The Public Accounts Committee has investigated HMRC deals in the past and it is of course welcome to do so again. It gave HMRC a clean bill of health on its approach to these things. We are introducing country by country reporting, and the regulations came into force last week. That is happening only because this Prime Minister put the matter on the agenda in this country and internationally, and I have been calling, at the EU and at the G20, for an international agreement on public reporting so that we can know what companies are paying in different jurisdictions rather than just reading reports about it.

The local economy in Hornchurch and Upminster comprises thousands of small businesses. Can the Chancellor offer any encouragement to the Federation of Small Businesses, which is pressing for tax simplification in order to reduce the burden of tax administration for small businesses?

One of the areas in which we can make progress is the digitisation of the tax system, which could help a lot businesses. I would also make the point that the Office of Tax Simplification has been strengthened and we are putting it on a statutory footing. We are looking forward to seeing a couple of reports from it over the next few days on what we can do to help small businesses in particular.

I endorse those comments completely.

Swap for swap outcomes are much more likely for RBS customers and the percentage of non-compliant sales that do not result in a tear-up of the agreement within RBS has gone from about 40% to about 60%, which is not in line with other examples. I would argue that the voluntary scheme that the FCA put together is not delivering and is not being monitored in accordance with the FCA’s mission statement. I will leave that issue there because I have spoken at length about interest rate swap mis-selling in this Chamber and made my concerns known time and again.

When the voluntary redress scheme was announced, we thought that the inclusion of consequential losses was a pleasant surprise. I am afraid that we were being overly optimistic. Our analysis of the redress scheme showed that in 50% of the 3,104 cases that we looked at, no consequential losses were received, and in 85% of the cases that did receive consequential losses, they amounted to less than £10,000. I have personally seen dozens of well-argued cases in the redress scheme that have been rejected by the banks without an explanation. Even worse, the business is allowed one appeal against that decision without knowing the basis on which it has been rejected—it has one opportunity to challenge, and invariably that challenge fails. On consequential losses we are again failing businesses.

Time and again cases go to court. They are often settled outside court, where the settlement will be better than what was offered under the redress scheme, and that should be a cause of concern for the regulator. Of perhaps even more concern to Members of the House is the fact that time and again gagging orders are placed on those settlements by a taxpayer-funded bank on the back of taxpayers. I find that utterly unacceptable.

Let me move on to the Connaught Income Fund, which creates a real problem concerning regulation in this country. The regulator was informed not of mismanagement but of fraudulent behaviour, yet it took four months before it put a notice on its website to highlight its concerns and say that the fund in question was not as safe as a bank account, and a further year before that fund was wound up. In the meantime, between the whistleblower informing the regulator about the problem with Connaught and the winding up of the scheme, more than half the total investments into the Connaught income stream occurred. It could therefore be argued that the regulator was responsible for at least half the fund.

Dr Eric Saunderson is one of many constituents who made significant losses under the Connaught investment scheme. He is not just frustrated by the inordinate length of time that the FCA is taking to investigate that and decide what compensation might be payable, but he is still unclear as to whether that compensation—once decided—will be paid directly to investors or put into the suspended Connaught fund.

My hon. Friend makes an important point, and there is a lack of clarity on that issue. Only last week the FCA decided to publish on its website the fact that there has been a settlement between the liquidators and Capita. Interestingly, the decision to publish that statement on its website breached the confidentiality agreement. Therefore, the regulator was commenting on a settlement, but the parties to that settlement were not able to offer any advice because there is a confidentiality agreement. The FCA has a track record of being accused of breaching confidentiality, but to publish the fact that it is taking a degree of responsibility for the outcome is utterly unacceptable in view of the fact that the liquidators are stating categorically that they are not associating themselves in any way, shape or form with the statement, that they do not agree with aspects of that statement, and that the only reason they had to force a settlement was that the mediation that the FCA decided to try to arrange was not successful. Indeed, the decision to finish the mediation was made without consultation. For the regulator to make a statement on its website that is then categorically denied by the parties to the agreement is a matter of concern.

I, like most colleagues here, have had correspondence with constituents who have lost considerable amounts of savings, with this often changing the direction of their lives as the amounts are so significant. Does my hon. Friend share my hope that the strength of feeling being shown in this Chamber today will force some urgency to be put into finding a solution to this?

Again, I fully agree with my hon. Friend’s comments, and I do think that tonight’s turnout indicates the concern across the House on this issue. It is important to point out again that the proposed mediation was described as the fastest way of getting some compensation to investors, which is why the APPG welcomed it, although with some reservations. A deadline date of 31 October 2014 was offered for the completion of that mediation. Subsequently, in November 2014 the FCA announced a new date of the end of January 2015, and then in January 2015 it announced a new date of the end of March.

On 9 March 2015, I was asked to meet Martin Wheatley, the now former chief executive of the FCA, in Portcullis House, where I was informed that the FCA was withdrawing from the mediation process—that was announced the following day. Again, the decision was unilateral. In effect, the decision to go for mediation was a unilateral one made by the FCA without consulting other stakeholders, as was the decision to end the mediation. As chair of the APPG, I think it essential that the FCA explains why it took those decisions. It needs to explain why it thought it was better to end the mediation rather than continue with a method of dealing with this issue that it had claimed would be the most effective way to proceed.

This is an incredibly serious issue and I am sure that Members on both sides of the House have dealt with tragic cases of women who have been put at risk of violence or who have suffered violence at the hands of their partner or someone close to them. Protective injunctions remain within the scope of legal aid, and immigration cases in which domestic violence is a factor continue to qualify for funding. We have also recently scrapped the application fee for those injunctions to ensure that there are no unnecessary barriers between people and the help they need, and we have said that if there are any other areas in which legal aid is not being made available, we want to be made aware of them. I am happy to look again at the issues that the hon. Lady has raised.

Most police officers are sympathetic and helpful when women report incidents of domestic violence, but sadly some still have a negative attitude. What more does my hon. Friend think can be done to ensure that police responses are consistent, which is particularly important for women reporting serial offences?

My hon. Friend is absolutely right that there are many excellent police officers up and down the country who respond incredibly sympathetically and supportively to those who make complaints or allegations of violence. It is important that victims of sexual abuse feel empowered to come forward to report that abuse and I know that my right hon. Friend the Home Secretary and all Ministers across Government would like to encourage people to do that and to encourage the police to take all allegations seriously. If there is anything further we should be doing, I am happy to look into it.

My hon. Friend is, of course, right, but we also need to consider the wider argument: it is good for women to have the choice about whether working is the right thing for them to do given their family circumstances and any other responsibilities they may have. We want to make it as easy as possible for women to work, and I will come on to discuss child care.

I am sure that the Minister will agree that half the population are women and the vast majority of them are successful and contributing, not victims, as the Labour party would have us believe. Does she agree that local businesses employing only a small number of people will benefit from more deregulation, which would enable them to offer jobs to even more women, which they can fit in with having children at school?

My hon. Friend rightly says that 50% of the population are female—it would be nice if we saw a few more women in the House of Commons to represent that, but all parties are working on it. She is also right about deregulation. I am sure that she welcomes, as I do, the National Insurance Contributions Bill, which the Exchequer Secretary is taking through the Public Bill Committee at the moment. It will give all small businesses a £2,000 employment allowance so that they can recruit more people.

Will the shadow Minister remind the House how much was lost to the Treasury during the period of the 50p tax rate, and does he accept that 100% of 45% is more than 50% of 50%?

I am not sure about the hon. Lady’s maths, but we are still within the period of the 50p rate. Of course we want to see the details of what has been happening. However, while the Conservatives have the notion that for those who are very wealthy, the higher tax rates are a deterrent and create avoidance, they do not say the same about the poorest and the middle-income families in the rest of the country. They can pay VAT at 20%; they can pay higher taxes. The hon. Lady takes a view that is taken by so many Conservatives. There is one law for those who are very wealthy, but everyone else must suffer because of the Conservatives’ failure on revenue and borrowing.

No. I shall make a little progress, devastating though the hon. Gentleman’s interventions so often are.

We have always been quite clear that the proposed mansion tax is an issue on which the two parties in the coalition have differing views. Our Liberal Democrat colleagues have supported the principle for some time. I am sure that the Under-Secretary of State for Communities and Local Government, my right hon. Friend the Member for Bath (Mr Foster) will make that clear when he winds up the debate. In contrast, Conservative Ministers have very real concerns over such a proposal. We have concerns that a third of the properties in London worth more than £2 million have been in the same ownership for over 10 years, and that a mansion tax could hit asset-rich but potentially income-poor households, a point made by my hon. Friend the Member for Battersea (Jane Ellison).

My hon. Friend will know that £2 million does not buy a mansion in London, and certainly not in outer London, where I have a number of constituents who moved out from inner London decades ago. Their homes have increased in value beyond their wildest dreams over a very long period, but they are in fact cash-poor, quite often living on a modest pension. The thought of paying very large amounts of tax every year for the privilege of owning a home that they have had for many years would be extremely frightening. Can the Minister think of any practical way that an elderly person in that position could possibly pay that tax?

I noticed that that very point was one that the hon. Member for Nottingham East seemed to struggling with. He seemed to suggest that there were ways in which the Opposition would address that. I am not sure whether that was included in the costings they have produced. There is an issue for the asset-rich, cash-poor which would need to be addressed in the design and would obviously have an impact on the costing.

May I start by saying that I appreciate the waiving of my customary Whiply silence, albeit temporarily, to enable me to participate in this debate?

I thank my hon. Friend the Member for Chatham and Aylesford (Tracey Crouch) for raising the important issue of the loneliness and isolation that can affect older people. I welcome the good work done by Independent Age, Age UK Oxfordshire, Counsel and Care, and the WRVS on the campaign to end loneliness. We are living longer, healthier lives. We should celebrate that, and seek to unlock the rich potential of our older population, as well as promoting their well-being.

We recognise the terrible impact that isolation and loneliness can have on people’s health and well-being. We know that multi-professional collaboration from a health and social care perspective on the needs of older people—including recognising isolation and those at risk from it—will make a huge contribution to keeping older people well and independent in their own homes, and to helping to maintain a decent quality of life for them. Of course, combating loneliness and isolation cannot be the job of health and care services alone. A range of services must be involved, including transport, housing and leisure.

We recently concluded the caring for our future engagement exercise, and we will produce a White Paper and a progress report on funding. That is planned for spring 2012. The engagement exercise considered six areas: quality and work force; personalisation and choice; shaping local care services; prevention and early intervention: integration; and the role of financial services. Throughout this engagement exercise we heard from a wide range of organisations, carers and people who use services, and the issue of loneliness and isolation among older people was raised.

Under the provisions of the Health and Social Care Bill currently before Parliament, local health and wellbeing boards will take responsibility for producing the joint strategic needs assessment and a local health and well-being strategy. I pay tribute to the London borough of Havering; it has shown great commitment in setting up its health and wellbeing board and it has already made significant arrangements for taking on this important new role. I also applaud the good work done by Age Action Alliance, an independent alliance of organisations working together to improve the lives of older people. It is aiming to prevent deprivation in later life, as well as challenging age discrimination and seeking to make older people feel valued and able to contribute to their local communities and the wider society. We look forward to the ideas that will emerge from that alliance.

We are doing everything we can, and we also support the efforts of others, to ensure that older people have access to all the help they need to reduce social isolation. I hope that that reassures my hon. Friend.

The hon. Member for Plymouth, Moor View (Alison Seabeck) made a thoughtful contribution about the understanding, patience and sympathy people with hearing loss need but do not always receive. She described the limitations on everyday activities such as using the telephone, and the absence of subtitles on television, which greatly disadvantage those with hearing loss. I shall refer those matters to the relevant Minister. We hope to improve the quality of life of people with hearing loss.

My hon. Friend the Member for Mid Derbyshire (Pauline Latham) raised concerns about two constituents who were unable to receive specific cancer drugs under their NHS treatment from the East Midlands strategic health authority. She has made her feelings very clear. The health authority will have heard her and will wish to respond with some urgency; and the Secretary of State will, I am sure, expect that to happen.

I thank the right hon. Member for Leicester East (Keith Vaz) for his question and his continued support for diabetes prevention and improving outcomes for people with diabetes. We pay particular tribute to his work through Silver Star, a charity he founded in Leicestershire that is invaluable in tackling diabetes within south Asian communities. As president of the Havering branch of Diabetes UK, I should like to take this opportunity to pay tribute to the late Sue Braeger, who sadly died recently. As chairman of the Havering branch, Sue was a formidable campaigner on diabetes issues, especially the management of diabetes medication for pupils in schools. She will be a hard act to follow.

We have learned this year—in the last few months, in fact—that nearly 3 million people in the United Kingdom have diabetes, a number that grows year on year. Worse, 24,000 people each year die unnecessarily from the disease—deaths that could have been prevented with better management and care. Much progress has been made in diabetes care since the publication of the national service framework in 2001, but prevention and early diagnosis remain a Government priority.

Next year, the National Audit Office and the NHS leadership team will be reviewing progress and considering whether there is need for further work, co-ordinated at a national level. Any such work would of course seek to reinforce and support activity led by clinicians at local level to improve outcomes for people living with diabetes.

We will also depend on the NHS health check programme, which has the potential to prevent many cases of type 2 diabetes and identify thousands more cases earlier. We will be continuing the change for life campaign, which raises awareness of the importance of maintaining a healthy weight and being physically active. As type 2 diabetes is linked to both obesity and inactivity, these public health initiatives are crucial.

For people diagnosed with diabetes, our priorities for treatment and care are to improve quality of life and reduce complications, and as a result to reduce cost. People with diabetes account for 15% of in-patient hospital beds in England. Their hospital treatment costs £600 million a year more than that for patients admitted without diabetes. Poor management of diabetes and insulin leads to emergency admissions and readmissions, and increased lengths of hospital stay. Poor care can also lead to deaths and permanent disability, with an estimated 80% of the 73 lower-limb amputations suffered each week by people with diabetes considered preventable.

The NAO will be reporting next summer on its study of the management of diabetes services. We expect it to provide robust recommendations on improving services and outcomes for patients and the public, and we look forward to seeing the results.

My hon. Friend the Member for Totnes (Dr Wollaston) raised the important issue of the role of community hospitals and leagues of friends. I should like to assure her that the Government are committed to helping the NHS work better by extending best practice on improving discharge from acute hospitals, and increasing access to care and treatment in the community. Community hospitals can be an important part of delivering this, especially in rural areas, providing both planned and unplanned acute care and diagnostic services closer to home. Community hospitals support best practice in admission avoidance and provide a range of services, from treating minor injuries to intensive rehabilitation. Subject to the passage of the Health and Social Care Bill through Parliament, clinical commissioning groups will be responsible for securing the best health care and health outcomes for their patients and locality.

The Department announced on 4 August that NHS trusts and NHS foundation trusts will also be given the chance to acquire estate from primary care trusts, including the community hospital estate. PCTs have reviewed and provisionally agreed lists of property for transfer to NHS bodies, and those will shortly be approved by the Department of Health. It is expected that the actual transfers of estate will commence in 2012. I know that this is a concern of my hon. Friend, but it is not expected that these changes will affect the role or function of local league of friends’ volunteers, who provide such valuable and important services in community hospitals around the country

The Government are also committed to increasing the scope of a more transparent rules-based funding system, where money follows the patient. Since its introduction, the payment by results national tariff has been mainly restricted to treatments provided in acute hospitals. We want to change that, but in a way that supports the delivery of high-quality services. That will not be easy, as there are significant challenges for us to overcome, such as making sure that activity that takes place in community settings is recorded and reported, as this is essential to plan services and drive payments, but we are making good progress. From April 2012, we will introduce the first ever tariffs for post-discharge care, with transparent prices to give more certainty about funding. I hope that that sets my hon. Friend’s mind at rest.
