare payments for the dynamic aspect of joint job support—I can come on to that in my remarks—and it was at odds with the tiering approach that we set out. There is a difference, I am sorry to say, between the approach taken by the Mayor of Manchester and the constructive approach taken by other local leaders. I do not accept the premise from the hon. Lady that the Mayor of Manchester alone speaks for Great Britain, and other local leaders in Liverpool, Lancashire, South Yorkshire and elsewhere do not, or that businesses in those areas should in some way be treated worse than the businesses in Manchester—that seems a remarkable position for the Opposition to take.

Commons ChamberThe service from the East Midlands Ambulance Service NHS Trust has been a considerable disappointment for many of my constituents in recent months. When I met them about the service, they told me that on a huge number of occasions they have ambulances sat waiting outside accident and emergency departments, rather than getting to the next call. What more can the Government do to make sure we get these A&Es cleared?
The hon. Gentleman is right to say that we need to improve those handovers. We have improvement programmes in place at 11 hospital sites in the east midlands, alongside which we are making a £4.9 million investment in 37 new ambulances. Part of this is also about the length of stay and addressing the pathway.

I was just about to come on to the voluntary sector, because that is where the hon. Lady’s speech started, but in her remarks she talked about the four CCGs coming together as part of the “efficiencies of scale”—her precise phrase—so I shall come back to the voluntary sector later.
I am probably in the position of largely agreeing with the Minister. I remember that, back in 2010, we had the Derbyshire primary care trust, but then the Lansley reforms came in, broke up the PCT and turned it into five different organisations in North Derbyshire. Can he imagine how galling it is for us to hear that those organisations, which went from a very strong financial position back in 2010, are now in utter financial chaos, so the Government are going to undo the Lansley reforms and to get those economies of scale that we were telling them about back in 2010?
There seems to be a slight contradiction in the hon. Gentleman’s argument. He is arguing that, on the one hand, the financial position was strong in 2016—I remind him simply that the Lansley reforms were in 2012—and, on the other hand, that the issue is with the Lansley reforms.
May I make a point of clarification, because the Minister is misquoting me? I said that the financial position was strong in 2010, not in 2016.
