It is a pleasure, Sir Edward, to serve under your chairmanship. I thank the hon. Member for Glasgow North East (Mr Bain) for securing this extremely important debate. As chair of the associate all-party group for the Republic of Sudan and South Sudan, his experience and knowledge is huge. His was a powerful exposition of the situation in South Sudan, and I thank him for that.
The UK is deeply concerned—I am deeply concerned—by the terrible violence in South Sudan that began on 15 December 2013. We have been at the forefront of humanitarian and political efforts. Conflict is continuing in parts of the country and there are large numbers of reported deaths. The ethnic dimension, which the hon. Gentleman mentioned, of some attacks is deeply worrying. We have been calling on all sides to ensure protection of civilians and respect for human rights. We are urging the leaders on both sides to remain engaged in the talks led by the Intergovernmental Authority on Development and to agree on the immediate cessation of hostilities while pursuing a sustainable political solution to the crisis, rather than continuing to seek resolution through military means.
South Sudan is close to my heart. It was the first country that I visited when I took up my post. I saw an 18 month-old country full of optimism, hope, possibility and potential. It was undeveloped and there were huge needs and challenges—but it was all there. It is heartbreaking to think how much of a setback the current situation will be and to wonder how any good things can continue. Even before the crisis, as the hon. Gentleman said, South Sudan had one of the largest humanitarian emergencies in the world, with a complex mix of needs, including refugees—more than 225,000 are Sudanese. I went to the camps on the border of Blue Nile and South Kordofan, and refugees were pouring across the border. As the hon. Gentleman said, there are no roads, and we had to go by helicopter. It is a logistical nightmare, particularly in the rainy season, when everything becomes inaccessible.
South Sudan, a poor, young and vulnerable country, already faced immense challenges, and now it has deep difficulties, including internal displacement of people, populations affected by floods, returnees from Sudan and a large population affected by chronic, severe food insecurity. Those pre-existing issues have not gone away, but the response is being disrupted directly in areas affected by the current crisis, and indirectly elsewhere, as a result of disruption of supply routes and markets, and the evacuation of humanitarian agency staff. In addition, the humanitarian situation that has developed as a direct result of the conflict is extremely urgent. We estimate that there are more than 350,000 displaced people within South Sudan, with large numbers in UN bases, and an additional 50,000 people have been displaced to bordering countries—the majority to Uganda. Many are in acute need of food, health care, shelter, clean water, sanitation and protection, and we are concerned about reports of the conditions in which some displaced people are living, including in UN camps. Thousands of people are yet to receive any humanitarian assistance, and remain fearful for their safety and that of their families.
The hon. Member for Wirral South (Alison McGovern) spoke about the people who were on a boat on the Nile and, sadly, drowned. People are swimming across the Nile trying to escape their dreadful situation, because it is better than staying. Aid agencies are doing a tremendous job in reaching those in need, where security allows. The ability to provide assistance remains severely constrained because of the fragile security situation and subsequent lack of humanitarian actors on the ground. Without progress on the security front, aid efforts will remain inadequate, especially in the areas worst affected by conflict, such as the town of Bor in Jonglei state.
The humanitarian crisis remains our priority and the focus of our efforts. Our existing significant humanitarian programme—more than £60 million in 2013—has put us on the front foot in responding to the crisis with the redeployment of funds to ensure a rapid response. DFID is supporting the efforts of aid agencies through direct funding and by supporting logistics. On 30 December the Secretary of State announced a further £12.5 million commitment to provide immediate supplies and support to key organisations operating in South Sudan, including the International Committee of the Red Cross and UN agencies. That package of support will provide emergency medical treatment for thousands of people, clean water to 5,000 people and tents for 7,500 people who have fled their homes, including family-sized tents which provide women, girls and young children with some privacy and a safe space. That issue was raised, rightly, and it is a priority for DFID. The support package will also support the logistics of the humanitarian effort, through support to the UN’s humanitarian air service, which airlifts aid workers and humanitarian supplies to those in need across the country. On 5 January a UK-funded emergency relief flight carrying life-saving water and sanitation equipment landed in South Sudan to avert a potential health crisis that was emerging from the conflict. Via trusted partners, we are closely monitoring the situation on the ground and we stand ready to provide further assistance as required.
The facilitation and provision of humanitarian assistance, on the basis of need alone, must be allowed. We have called on all parties to ensure safe, secure access for humanitarian agencies and respect their neutrality, and to meet their own obligations to avoid civilian casualties and direct attacks on civilians and civilian infrastructure.
As I have said, before the current crisis, South Sudan was already one of the poorest countries in the world and more than half of its population were below the poverty line. Alongside our emergency humanitarian funding, the Department for International Development has a long-term development programme. However, that has been completely disrupted—I cannot put it any other way—because so many of our staff have had to leave, as have our implementing partners. Our priority at this point is to assess and respond to the immediate humanitarian situation. In the UN Security Council the UK was quick to agree to an emergency troop uplift of 5,500 to the UN mission in South Sudan, almost doubling existing numbers. We are now considering how to give more support to UNMISS through airlifts and by helping to fill gaps.
As to the specifics on what representations have been made, we are co-ordinating closely with other donor agencies and development actors and we know that further pledges are coming down the line, and are likely to be confirmed in the coming days, to contribute to the immediate funding shortfall that the hon. Member for Glasgow North East raised. As I have said, we have already contributed an extra £12.5 million. On the matter of UNMISS, I heard criticisms of the UN operation when I was in the country, and particularly in Jonglei. The need for regular review of all peacekeeping missions is built into the system, and clearly the latest events will have to be taken into full consideration at the next review.
The UN has been gathering disturbing reports of human rights abuses, and we are deeply concerned, as everyone must be. We welcome efforts by the UN Mission in South Sudan to investigate reports of abuses and ensure that civilians are protected. The UN Security Council agreed on 24 December to strengthen the UNMISS human rights component. We of course welcome the Sudan People’s Liberation Army’s announcement that it will investigate the involvement of organised forces in the killings in Juba. The UK is currently in discussion with international partners on how best an independent review can be taken forward. We welcome the UN Secretary-General’s announcement on Friday that he would send the assistant Secretary-General for Human Rights, Ivan Simonovic, to South Sudan.
On 10 January the UN Security Council statement gave a clear welcome to the African Union Peace and Security Council’s decision to establish a commission of investigation to ensure accountability, reconciliation and healing among all South Sudanese communities. The hon. Gentleman raised those issues and they are at the core of the matter. The situation may have started with political machinations, but the people were living side by side perfectly peacefully and they have now been drawn completely unnecessarily into ethnic divisions. The immediate focus is obviously on the cessation of hostilities, but I agree about the way forward in the longer term.
Humanitarian access is severely constrained. UN agencies and NGOs are on the ground and providing support where they can. I do not have the full breakdown of the uses to which new UNMISS troops will be put. DFID is still in the process of assessing needs, and therefore the deployment will presumably follow the needs that are expressed.
I will touch on the political side, and the work of the Foreign and Commonwealth Office. The Foreign Secretary has been in touch with both Kiir and Machar and is trying to bring all sides together. The Intergovernmental Authority on Development is the mechanism for taking those matters forward, but we are urging President Kiir to ensure that the issue of detainees will not prevent constructive discussions from progressing. We are working closely with our Troika partners, the US and Norway, and the EU and regional players, to support the political process.
I thank all hon. Members who have attended the debate for their interest in and concern about a matter that is close to my heart. The hon. Member for Glasgow North East gave an excellent exposition of the situation. My Department, working hand in hand with the Foreign Office and Ministry of Defence, will continue to focus our efforts on ensuring that we are responding to the humanitarian situation and continuing efforts to support the political process.

It is a pleasure to serve under your chairmanship, Ms Clark. I welcome this opportunity to speak on behalf of the Department for International Development in response to the debate on the report of the International Development Committee about global food security.
The report was warmly welcomed by my Department. It addresses an area of critical concern, as many Members have mentioned, and I congratulate all hon. Members on their contributions today. There has been a lot of wisdom in the speeches from the Committee members and Opposition Members about this critical issue. It is critical because feeding a growing human population sustainably into the future, in the face of climate change and resource depletion, is challenging. In a world where 842 million people go to bed hungry and 26% of the world’s children suffer from stunting due to malnutrition, an equally difficult challenge to address, it is vital to ensure that the UK’s aid and development efforts are effective in making a difference.
I want to address as many as possible of the points raised, and to make some of my own. The report was studied closely in DFID and other Departments, including the Department for Environment, Food and Rural Affairs, the Department of Energy and Climate Change and the Department for Transport. The Government’s response combined all those perspectives and departmental priorities.
My opposite number, the hon. Member for Wirral South (Alison McGovern), said that we are lukewarm, but I do not agree. The Government agree or partially agree with 33 of the 39 recommendations in the report and that is not a bad response to a report with so many recommendations. Everyone wants DFID to do everything, which is one of the challenges that we must try to accommodate.
We disagree with only two recommendations. I will go into them in more detail, but one was food waste, and DEFRA noted that voluntary controls rather than mandatory targets work best in reducing waste. On the recommendation on strategic food stocks, the Government believe that functioning markets rather than Government intervention are a better way to manage food stocks. I will address that fully in a moment.
In areas where my Department leads, the report addresses food and nutrition security, focusing on production, the role of smallholder farmers, reducing waste and loss in the food system and providing social safety nets for the most vulnerable people. Hon. Members raised those points, and DFID already prioritises all of them. The report also tackles more contentious areas, including using food crops to produce biofuels, which I will come to in a moment and which was raised by many, if not all, hon. Members, and the role of genetic modification in meeting yield gaps—especially in challenging natural environments, an issue that was not raised during the debate.
The growing potential of the private sector is recognised and the report reflects on how this sector may become a hugely more significant player in securing food security goals, particularly by working more closely with smallholder and commercial farmers, which is an area of great expansion in DFID’s work. We recognise that food security is as much about the quality of food as having enough to eat. Stunting is a critical issue to address because it is the future of the nation. If 20% of children in a country, or even up to 50%, are stunted, the future of that country is in jeopardy because it cannot achieve the necessary skills base.
The UK is scaling up nutrition programmes in more than 10 countries. In Bangladesh, for example, my Department is integrating the delivery of vitamins, minerals and other nutritional support into three existing programmes that tackle extreme poverty. Those interventions will reach 243,000 adolescent girls, 103,500 pregnant women and 225,000 children under five.
I was marginally upset that no one referred to the Nutrition for Growth event, which was a great step forward and indicated our seriousness about tackling nutrition and global food security. Food alone is not enough to ensure the future of nations. At the event, DFID gave a commitment to triple investment in nutrition-specific programmes between 2013 and 2020, which will reduce stunting by 20 million by 2020 and save the lives of at least 1.7 million children.
On emergency assistance, DFID is not abandoning commitments to continue to provide assistance to the most vulnerable and impoverished countries, including those affected by recent crises such as Typhoon Haiyan in the Philippines and victims of the ongoing conflict in Syria. These responses will continue to include emergency assistance that may, when necessary, include direct provision of food aid and, when appropriate, cash transfers rather than food aid to allow disaster victims to purchase food when food availability is not the problem and available cash is the bigger issue. Access to food is an issue and when it is available locally, it is much better to enable people to purchase the food rather than simply giving it to them.
In some areas, progress has been more difficult. The Government have repeatedly stated that in relation to investment in biofuels in developing countries, food production must always take precedence over the production of energy from food crops. However, we are legally bound under the EU renewable energy directive to our commitment to source 15% of our overall energy, and 10% of the energy used in transport, from renewable sources by 2020.
As many hon. Members mentioned, the UK is, thankfully, the most progressive EU member state in addressing the developmental and food security impact of biofuel development. We actively lobby in Europe to minimise that impact. However, it is recognised that many member states do not see eye to eye with us on this issue. Securing strong political alliances with like-minded EU Governments is essential. The UK’s present position is not shared by a majority of states and we continue to make our case forcefully. Balancing legitimate business and investment concerns against the impact on the food security of some of the poorest people in the world is essential.
My hon. Friend the Member for Mid Derbyshire (Pauline Latham) said she was confident that we could make progress on biofuels, but I do not totally share her confidence. EU members are not in line on this because there is a conflict between two goods. The hon. Member for Edinburgh North and Leith (Mark Lazarowicz) referred to the EU renewable energy directive versus the use of land and inappropriate production of biofuels that could impact on global food security.
Perhaps my right hon. Friend will wait a moment. My view is that we must start to think about 2020, which will be the end of the current target period to which we have signed up and which we cannot get out of. We must negotiate so that the onus is not on us and we can talk about fuel from waste and not from land that could be used for growing food.
All the comments from all hon. Members are important. There is an issue and we must drive harder at it. We will continue to press the EU, but we cannot control the issue so I want to lay plans in advance, if there is no change up to 2020, so that we are ready then to force through a change.
I thank my hon. Friend for explaining the position. It is as well to be up front and honest. The problem is not the Government’s position, but our partners’. However, too often the EU and sometimes our own Government do not look at the joined-up impact of some policies. The challenge is that if the EU really does care about poverty in sub-Saharan Africa, it should be prepared to re-examine its own policies and not put its commercial interests at the top of the list. The same applies to CAP reform. The Minister would have the support of my Committee if she argued that case energetically and tried to win support, but I accept that she is in a difficult position.
I thank my right hon. Friend. I could not agree more. I am simply being open and straightforward about the challenge that lies ahead. I am not saying that we will not tackle it and strive with European colleagues to change it before 2020, but I do not want to get to 2020 without having put in the work to ensure that if that is the point at which we have the opportunity to change, we have made enough allowances to make that change. It is the fall-back position.
One area where perhaps the IDC report did not give a sufficiently strong emphasis is one that is close my heart: the status and economic empowerment of women and girls. Women and girls benefit most from efforts to strengthen people’s food and nutrition security and to make them resilient to stresses and shocks. DFID recognises that as a high priority and is committing more time and resources to working with corporations and Governments globally to ensure that women and girls equally benefit from new investment opportunities in agriculture, as entrepreneurs and at a household level.
For example, the new DFID-supported Propcom Mai-karfi programme aims to raise incomes by up to 50% for more than half a million people in northern Nigeria, half of whom are women. That speaks to something else that Members raised, which was the improved productivity from agriculture. DFID puts an enormous amount of energy into that. I think we call it “stepping up”, so that everyone improves their income and their productivity through their actions.
I have a brief question on incomes. The Minister did not mention anything about systems of social protection. Would she like to do so?
I am coming to that. I have a whole list of points to get through. My right hon. Friend the Member for Gordon (Sir Malcolm Bruce) asked why it was that only 14 countries are in the programmes when there are 29 DFID countries. I hope to get to that.
The Government believe that functioning markets are a better way to manage food stocks than Government interventions. The hon. Member for Stafford (Jeremy Lefroy) made a number of points that went above and beyond the recommendations made in the report. One idea he mentioned was the World Food Programme holding universal stocks to improve availability. He also mentioned involving neighbouring countries and so on. As he said, those ideas go beyond the report’s recommendations. The evidence we have is that universal stocks are not the most effective use of money.
I will not return to the cap as an example, because it is clear that Members did not favour that view, but Malawi, for example, has recently had food shortages. They hold stocks, but when push came to shove and they looked at their grain stock reserve, it was not as high as they thought. Much of it had disappeared. There are a number of issues outside of simply whether stocks are held for emergencies. We do not have the evidence to say that that proposal is an effective use of money, but my experience is that a whole range of unintended consequences come from stockholding.
I fully understand what the Minister is saying and agree that this is not an easy area. We received evidence from the deputy director-general of the Food and Agriculture Organisation. He said:
“The experience of the price spike and the impact of that in various countries related in large measure, in India and China in particular, to the ability to cushion the impact on their population by providing access to food at a lower price because of their grain reserve policies. Certainly in the 2008–09 experience, the impact on poor consumers in India and China was much less than it was in countries in Africa, for example, without the same capacity to do that.”
That is the basis of what we were saying.
I understand that there are differing views, but DFID does not have sufficient evidence and the evidence that we do have shows that attempts by Governments to manage price levels through public stockholding have not been effective in achieving food security objectives. For the moment, we will have to differ on this issue.
The issue of targets for food waste was mentioned. Our experience shows that the voluntary approach is effective and has allowed businesses to reduce waste and become more efficient. The hon. Member for Wirral South asked at what level we would change that. The Department for Environment, Food and Rural Affairs has worked successfully with industry on a voluntary basis through the Courtauld commitment to reduce supply chain food and packaging waste by 7.4% over the past three years. Household waste is down by even more: 15% since 2007. Our approach is having an effect and there is not an ultimate target where we will suddenly change horses. We agree that waste is a big issue and we are working through these voluntary mechanisms, which appear to be working.
If the hon. Gentleman will forgive me, I will not, because I want to give a couple of minutes to the Select Committee Chair at the end and I have a huge number of points to get through.
My right hon. Friend the Member for Gordon asked about the scaling up of safety nets, as did the hon. Member for Wirral South. DFID is more than doubling the number of countries where it supports social protection programmes. We had seven countries in 2009 and we will reach 15 in 2014. It may be that that support is the answer everywhere, but with the best will in the world we cannot scale it up on our own without the mother countries agreeing with us, and not just in policy terms. Even with 0.7% of GDP spent on aid, we do not have infinite funds to do everything in every country without research and without working with mother country Governments.
We will continue to support such programmes. We think that they are excellent and are demonstrating great benefits. We use evidence of that, where appropriate, in conversation with Governments that are new to the idea of social protection. I have been to some countries that do not want these protection programmes introduced. We disagree with that, but we are not a colonial institution that says, “You must have this.” We try to demonstrate the evidence of how successful and useful the programmes are and how they work in those countries.
No, because I will not get through any of these points if I do.
On the Government’s support for the Fairtrade Foundation, we absolutely recognise the important work that it does to promote smallholder access to global markets. We welcome the attention it has brought to finance for small-scale farmers. The UK provides core funding to the foundation and we look forward to working with them and discussing the Committee’s recommendations.
On meat, the key to a healthy diet is getting the balance right. That means eating a wide variety of foods in the right proportions. Red meat can form part of a healthy diet and is a good source of protein, vitamins and minerals, such as zinc and B vitamins. It is also one of the main sources of vitamin B12.
However, not all meat is good. Some meats are high in fat, especially saturated fat. I think it was the right hon. Member for Gordon who mentioned UK farmers. Encouraging people in the UK to eat less and to eat more healthily would not impact on UK farmers. UK commodity prices follow those in the wider international market, so trade flows would adjust. That, at least, is the evidence we have. The fortunes of UK producers are more dependent on their competitiveness within the wider market.
I am glad that the work that DFID does on land and property rights has been recognised. We have signed a new agreement with Ethiopia to go the same way as we have with Rwanda. We are scaling up our land programmes in at least six other countries and we intend to continue our partnerships.
I make a grateful nod in the direction of the Chair of the Select Committee for his recognition of our work on beekeeping. The “World at One” bumped me on Christmas eve, when I was going to expand on our international work on beekeeping. The weather in Britain took precedence.
I am sorry that I have not spoken to all the points, but, to conclude, my Department is working with international partners to prepare for the next series of international development goals after the millennium development goals. The IDC report helps my Department to remain challenged, focused and a world leader in international development policy and practice. I thank the International Development Committee for its continued engagement with the work of DFID and its insightful and useful observations and recommendations, and I thank all Members here today.

It is a pleasure to serve under your chairmanship, Mr Dobbin. I congratulate the hon. Member for Airdrie and Shotts (Pamela Nash) on securing this important debate so soon after world AIDS day and just after the Global Fund to Fight AIDS, Tuberculosis and Malaria replenishment last week. I congratulate her on the important work that she does as chair of the all-party parliamentary group on HIV and AIDS, and on her powerful contribution to today’s debate, which was truly excellent. All who have contributed are part of the cohort who go out and fight the fight against HIV/AIDS because, as hon. Members have emphasised, it is such an important and ongoing cause.
When I came into post, I made HIV/AIDS one of my top priorities. When I was shadow International Development Minister—the post now occupied by the hon. Member for Wirral South (Alison McGovern)—I went to South Africa with Business Action for Africa, along with a Labour and a Conservative Member of Parliament, to look at AIDS projects. During that visit, we went into the townships around Johannesburg and saw the conditions there. The trip had a profound effect on me. Many hon. Members have raised the phenomenal work done by Nelson Mandela. I was in South Africa at a time when the treatment for HIV/AIDS recommended by the country’s leadership was to take a shower. We can see the effect of Nelson Mandela’s work from the way in which things have changed and the amount of Government-funded work that now takes place.
When I visited South Africa, only the big corporations such as SABMiller and Anglo American provided facilities for their own employees, and they did so to stop them dying, not from pure altruism. Many hon. Members have spoken of the stigma associated with HIV/AIDS. I went into a hospital built by Anglo American where people came forward and declared their HIV-positive status in front of other members of staff. That gave those members of staff, who were afraid of the associated stigma, the courage to declare themselves and ask for testing. That was one of the most moving experiences of my life. I say to all who take MPs on trips to enlarge, inform and develop them that that trip, eight years ago, may have been a reason why I made HIV/AIDS one of my priorities when I came into office. In addition, I grew up in an era when HIV/AIDS first became an issue. Being terrified by the AIDS prevention adverts and having many friends who died of HIV/AIDS long before there was any treatment for it, left its mark on me.
I will address the points that have been raised as I go along, after which I will try to address any that are not in my speech. There is much to celebrate. The latest UNAIDS figures show an unprecedented pace of progress in the global AIDS response. There are 1 million fewer new HIV infections each year across the world than there were a decade ago, especially among newborn children. We do a lot of work on preventing mother-to-child transmission, which is an obvious stop point, and that work is delivering results. Nearly 10 million people now have access to treatment. Although international assistance remained flat, low and middle-income countries increased funding for HIV, accounting for 53% of all HIV-related spending in 2012. That shows that we are moving towards a lasting response.
That is all excellent news, but, as we debated in Washington last week, we need to put renewed efforts into going the extra mile and achieving an AIDS-free generation. We cannot take our foot off the pedal. Risks remain that might seriously jeopardise the incredible progress we have made. Too many people are still getting infected; 2.3 million were infected last year. As many hon. Members have said, girls and women remain disproportionately affected by the virus. Infection rates in young women are twice as high as in young men. Although tremendous progress has been made on treatment scale-up with the change in the World Health Organisation treatment guidelines in 2013, at least 16 million people who are in need of treatment are not currently receiving it. Stigma and discrimination continue to drive key affected populations underground, which inhibits prevention efforts and increases the vulnerability of those populations to HIV. In 60% of countries there are laws, regulations or policies that block effective HIV services for key populations and vulnerable groups. I will return to that point.
The UK Government were delighted and proud to pledge £1 billion of UK funds at the fourth Global Fund to Fight AIDS, Tuberculosis and Malaria replenishment in Washington last week. The UK pledge alone will save a life every three minutes for the next three years, and it will deliver life-saving antiretroviral therapy for 750,000 people living with HIV. The hon. Member for Strangford (Jim Shannon), who is not in his place and has sent his apologies for having to leave, raised the issue of leverage. The UK contribution helped to leverage, and contributed towards, an unprecedented $12 billion replenishment total. That is 30% more than was pledged at the equivalent event in 2010, and 50% of those funds will go towards dealing with HIV and AIDS.
The UK now calls on all outstanding donors to step up to the plate over the period from 2014 to 2016 to ensure that the target figure of $15 billion is reached and there is maximum impact in terms of lives saved. The Secretary of State and I are telephoning other countries to lobby them. The contribution from one country—I believe it was Switzerland, but I will correct the record if I am wrong—tripled after my telephone call. That is the point of the lobbying effort across the world, which will not end with the pledging in Washington. We must continue that effort to ensure that we reach our targets. We are also working with recipient countries to help them realise increased domestic contributions in the fight against the three diseases. We were delighted by the political commitment of recipient countries at Washington and by the financial commitment of Nigeria, which pledged $1 billion to the national fight against the three diseases. The fight is becoming truly global, with equal partnership and purpose.
This year, we conducted an internal review of our 2011 HIV position paper, which we published last month. I thank STOPAIDS for its help; I see Ben Simms wherever I go in the world. Two years on, DFID is making good progress against its expected results. Treatment-related commitments have already been achieved, and the remaining targets set out in the HIV position paper are on track to be met by 2015.
Several hon. Members mentioned the shift in funding from bilateral to multilateral. Over the past two years, we have been sharpening our focus and working more to our comparative advantage in our bilateral programmes. As the 2011 position paper predicted, the balance between multilateral and bilateral funding has shifted and our bilateral efforts are focused on fewer countries where the need is greatest. The hon. Member for Newcastle upon Tyne North (Catherine McKinnell) asked what we were doing in the programmes where we are shifting the balance of our funding. We now have some exciting new programmes in southern Africa, which is the region hardest hit by the epidemic. Given the urgent need to reduce new infections, we have prioritised critical prevention gaps and we are moving towards complementary work to deal with those gaps. As hon. Members have said, civil society has been, and remains, an essential partner for DFID in addressing those gaps. We are proud to support other multilateral organisations, such as UNAIDS, to ramp up their efforts in the global HIV response. That will reach many more countries, at a much greater scale, than the UK alone could help.
As I have announced, we will increase our annual core contribution to UNAIDS by 50% to £15 million in 2013-14 and 2014-15. That will give the organisation an extra £5 million a year to support its critical role in co-ordinating the world response to HIV and AIDS. In total, our combined bilateral and multilateral contributions secure the UK’s place as a leader in the global HIV response and demonstrate our commitment, in providing a considerable share of total global resources, to universal access to HIV prevention, treatment care and support.
The review paper highlighted three areas of particular focus for the UK: being a voice for key affected populations; renewing efforts on reaching women and girls affected by HIV; and integrating the HIV response with wider health system strengthening, which hon. Members raised, and other development priorities. That includes tackling the structural issues driving the epidemic.
I shall refer to human rights, which many hon. Members raised. In countries with generalised epidemics, HIV prevalence is consistently higher among key affected populations: men who have sex with men; sex workers; transgender people; prisoners; and people who inject drugs. Over the years, DFID has spearheaded support to HIV programmes for key populations. They have been and they will remain a key policy priority for us. We will use DFID’s influence with multilaterals to be a voice for key populations and to push for leadership and investment. We will focus on evidence-based combination prevention services, such as condoms, HIV testing and counselling, and comprehensive harm reduction programmes.
Of particular importance are the programmes and initiatives we are supporting to reduce stigma and discrimination. Our ultimate vision for key populations is for their human rights and health to be recognised, respected and responded to by their Governments. The UK is proud to be a founding supporter of the Robert Carr civil society Networks Fund, through which we support those particularly vulnerable groups. Valuable lessons have been learnt from the fund’s first year and this world AIDS day, the fund announced a second round of grants.
Before the Minister moves on from the Robert Carr fund and key populations, will she clarify whether any DFID money will go to grass-roots organisations? As I said earlier, the Robert Carr fund operates regionally and I know that a lot of money goes through multilaterals. It would be good to have some clarification on how we are getting money through to smaller groups.
I will come back to that issue shortly.
Human rights was one of the key issues raised by hon. Members. The UK Government are at the forefront of work to promote human rights around the world. We regularly criticise Governments who violate those rights, including those that discriminate against individuals on the basis of sexuality. I have personally raised those issues with Ministers, Prime Ministers and Presidents in Africa. We take some of our lead in DFID bilateral countries from activist groups in the LGBT community, so that may take place behind closed doors due to the difficult, sensitive and dangerous nature of some of the work they do in countries where the law is such that they may face prosecution and for which they could face a backlash. I am committed to raising such issues with Governments across the world, as is the Foreign Secretary and many others across Government. Human rights is at the forefront of our work.
Women and girls are at the centre of our HIV response. Globally, the rate of new HIV infections among women and girls has declined, but the pace of decline is not as rapid as we would like and it is a critical area for renewed UK and global efforts. Gender equality and women and girls’ empowerment lies at the heart of DFID’s development agenda. Since 2011, each of our bilateral programmes has seen a greater focus on HIV prevention addressing the needs of women and girls. We are supporting research to improve outcomes for women and girls, including the development of female-initiated HIV-prevention technologies, and we are looking into how gender inequality drives epidemics, with a particular focus on improving what works for adolescent girls in southern Africa.
We know that in a crisis, girls and women are more vulnerable to rape and transactional sex. The highest maternal mortality and worst reproductive health is in countries experiencing crisis. Contraception, prevention and treatment of HIV and other sexually transmitted infections, and safe abortion are life-saving services, yet they are often ignored in humanitarian responses. That is why DFID is currently developing a new programme on sexual and reproductive health in emergency response and recovery, including services to reduce the transmission of HIV. We welcome the fact that the global health fund will also prioritise women and girls more in 2014 and we look forward to working closely with it on that.
In terms of integration with the wider health system, we know that for a response to be lasting, we must integrate HIV within other sectors and find concrete solutions to sustainable financing. We recognise that a strong health system is an important way to improve the reach, efficiency and resilience of services. The co-infection connection and the integration of HIV services with TB services, sexual and reproductive health services and the wider health system were raised. People living with and affected by HIV, including children and people with disabilities, need to be treated holistically and not just as a series of health problems.
We are also working with countries to ensure that they are in the lead role and increasingly financing their own national responses. In the end, that is the only way to sustainability. We are also working with the global health fund and others to look at market shaping. The hon. Member for Newcastle upon Tyne North mentioned tiered pricing—we term it market shaping—as a way of further reducing commodity prices not only for low-income countries, but for middle-income countries graduating from donor support, which many hon. Members mentioned.
I have tried to cover most of the points raised, but I have left a few things out. Integrated responses to tackling TB-HIV co-infection were highlighted in the HIV position paper review as a key area of current and ongoing effort. It will contribute to the global results to help halve TB-related deaths among people living with HIV by 2015. A cross-Whitehall group on harm reduction was called for. The UK Government remain committed to supporting harm reduction efforts to ensure that that goal gets back on track. DFID is currently liaising with other Whitehall Departments on the drafting of the Commission on Narcotic Drugs ministerial statement, and will remain engaged on that crucial issue in the lead-up to the UN special session on drug control in 2016.
Hon. Members mentioned access to medicines, which is vital. The access to medicines index, last published in November 2012 and supported by DFID, shows that companies have their own strategies for managing their intellectual property and supporting access to medicines. The medicines patent pool currently has agreements with the US National Institutes of Health, Gilead Sciences, ViiV Healthcare and Roche. The UK will continue to support actively that collaborative initiative to enhance access to more affordable treatment and to promote the development of appropriate treatment for children. The UK strongly encourages other companies that have patents for the new first-line treatment for HIV to consider beginning formal negotiations to enter the pool. The medicines patent pool idea was endorsed by the G8 and the UN General Assembly session on HIV and AIDS, to support the availability and development of new first-line treatments for HIV and AIDS.
In addition to funding for antiretroviral drugs through the global health fund, UNITAID and other agencies, DFID also works to make markets for antiretrovirals work better to reduce prices, increase the number of quality suppliers and enhance access. Our partnership with the Clinton Health Access Initiative has already contributed to secure price reductions of almost 50% on both first and second-line therapies for HIV, saving African Governments more than £500 million. That is sufficient to put an extra 500,000 people on AIDS treatment for three years. As has been said, that fall in price from $100,000 per treatment to $100 is the most incredible result. We need to keep pushing down those prices for as long as we can. In terms of civil society, we continue to provide funding for work at the grass roots through our civil society programme partnership arrangements and other DFID civil society grant awarding schemes.
I have only one minute, so I will reply to hon. Members by letter if I have missed any points. The UK and others made huge contributions last week in Washington. There is a great sense of excitement and common purpose in the world, leading towards the vision we all hope for—an AIDS-free generation—an historic moment. A sad truth of the HIV epidemic is that it is often women and girls who are most at risk of human rights abuses in developing countries and least able to get access to the services they need. Addressing gender inequality, stigma, discrimination and legal barriers remains our priority.

Commons Chamber7. What progress has been made on the most recent replenishment round for the Global Fund to Fight AIDS, Tuberculosis and Malaria.
I am pleased to say donors have pledged $12 billion, which is an impressive 30% increase on the amount that was pledged at the 2010 replenishment conference, demonstrating global confidence in the fund. The global fund provides excellent value for money and delivers life-saving results on a global scale.
The significant increase in DFID’s contribution to the global fund to £1 billion will contribute to the scaling up of proven TB REACH programmes that are included in the national strategic planning process. We have reviewed the mid-term evaluation of TB REACH, which shows that it is effective and that it reaches very important populations. However, given that there are so many small projects, there are concerns about sustainability and about the ability to scale up. We will obviously keep that in mind.
If left untreated, tuberculosis kills 50% of those with an active infection. Will the Minister ensure that as much funding as possible goes to the African and Asian countries where up to 80% of the population carry the latent tuberculin bacteria?
Yes, we are very keen to help the countries that have such a high burden. We are encouraging the global fund to change its remit to give more than 10% of the support to Nigeria. Interestingly, Nigeria pledged $1 billion to the global fund yesterday at the pledging conference. That is a tremendous move forward for that country.

Written StatementsI am pleased to take this opportunity to update the House on my role as Ministerial champion for tackling violence against women and girls overseas.
Violence against women and girls is a global pandemic—it is one of the most systematic, widespread human rights abuses in the world. The UK Government believe this can change, that every woman and girl has the right to live free from violence and abuse.
The establishment of the role of Ministerial champion for tackling violence against women and girls overseas is the first of its kind internationally. I was appointed to this role by the Prime Minister in 2010 and for the last three years I have had the privilege of being at the forefront of UK efforts to successfully drive forward progress towards eliminating violence against women and girls internationally.
In my role I am also working to improve policy coherence across UK Government Departments, ensuring that what we do has the greatest possible impact overseas. I have also championed this issue with the media, civil society and with parliamentarians in the countries in which we work. As a result we are now seeing an unprecedented level of commitment to this issue that I would like to bring to the House’s attention.
As champion, I am passionately committed to tackle all forms of violence against women and girls but have identified three priority areas:
Addressing violence against women and girls in humanitarian emergencies—because women and girls are often the most vulnerable in emergencies and in times of conflict;
Ending female genital mutilation/cutting—as this is one the worst kinds of gender violence, where girls around the world have suffered a lifetime of damage, sometimes even death, as a result;
Building the evidence base—so we better understand what works in preventing violence against women and girls.
During my time as champion, the UK Government have shown considerable leadership in this area at all levels. This includes the recognition of violence against women and girls in the Prime Minister’s “Golden Thread” agenda for International Development and his push for its inclusion in the framework that will replace the millennium development goals; the Foreign Secretary’s commitment to addressing sexual violence in conflict through the preventing sexual violence initiative; and DFID’s scaling up of its response to violence against women and girls, including more in-country programmes on preventing violence, support to building the evidence-base on prevention including launching a £25 million research programme, and high-level leadership on addressing violence against women and girls in humanitarian responses.
I am championing our work on female genital mutilation/cutting (FGM/C), of which I am very proud—demonstrating UK leadership in backing an Africa-led movement on this issue. In March 2013 I announced DFID’s flagship £35 million programme which will help to end this practice within a generation with a specific objective of reducing it by 30% in at least 10 countries in five years. There are clearly links between the diaspora and what is happening within their countries of origin. Within our FGM/C programme, we will be working with diaspora communities with the specific aim of supporting ways for them to influence change in their countries of origin.
Important steps have also been taken this year at the international policy level in which the UK has played a key role. This includes the agreed conclusions reached at the 2013 UN Commission on the Status of Women which gives the international community a strong mandate to take a stand on ending violence against women and girls.
We have also seen the Foreign Secretary deliver the preventing sexual violence initiative, ensuring our efforts across Government are fully complementary. So far, the initiative led to the historic G8 declaration, and in September 2013 the ambitious declaration of commitment to end sexual violence in conflict, which, so far, 134 countries have endorsed.
Looking forward, building on the G8 commitments, on 13 November 2013, the Secretary of State for International Development will host a high level call to action on protecting girls and women in emergencies, to be held in London. This event will shine a spotlight on the actions the international community will take to better protect women and girls in emergencies.
We are also developing a new UK national action plan on women, peace and security, to be launched in March 2014. This will ensure that the UK’s work in conflict resolution and peace-building takes into account the impact of conflict on women as well as the important role they play in building a lasting peace.
Alongside the International Development Secretary, I will continue to push for the inclusion of a stand-alone goal on gender equality, which includes a target on ending violence against women and girls, in the framework that will replace the millennium development goals after 2015.
As recommended by the International Development Committee in their recent report on violence against women and girls, I am working to ensure that we continue to scale up the development and implementation of programmes to tackle violence against women and girls in all DFID priority countries.

Commons Chamber3. What support her Department provides for the Global Fund to Fight AIDS, Tuberculosis and Malaria; and if she will make a statement.
The UK provides support to the fund, both financially and through membership of its governing board. We are the fund’s third-largest donor. Through DFID country offices, we provide a range of complementary funding and support to national health plans and global fund-supported programmes.
I thank the Minister for that answer. What steps is the UK now taking to galvanise support for the global fund from other donors?
We are using our influence with all other donors to ensure that they step up to the mark in the autumn replenishment, as we intend to do.
What discussions has my hon. Friend had with the Department of Health to ensure that the right sort of drugs, particularly antibiotics, are prescribed in these development areas?
I personally have not had such discussions, but we nevertheless work closely with the Department of Health and drug companies to ensure that the right drugs get to the right people for the right diseases in the right places.
Ministers have consistently suggested that the UK would be willing to consider doubling its contribution to the global health fund. In view of the fact that the fund has made major changes and is under new leadership, will the Minister advise the Secretary of State to stop dithering and confirm the UK’s increased contribution before the summer recess? That would incentivise other countries to step up to the plate and ensure that not one more day is wasted in the fight to defeat AIDS, TB and malaria. Will the Government please get on with this?
What we are doing is the absolute opposite of dithering. We have stepped up to the mark: we are providing £1 billion as promised and ahead of schedule. The hon. Gentleman is right inasmuch as the global health fund has made serious moves towards reform and has overhauled its strategy and governance. We want to look at it strategically, and we need to look at the “mini-MAR”—multilateral aid review—the International Development Committee response, the National Audit Office report and the HIV provision paper. I am sure that the hon. Gentleman would not want us to spend money inappropriately. We will lead, we are leading and we intend to carry on leading.
Stop TB UK described the Government’s response on malaria as a model aid agency response, but it is worried that TB is a poor relation of the three diseases. It hits the poorest hardest, but interventions to stop TB are very cost-effective. Will the Minister meet Stop TB UK to discuss its concerns?
My right hon. Friend the Secretary of State has met Stop TB UK, and I have just returned from South Africa, where I convened a round table on TB, particularly on the theme of TB and HIV in the mines. This is such an important issue that we want to move forward on it. Spending money to stop TB in other countries helps us to stop TB in this country.
6. What assessment she has made of likely population growth in north and west Africa by 2050; and if she will make a statement.
The United Nations released revised population projections on 13 June. The population in north Africa is projected to increase from 200 million in 2010 to 319 million in 2050, while in west Africa the projected rise is from 305 million in 2010 to 815 million in 2050.
The region is already experiencing substantial instability and extremism, and the likely outcome is that millions of young men and women will have bleak economic prospects. Given that no country has emerged from poverty without first addressing its levels of population growth, will the Minister give the region priority in her population programmes?
My hon. Friend has made an important point. West Africa has particularly high rates of population growth, and there is much less take-up of family planning there. DFID’s work involves not only family planning—which is a complex issue—but the delaying of first pregnancies, access to economic assets for girls, getting girls through secondary school and preventing violence, all of which contribute to making the population richer and more successful.
11. The international remittance trade is worth $500 billion a year. How will DFID support United Kingdom links with countries in north and west Africa where many people depend on remittances?
Remittances are indeed very important. My right hon. Friend the Secretary of State has met the head of Barclays to discuss the issue, with the aim of ensuring that remittances can be sent back to a country when they have the potential to increase that country’s GDP.
7. What steps her Department is taking to end aid dependency through jobs and economic development.
T2. HIV/AIDS is a devastating illness affecting 34 million people worldwide, 69% of whom are in sub-Saharan Africa. This week, the White House published its HIV/AIDS strategy, so when will the Government commit to publishing one for the UK?
We are in the middle of reviewing our HIV position paper. I have just returned from a round table meeting in South Africa that examined this issue. It is an important issue and we are on it.

It is a pleasure to serve under your chairmanship, Mr Havard.
I once again thank the International Development Committee for its report, to which the Government replied on 14 March. I congratulate the Committee on securing a debate on this important topic, and my right hon. Friend the Member for Gordon (Sir Malcolm Bruce) on an excellent pre-emptor to the high-level panel discussions. We have heard many excellent contributions from Members this afternoon, and I will try to get through as many points as I can, but my time is somewhat limited.
Since the International Development Committee’s report and the Government’s response, we have seen the high-level panel’s report on post-2015 development. I hope that all those who have seen the report will join me in saying that the Prime Minister and the panel have set the bar high for the next two years of discussion. They have laid out a truly ambitious vision for eradicating extreme poverty within a generation, tackling the difficult but necessary issues head on.
I want to take a moment to talk about the high-level panel’s report, because I believe, and the Government believe, that the vision it sets out for what the new agenda might look like marks a step change. I hope that the International Development Committee is pleased that many of its recommendations are reflected in the report.
The five transformative shifts that drive the new development framework are key. First and foremost is the commitment to leave no one behind, which goes to the heart of the issue that many Members raised, and to keep faith with the original promise of the millennium development goals and finish the job by eradicating extreme poverty in a generation.
A number of Members raised issues of equity and equality, and there is a commitment to ensuring that every single goal is achieved for everyone, in every social and income group, regardless of gender, ethnicity, disability, where someone lives, what religion they practise, or whether they are in extreme poverty. That is a radical departure from the previous MDGs, and is the shift that will make the most significant difference, because countries will not score unless they have done hard work for the hardest to reach, the most marginalised, the most difficult and the poorest extremes.
Secondly, the panel called for sustainable development to be at the core of the new framework. Several Members have raised the issue of integrating the millennium development goals with the sustainable development goals. Although the streams are separate—the open working group is working on the sustainable development goals—that second transformative shift will put sustainable development clearly at the core of the new framework.
That recognises the fundamental link between the environmental, social and economic pillars of sustainable development. For example, we can deliver food security for all only if there is an efficient and sustainable use of natural resources. It is absolutely clear that we need to bring together the review of the millennium development goals and the Rio+20 follow-up to deliver a single development framework, as the Committee recommended in its report.
Thirdly, we need to transform economies to provide jobs and inclusive growth, which was mentioned by several hon. Members. Clearly, we need to lift people out of poverty through economic development and growth, to unleash the dynamism that gives everyone economic opportunities, and to harness investment and the private sector as the drivers of development. As has been said, we need inclusive and pro-poor growth, and it is important that those things are interwoven. The hon. Member for Bethnal Green and Bow (Rushanara Ali) raised some concerns about the involvement of organisations and companies, such as CDC or ones in the private sector. It is vital that all who work in or profit from such organisations, share those profits and ensure that everyone benefits from such growth.
Fourthly, although this has been less mentioned in the debate, we need to build peace and to build effective, open and accountable institutions for all. As the Prime Minister has said,
“Freedom from fear, conflict and violence is the most fundamental human right”.
Human rights have been mentioned in the debate. Without accountable Governments, safety, freedom of speech, free political choice, the rule of law and all the elements of good governance and peace, how can we eradicate poverty?
We often forget that freedom of belief—freedom of thought and belief, to hold a religion or not to hold any religion—goes alongside freedom of speech. That is not always remembered, but it should be.
Well, it is remembered by the Government. We hold that dear, and we work closely not just with people in terms of respecting their religions in their countries, but with our own faith groups and faith NGOs throughout the world. We cannot really do development, if we do not work in partnership with the faiths of the countries in which we work. That is the only way forward.
The issues of women, and of women in conflict, have been raised. In relation to providing peace and stability, DFID—the UK Government—have put women and girls right at the heart of all our development work. With respect to lifting families out of poverty, if a woman is empowered with education, has children later, has some power over her own life and has economic empowerment, her children and the community will be better off. As the international champion in the fight against violence against women and girls, our fight is obviously against violence against women—how can we have development when half the population basically cannot go outside their own door?—but there are also campaigns on female genital mutilation, which is a symbol of women’s oppression.
Finally, the panel has called for a new global partnership and has set out the principles of that new partnership and the spirit of co-operation needed between Governments, civil society, businesses, international agencies and people living in poverty themselves to make the post-2015 vision a reality. We have all learned that there is no one answer and that no one body or person can deliver across all the areas that are needed in this world, which we will achieve only through genuine and sincere partnership.
One big jump made by the high-level panel report is its use of illustrative examples of how the transformative shifts could be made into goals themselves. I am obviously biased: I am very keen on the stand-alone gender goal, which I think is imperative. However, there could be goals on poverty, hunger, education, equality, jobs, economic growth, good governance, peace and stability. Hon. Members have spoken compellingly about the importance of all those issues, and I hope they are happy with the concrete, measurable and compelling goals and targets that have been suggested.
The report was a remarkable piece of work. I did not expect it to be as good and succinct as it is. This is the beginning of the process, and the next two years will demand a huge amount of work if we are to bring that seminal piece of work to a concrete conclusion that we can all deliver. As has been said, the early MDGs were phenomenal drivers for good, but they did not always achieve what they set out to do. Like other Members, I have visited schools, including one in Zambia that has 100% attendance, but a 96% failure rate. We have learned from the first MDGs, so I am very hopeful that we will do better with the post-2015 ones.
I want to highlight three of the Committee’s recommendations that are particularly important. The first is on the rights of women, which I have already touched on. The more times that that can be raised by more bodies, the more capital it will gain until we reach universal agreement that part of, if not all, the answer is the empowerment of women. The second recommendation is that the post-2015 development agenda reflects the needs of the poorest, about which I could not agree more. As has been said, we need to listen to the voices of the poorest, and that is what the high-level panel did. For the first time, from surveys such as the My World survey, and mobiles, the internet and old-fashioned clipboards and pencils, schoolgirls in Rwanda and urban workers in Brazil have all been heard. The third recommendation highlighted the importance of keeping up the hard work, and I absolutely concur with it.

I want to touch on some of the points raised in the debate. I absolutely agree with my right hon. Friend the Member for Gordon that we should take the MDGs as a starting point for the post-2015 goals. He highlighted the important lesson that halving poverty sometimes has the perverse incentive whereby we do not try to reach the very poorest. It is because that is important that the high-level panel has called for disaggregated data for all groups to ensure that the most vulnerable people are not left behind.
My right hon. Friend raised the issue of fragile states. He rightly identified the real issue that countries emerging from, or still in, conflict can be left behind in relation to development. The high-level panel report recognised that conflict plays a critical role in relation to security. It has addressed that through the stand-alone illustrative goal of ensuring stable and peaceful societies, for which targets in the framework include those on violent deaths, access to justice and the behaviour of security forces. The Government will work hard to ensure that that important recommendation is reflected in the final framework.
Several hon. Members raised the issue of young people and mentioned the burgeoning number of young people in some countries. The panel has called for a jobs target with a specific indicator for youth employment.
Secondary and tertiary education has also been raised. We have found from the evidence that the most benefit for economic development comes through primary and lower secondary education, but as countries develop, people need to stay in secondary and tertiary education and, even more importantly, to have jobs at the end, so that those who have been through tertiary education are not left with nothing and with nowhere to go, except to leave those countries that so greatly need them.
Everyone has praised the 0.7% level, about which there is cross-party consensus. My hon. Friend the Member for Tewkesbury (Mr Robertson) spoke about his experience in Ethiopia. I have felt the same when I have been there. Each time I go, I see that it has opened itself to the world a little more. I also admire its control over its own development, because it has its own best interests at heart. As I have said, growth should be inclusive and pro-poor.
I could not agree more with my hon. Friend the Member for Stafford (Jeremy Lefroy) when he spoke about strengthening health systems. In the nine months that I have been in post, I have found that if Government public health systems are not there and everyone—whether an NGO, a non-state actor or whoever—does their separate bit, however well-intentioned, it is very piecemeal. It is only with the stability of a national health system, as it were, that services can be combined, as I have seen in the very poor state of Marsabit, where the Government of Kenya have done so in relation to nutrition programmes, vaccination programmes, transition of HIV, and so on.
I am running out of time, but I want to thank my hon. Friend for his kind words about DFID’s work on titles in Rwanda. Land ownership and land titling is hugely important.
Lastly, let me reassure the House that the Government’s commitment to this vital agenda will go on. The agenda, which will shape the UK’s work on development in the coming decades, will continue over the next two years of discussions and negotiations. Thank you, Mr Gray, for the opportunity to speak on this important topic. I thank all Members who have spoken.

What a pleasure it is to serve under your chairmanship, Mr Caton. I congratulate the hon. Member for Poplar and Limehouse (Jim Fitzpatrick) on securing this important and timely debate, and I thank him for having done so. I will try to get through all the points that have been raised, but if I do not we will contact hon. Members afterwards.
Tuberculosis is an age-old disease. It is tenacious and persistent, and affects the poorest people in the world and those who are socially marginalised. Every year there are 9 million new cases and nearly 1.4 million deaths. Although its incidence has been declining slowly since a peak in 2004, and mortality rates have fallen by 41% since 1990, the vast majority of TB deaths—more than 95%—are in the developing world.
Despite some progress, there were 400,000 cases of multi-drug resistant TB in 2011. As honourable colleagues will be aware, MDR-TB is more difficult and more expensive to treat than TB. Its spread is threatening the global response to TB, and makes TB control even more difficult. It is true, therefore, that TB continues to affect the poorest people in the poorest countries, and remains a serious threat to global health, especially through the rise of MDR-TB.
The coalition Government share the concerns about drug resistance, and we remain committed to the global goal of halving deaths from TB by 2015. The emergence of drug-resistant strains of tuberculosis poses a serious threat to the achievement of that goal and, indeed, to the effectiveness of our current armoury of medicines and treatments.
Our priorities for TB, and for MDR-TB, are to help to increase access to effective diagnosis and treatment of TB; to invest in research and product development in more effective treatment, diagnostics and vaccines; to support countries to strengthen health systems to deliver quality TB programmes—a really important point—and to work with our partners to tackle the risk factors for TB, including poverty and malnutrition. That is not always highlighted, and most of the work of the Department for International Development focuses on dealing with poverty and malnutrition.
As highlighted by the hon. Member for Poplar and Limehouse, Public Health England is developing a national strategy for TB, and engaging with key partners such as local government, the National Institute for Health and Care Excellence, NHS England, academia, the voluntary sector and the Department of Health. DFID will obviously input into the process, and will work with the partners on their strategy, to produce national and international policy and to ensure that there is co-ordinated action on domestic and global approaches to reducing rates of TB.
Our first priority is to improve basic TB control. Basic control includes early detection and diagnosis, effective and complete treatment, and contact tracing. Basic control is also critical in preventing the further spread of drug-resistant tuberculosis. If we do not deal with basic TB, the incidence of MDR-TB will be accelerated. We also help to strengthen all aspects of TB control through direct and indirect funding channels in a range of high-burden countries.
I will quickly give three examples. We are working with the Government of South Africa to expand the quality of and access to public sector services, including that of TB control, and are increasing the speed with which new TB drugs get registered. We have engaged in a new partnership with the private sector in South Africa and the World Bank that aims to reduce TB in mining communities, which I think will be welcomed on both sides of the House.
In India, DFID is working with Indian pharmaceutical manufacturers to improve the price and security of supply of high-quality drugs for resistant TB and the manufacture of new low-cost diagnostic products. In Burma, we are providing bilateral funding to the 3MDG fund, a multi-donor fund for the health sector, which is supporting disease control among the poorest communities.
I, too, am a member of the all-party group on global tuberculosis, and I visited South Africa recently with Lord Fowler. Is that country not a good example of the problem of drug-resistant TB? A full third of the budget that South Africa has to deploy in dealing with TB is spent on drug-resistant TB, yet the incidence of such TB is only 2%. That underlines the importance of getting on top of that form of TB so that the costs do not run further out of control and undermine the fight against the disease.
My right hon. Friend makes an excellent point. South Africa is an epicentre, so far as its spend on what is a relatively confined industry is concerned.
I was talking about Burma. It is estimated that between 2013 and 2016, the 3MDG fund will spend $20 million on tuberculosis. Funding is an important strand. DFID also supports a number of global partnerships that work on strengthening basic TB control. For example, the Stop TB Partnership plays a critical role in helping countries to strengthen their TB policies, and in supporting the improvement of funding applications for large TB-control grants.
The UK’s contribution to UNITAID, of up to €60 million per year, has funded new laboratory infrastructure in 18 countries, 10 of which now routinely diagnose MDR-TB. The network will have detected approximately 12,000 MDR-TB cases by the end of 2011, compared with only 2,300 cases in the same countries in 2008.
I will move on to the Global Fund to Fight AIDS, Tuberculosis and Malaria, because I know it is of particular interest—this is not the first occasion on which it has been raised with me. The majority of UK funding to global TB control is channelled through the Global Fund to Fight AIDS, Tuberculosis and Malaria, and we have increased and accelerated our funding and are on track to meet our £1 billion commitment to the fund for 2008 to 2015. The fund is, as hon. Members have mentioned, absolutely critical to achieving many of the UK’s health-related international development objectives, so it is important to us that it continue to deliver ever-more impressive results. The UK intends to increase its contribution, pending, as we have said, progress on the implementation of crucial reforms. That obviously falls within my portfolio, and I have had reports from all DFID offices around the world, having asked them to report to me on the fund. Recently I was in Nigeria and had a meeting with recipients of global funding from across the three diseases, to understand the changes that are being heralded in with the reforms at the global fund—so far so good.
We are committed to working with others to ensure that the planned autumn replenishment is a success. We are a world leader, but sometimes it would be nice to be at least equalled in some of these things by other donor countries. We will use our influence to draw in more overall financing. I understand the call to go early, but there are many multinational decisions to be made and, as I have said, this all depends on progress.
On investment in research and innovation, which I think all Members would agree is critical, DFID has a strong record of supporting research and development for effective treatments, diagnostics and vaccines. An example of that is our effort to increase the affordability of diagnostic testing for MDR-TB. DFID’s support of the Foundation for Innovative New Diagnostics has contributed to the development of a rapid molecular test, GeneXpert, which has the potential substantially to improve the diagnosis of TB and drug-resistant TB.
DFID aims to continue our strong record of supporting investment in TB research and development, including through product development partnerships, and we will strive for value for money in such investments. On DFID’s support for innovation, we will consider the hon. Gentleman’s request that we fund TB REACH against, obviously, the competing priorities and commitments in our international health financing decisions.
Will the Minister recognise the importance of TB REACH? We can have all the drugs in the world, but if we cannot find the people with TB, we cannot use those drugs.
Absolutely. The point is that we are waiting for the evaluation. TB REACH worked by giving a small amount to a great number of organisations to test how to reach people in difficult circumstances. It had precise pre-specified targets and cost-effectiveness benchmarks, and we have to await the evaluation of that first phase to assess what our funding might be for the second phase. We cannot go ahead of that, although I understand that reaching people is critical. We should also work to strengthen health systems, because ultimately we want health systems that are able to reach every individual in a country and dispense whatever medical care is necessary, but I understand the point in relation to TB.
On Ethiopia, about which I have not yet responded, DFID provides significant support to its health system, directly supporting community health workers, and we agree that they do a great job, including on TB. I have been to Ethiopia myself—twice, in fact.
In conclusion, I am very proud to serve in the coalition Government who, even in tough times, have protected the development budget and will reach the target of 0.7% of gross national income this year. I am also proud that we have cross-party consensus in this Parliament: it is one of our finer moments. We are equally clear about the responsibilities that come with those resources, particularly when this country is itself struggling for survival. Those responsibilities are to spend taxpayers’ money well, to deliver aid that is accounted for transparently, and to ensure that our support delivers value for money and gets to where it is most needed.
Significant progress has been made in controlling TB since 1995, with more than 51 million cases treated and 20 million lives saved. That progress was rooted in improved partnership, policy, innovation and leadership, so there is cause for optimism. I thank all hon. Members here, because the issue is really important and I appreciate their continued pressure. The issue needs to be worked on in all the ways they have proposed if we are to get the better of this disease: our progress is good, but not remarkable. The UK is playing its part, but as I have said, we are all clear that significant challenges remain.

Commons Chamber2. What support her Department provides to WaterAid.
The total funding provided by the Department for International Development to WaterAid for the financial year 2011-2012 was £5.8 million. The figure for the funding allocated to WaterAid in 2012-13 will be available at the end of this financial year.
I thank my hon. Friend for that answer. Will she join me in preparing for world water day on 22 March? Will she also recognise the massive contributions from individuals and others, including water companies, who contribute to WaterAid, and from projects such as EcoLink, applied by Nestlé in South Africa, as that all benefits developing countries?
I thank my hon. Friend. I know of her interest in water as chair of the all-party parliamentary water group and I congratulate all those who make a contribution on the key issue of water in developing countries. My hon. Friend mentioned world water day. The Department for International Development will host events on that day, particularly on how water impacts on girls and women.
It is good that there has been progress on access to safe water and sanitation, but there has been much less progress in much of Africa, in both urban and rural areas. What are the Government doing to address that inequality?
The Government are taking a great many measures on water, sanitation and hygiene—WASH. We have so far enabled 1.9 million to gain access to clean drinking water and 2 million to gain access to improved sanitation, and 6.6 million have been reached through DFID support for hygiene promotion. We know more has to be done, particularly in urban areas as those areas increase.
I welcome the Minister’s positive comments on WaterAid and her commitments in respect of world water day, but does she recognise that, currently, UN statistics on the millennium development goals measure only who has improved water, not how many people actually have safe and sufficient water? Will she ensure that a more robust standard is used and is at the centre of DFID’s work?
I will certainly take up the hon. Gentleman’s point on how we measure such things, but the Government have doubled their commitment to reaching 60 million people with WASH funding. We are looking to scale up WASH, because we simply are not reaching enough people at the moment and the millennium goal is off track.

4. What discussions she has had with her counterparts in the UN on reinstating bilateral aid to Mali.
The UK Government do not have a bilateral aid programme in Mali, but we are the second-largest humanitarian donor, providing £13 million in 2013. We have had discussions with our EU and multilateral partners, including the UN, on the importance of co-ordinated resumption of aid.
Bilateral aid from the EU and the World Bank provides a key step in protecting people in Mali from drought and violence. What steps have the Government taken to ensure that that aid is reinstated as soon as possible?
The hon. Gentleman makes an important point. We are encouraging the resumption of aid by bilateral partners. In May, there will be a donor conference on Mali, which is an important step, not just as a pledging conference. There is a political crisis in Mali, and the solution is political, which is why we welcome the recent announcement by the Malian authorities to initiate a commission on dialogue and reconciliation.
I thank my hon. Friend for her encouraging answer, but may I ask her to go further and consider whether the UK Government could work with France and the EU to address the fragility across the region and deliver co-ordinated and sustained development assistance to the Sahel and the whole region?
I thank my right hon. Friend. This very issue was discussed at the recent Development Ministers meeting. Stability in the Sahel—the wider region—is of absolute importance. The UK has committed £78 million in humanitarian support to the Sahel through various United Nations agencies, and we continue to work right across the region to create stability and peace.

Commons Chamber1. What steps her Department is taking to improve standards in education in Nigeria.
As discussed with you, Mr Speaker, and as Labour Front Benchers have been advised, my right hon. Friend the Secretary of State is in Kuwait for an international conference on the humanitarian crisis in Syria. I hope that the House will accept her apologies for not being here to answer questions today.
Our education programmes in Nigeria have already reached 1.25 million children by improving the quality of education in 3,700 schools, and 6,800 more schools will be reached by 2014. We are supporting school-based management committees to make schools and teachers more accountable to parents, and we are providing training to more than 60,000 teachers.
I thank my hon. Friend for that answer and for the reaffirmation of the Government’s commitment to Nigeria, where there are 10.5 million out-of-school children. Inevitably, we focus on the number of children at school, but given the pressures on teacher training, the infrastructure of schools, and the number of children in classes, can we also focus on the quality of the education that those children are receiving?
I thank my hon. Friend for his question and for his work on the all-party group on global education for all. He goes to the heart of the matter, which is not just the number of children coming into school but the quality of the teaching. The statistics show that in Kwara, for example, only 75 of 19,000 teachers passed a test for nine-year-olds—that gives some idea of the scale of the challenge that we are facing. My hon. Friend will therefore be pleased to know that we have a new teacher development programme supporting over 60,000 teachers.
In addition to the excellent work that DFID does in education in Nigeria, what more can the Minister do to suggest to the large number of British companies in Nigeria that they should also be getting involved in taking on responsibility in this respect?
My hon. Friend raises an important point. All UK companies have the opportunity to get involved and engaged. If he knows the names of the companies concerned, I would be only too happy to contact them myself.
2. How her Department plans to reach its target of spending 0.7% of gross national income.
3. What steps her Department is taking to improve food security in west Africa.
The UK is improving food security in west Africa through investment in agricultural research, innovative agribusiness, improving access to markets and supporting national food security plans. We work through country programmes in Ghana, Nigeria, Sierra Leone and Liberia, and international organisations such as the World Bank.
The Minister will be aware that the impact of climate change in that region has led to a dramatic fall in crop yields. What efforts is her Department making to secure global agreement on finding new sources of finance, so that countries in the region can plan properly for their future food supplies?
The hon. Lady points to the fragility of this area and its food security. Crises arise from chronic vulnerabilities that need long-term solutions. We are supporting multilateral efforts to promote resilience in the Sahel to ensure that its communities can deal with the shocks and do not face dire consequences in future. We are currently preparing a Sahel resilience strategy.
Food security in west Africa is indeed threatened by climate change. Does the Minister also agree that the Prime Minister’s insistence on addressing the property rights of the world’s poorest farmers tackles one of the underlying causes of food insecurity?
The Prime Minister is quite right. The work that we are doing to give land title to smallholders means that they have security and can work their land without it being taken from them.
I join Members from all sides of the House in expressing my support for the If campaign, which seeks to end food insecurity and global hunger. One of the main causes of food insecurity is the illegal acquisition of large areas of land by investors. What steps has the Department taken to support good land governance in west Africa?
As I said just now, some of our programmes involve land titles for smallholders, and the UK welcomes the successful negotiation of voluntary guidelines on the responsible government of land tenure, fisheries and forests that was concluded at the Committee on World Food Security last year. The UK is working to promote transparency of land administration and security of tenure in a number of countries. For example, in Mozambique we are helping local communities to register their land, and we want to continue that progress.
9. What assistance her Department is offering to the Government of Mali.
There is a serious humanitarian situation in Mali, with over 360,000 displaced people since March 2012. We do not give bilateral development aid directly to the Government of Mali, but we provide significant assistance to the region through the World Bank, EU and other multilaterals.
I thank the Minister for that response. My constituents usually recognise the great contribution that our aid budget and programme makes, but they also have concerns about the effectiveness of that spending. Will the Minister confirm that in crisis situations, such as that in Mali, money is being spent effectively and will deliver massively good outcomes that I can be proud of?
My hon. Friend raises an important point. British people who support our aid and development programme need to know that money is being spent effectively and I can give him the assurance he seeks. Even in the crisis situation in Mali, agencies in receipt of our humanitarian support are tried and trusted, neutral and impartial humanitarian organisations with a history of effective operations in the most challenging of environments.
Is not Mali a tragic of example of instability and conflict rushing in where democracy breaks down, as so often happens? In this case, that has threatened Mali’s security. Does the situation not further underpin the importance of focused and intelligent aid to support democracy in the developing world?
My hon. Friend is entirely right. Where instability and conflict reign, into such ungoverned space come threats, not only to those in Mali but to the wider world, including the UK. That is why the territorial integrity of Mali must be protected, democratic government restored, terrorism dealt with, and the humanitarian situation addressed. My hon. Friend seeks assurance. We are providing considerable aid support through the UN, the EU and other agencies to promote increased economic resilience across the Sahel, including Mali.
Is the Minister satisfied with the distribution of aid in northern Mali, and particularly in those parts that have been retaken? Have the Government had any discussions with their French counterparts? [Interruption.]
Thank you, Mr Speaker. If I heard correctly, the right hon. Gentleman’s question was about whether we can access those areas. Health non-governmental organisations are still operating in some hospitals and health centres in northern Mali, although NGOs and aid agencies have in some cases been forced to suspend their outreach work temporarily for security reasons. They want to carry out an assessment in the inaccessible areas. Humanitarian agencies are waiting to return to conduct those assessments so that we can respond to those needs. At the moment, they are pretty much confined to the accessible areas.
Although every country has its particular circumstances, everyone knows that the underlying problems that have led to the situation in Mali could exist in many other countries in west Africa. Will the Government agree to make an international effort on a long-term basis to provide support and development for countries in west Africa a major focus of their G8 presidency, and particularly of the summit in Northern Ireland later this year?
I appreciate that desire, but it is not possible to do everything at the G8 that everyone would wish us to do. However, the hon. Gentleman is right. The only solution in the end is a long-term, measured and intelligent political solution.
6. What estimate she has made of the number of (a) internally displaced people in Syria and (b) Syrians displaced to Turkey and other countries; and if she will make a statement.
