Commons ChamberMy hon. Friend is quite right to highlight the significance of smoking as a cause of cancer. We have a number of measures, including the programme to move 1 million smokers on to vaping, the financial incentives to encourage pregnant women not to smoke, the tougher enforcement and the consideration of inserts for packaging. The Government are taking a range of measures to address the very important issue that my hon. Friend rightly raises.
The Secretary of State may be aware that, following work that I have been doing with Cancer Research UK, I have written to him and to the Minister for Social Care to outline my specific concerns about the lack of a cancer strategy. I would be very grateful if he or the Minister came back to me.
As the Secretary of State will know, cancer does not affect everyone equally. When it comes to health outcomes—the Secretary of State made this point—it is often more economically deprived areas, such as coalfield communities like Barnsley, that continue to lag behind. I completely agree and accept his important point about smoking, but studies have also shown that those who worked in the coal industry have a higher risk of lung cancer. I ask the Secretary of State to ensure that ex-miners are considered in the roll-out of the new targeted programme.
The hon. Gentleman raises a valid and important point on the targeting of mining communities. Of course, the roll-out will be shaped by clinical advice, but I will flag that point as we consider the targeting of the programme as it expands.
On the hon. Gentleman’s first point, the major conditions paper will look at these issues in the round. That matters because one in four adults has two or more conditions, so it is important that we look at conditions. A moment ago, I touched on the fact that obesity is the second biggest cause of cancer after smoking, so it is right that we look at multiple conditions in the round. His point about targeting is well made, and I will make sure the clinical advisers respond.

Commons ChamberI absolutely agree with my hon. Friend and congratulate him on quitting. The swap to stop scheme that we are rolling out nationally, which is the first of its kind in the world, is based on strong local evidence. We know it works from local pilots, which is why we are rolling it out at scale.
Smoking remains the biggest preventable cause of cancer and we know that smoking cessation services are vital to kicking the habit, but smokers in England face a postcode lottery when trying to access them. What is the Minister doing to ensure that everybody who needs those services is able to access them?
Absolutely. In total, public health grants will go up by 5% in real terms over the next two years. We want to reduce the postcode variation, because these are important services. I am keen to speak to anyone who wants to work with us at a local level.
It is a huge opportunity. My right hon. Friend and I have discussed this matter outside the Chamber, and I met Ben Goldacre in the summer to discuss his fantastic work in the context of covid. It is absolutely right that, given the potential of artificial intelligence, there are huge opportunities in relation to health inequalities and allowing us to better target provision. I think my right hon. Friend would agree that we should do that through the prism of patient consent. One thing that we are trying to build into the NHS app is the ability to better empower the patient to decide what they wish to sign up to and what they would like their data shared with.
We are committed to a major conditions paper, not least because many people with cancer have multiple conditions; that is why it is important that we look at these issues in the round. With the Minister for Social Care, I had a very useful roundtable with key stakeholders, including the cancer charities. The key issue is that as part of our work on cancer checks, over 320,000 more people are receiving treatment for cancer compared with last year—that is around fifth higher—and we are expanding our capacity through the diagnostic centres, the surgical hubs and the expansion of the workforce. All of that fits within the strategy we have through the major conditions paper.

Commons ChamberI know that my hon. Friend has a real understanding of this particular challenge. In March 2022, NHS England launched an aortic dissection toolkit to support the implementation of robust clinical pathways to identify and manage type A and type B aortic dissection, and English regions are working with the cardiac pathways improvement programme to improve diagnosis and treatment.
Tens of thousands of NHS workers are entirely dependent on the use of their car to do their vital job, but while petrol prices have rocketed, NHS mileage rates have remained frozen since 2011. Will the Secretary of State look at what can be done to increase NHS mileage rates?
That fits within the wider issue of how we work with and support our NHS workforce and what packages we can look at in discussion with Treasury colleagues. Of course, the approach to the NHS is also in line with other Government Departments, but the hon. Gentleman raises an important point and it is obviously one that Ministers discuss.

I agree with the hon. Gentleman. One of the big concerns that has not been answered by the Government so far is why their approach to this drug is so inconsistent with their approach to others. As I say, Ministers have referred Evusheld to NICE, and it is not at all clear why their treatment of it is so different.
Meanwhile, the Department of Health has proposed that immunocompromised patients have an antibody test, and that those who do not respond well enough could join an Evusheld trial. Let us be clear what that trial would mean in real life. It would require some of the people who have been shielding to stay alive for two and a half years to come out of shielding like the rest of us, but without any protection from covid vaccines, knowing that they might only be given a placebo. It would be like taking lambs to the slaughter. I would be astonished and appalled if that proposal passed anyone’s ethics test. I do not know if the Minister would support one of his loved ones taking part in such a trial, but I certainly would not. Can the Minister therefore tell us why his Department wants to take this dangerous approach instead of the approach suggested by the Drug Safety Research Unit, which has called on the Secretary of State to roll out Evusheld now, for this winter, and to run an observational study of the impact?
Another question that has arisen is whether there are problems with supply. The answer is no. AstraZeneca has dismissed that claim.
The hon. Lady is presenting a powerful case; I congratulate her on securing this important debate. Max Johnson is a 14-year-old heart transplant recipient. He was the key figure in the organ donation campaign that was supported by Members right across the House. Max and his family are being supported by their local MP, the hon. and learned Member for Eddisbury (Edward Timpson), but we have kept in close touch. Max’s life has been turned upside down since the start of the pandemic, with no light at the end of the tunnel of isolation. Does the hon. Lady agree that Max, along with thousands of other people across the country, has the right to a better life and should be allowed access to Evusheld without any further delay?
I completely agree.
I do not know how much a dose of Evusheld costs. I hope that, when the Government enter into negotiations with AstraZeneca, they will get a good price for the taxpayer. However, all Members of this House know what the cost is to our constituents who are affected. They have given up jobs, caring responsibilities and vital moments in their lives—they could not go to weddings, funerals or births. Some of them no longer run vital volunteer-led community services. Some have lost their life, and some are suicidal; they are thinking of taking their life because the psychological torture of prolonged shielding is too much for them to bear.
Those people are getting even more ill, because no one can be locked up for two and a half years without getting problems with their back or knees, or experiencing extreme loneliness and mental ill health. Those are all problems that are piling up for our already overstretched NHS. If those immunocompromised patients get covid, they are far more likely to end up in an intensive care unit, which we know will cost us thousands and thousands of pounds. There is also a risk of variant escape, because several studies show that new variants and virus mutations are more likely in immunocompromised patients. Against that backdrop, Evusheld is the cheap option.
Two years ago, Members stood in this room and begged Health Ministers to change their minds on care home restrictions, which were supposed to protect people but were so tight that some people started dying of neglect. We are at risk of that happening again. The Government changed course after that debate and can do so again today. The people affected by this feel that they do not have a life; it feels like a life sentence. Ministers have it in their gift to give those people their life back, and on behalf of the half a million, I urge the Minister—I beg him—to do it today.

Commons ChamberI am sure I speak for the whole House in expressing condolences to my hon. Friend for the loss of her mother. I know she is a doughty champion for addressing health inequalities in her constituency. While the publication of the 10-year cancer plan is under review, we remain committed to tackling inequalities and levelling up outcomes, experience and access. That is a key focus of the NHS Long Term Plan and 2022-23 planning guidance, and it remains a priority for the Government and the NHS cancer programme. Approaches to support that are embedded throughout the programme —for example, increased accessibility for the cancer quality of life survey, to help increase representation results and, as I mentioned, the targeted lung health check programme is focused on areas with high lung cancer mortality, where typically there are also high levels of deprivation.
Weston Park Cancer Centre provides outstanding cancer care for patients, not just in South Yorkshire but around the country. I met two previous Secretaries of State, and the outgoing Minister, to press the case for investment in the site, given that it is now 50 years old. Will the Minister please look at the case for investment in Weston Park, so that it can continue to provide outstanding care for patients around the country?
I am very happy to meet the hon. Gentleman. We are investing further money—£350 million in diagnostics—and making efforts to address the backlog caused by the covid-19 pandemic.

I am always delighted to take questions from the hon. Gentleman. I absolutely agree that if there is parity in all four corners of the United Kingdom, there is an opportunity to ensure that all citizens can get the organs they need; that they can get on to the register where possible; and that there is a developed and comprehensive transplantation strategy across the country. I understand that the hon. Gentleman went further than his party and was by far one of the earliest supporters of the opt-out initiative. I know to my heart that he was a pioneer in leading his party and getting them to where they needed to be to see the changes in Northern Ireland. I am grateful for his question.
As I was saying, the Government and the NHS should be proud of the campaign that they have run to date, and the undeniable progress that it has delivered. That brings me to my second point: the organ donor register. I am sure that all colleagues here today will agree that it is essential that we encourage as many people as possible to sign up to the organ donor register. As of 31 March 2021, 38% of the population had joined the register, while 3% had opted out. Initiatives are already in place to increase registration, with a number of routes available, whether through the Driver and Vehicle Licensing Agency, the NHS app, applying for a Boots advantage card, or even through the NHS Blood and Transplant organ donation website, which, just for clarity, is at www.organdonation.nhs.uk.
It is vital that we continue to keep as many avenues open as possible, and that the campaign continues to be fully supported and championed by the Government. With that in mind, I ask the Minister what plans are in place to continue to raise awareness of the organ donor register and to encourage continued conversation and education around organ donation. Secondly, would the Minister consider extending the scheme to include other official forms? That might include, but not be limited to, those signing up to the electoral roll or giving blood.
The organ donor register moves an individual’s organ donation from a passive decision to an active one. For every individual that decides to sign up to the organ donation website, they are providing a record of their consent to help save lives should the unimaginable happen to them. Importantly, by signing that register, individuals are providing an affirmation of their desire to be an organ donor, which I hope that their family members and loved ones will honour—I will touch on that again shortly.
As of 13 February 2022, there are 6,157 people waiting for an organ transplant in the UK. Even the large numbers that I have buried the House in thus far hide the fact that there is a shortage of donors in the UK. Between April 2020 and March 2021, in the UK, there were a total of 1,180 deceased donors and 444 living donors, which resulted in 3,391 lives being either saved or dramatically improved by an organ transplant. However, 474 people died while on the active waiting list and a further 693 were removed, primarily because of deteriorating health. Of course, I accept the varied reasons why people come off the list, but the numbers provide an indication that while the situation is improving, there is still work to be done.
The NHS Blood and Transplant strategy, “Organ Donation and Transplantation 2030: Meeting the Need”, published on 1 June, calls for a highly public campaign broadening the settings in which people might find information around organ donation. It also includes six key points: making living and deceased donation an expected part of care; developing and pioneering new technologies and techniques; ensuring recipient outcomes are the best in the world; ensuring that people of all backgrounds and circumstances have timely access to the organs they need; maintaining a sustainable service across the UK; and building a pioneering culture of research and innovation in donation and transplantation in the UK. I ask the Minister how those six action points are being monitored, and how often they will be reviewed. Furthermore, does she feel that anything should be added to those points since the introduction of the 2019 Act?
Although I promised to be positive and congratulatory about the Government’s action on this matter, I am aware of a few areas relating to organ donation that are causing some concern. As mentioned already, under the 2019 Act, and specifically the opt-out system, all over-18s—albeit with a few caveats—are considered to become organ donors when they die unless they opt out. An individual can also actively register, as I have mentioned already, through the organ donor register. However, a family member or loved one can—and often does—overrule the donation of an organ in both instances. As mentioned already, the consent rate for eligible donors was 68% between April 2020 and March 2021, meaning that loved ones, for various reasons, refused to support 32% of potential donations. That equates to 695 donors.
There are myriad reasons why consent for deceased donors might not be given: the patient expressing a desire not to donate, but not opting out; a lack of desire for further surgery on a body; a feeling that the patient had suffered enough; the fact that the process takes too long; or the fact that the donation was against religious beliefs. Of course we must respect the decisions and views of family members and loved ones; staggeringly, however, 10.2% of those 32% of organ donations were refused because family members were unsure about whether the patient would have wanted to donate. Surely that clearly shows the continuing need to have a conversation and actively encourage greater sign-up to the organ donation register. In actual numbers, that 10.2% equates to 71 individuals whose organs might have helped to save a great number of lives. Of course, I make no judgment about those families and the decisions that they take in incredibly difficult circumstances, but there is an opportunity for us to go that little bit further and help save those extra few lives.
With that in mind, what progress has been made with the Leave Them Certain campaign mentioned in the NHS Blood and Transplant strategic plan, which I referenced earlier? I understand that the Human Tissue Authority guidance specifically states that families will always be consulted and that scrutiny is needed in the process. However, where possible we should be trying to eliminate the second-guessing and possibility of going against the deceased’s final wishes.
I asked for this debate because among the regular correspondence that I have had with constituents on the matter of organ donation, I have had the incredible good fortune of having been introduced to Sarah Meredith and her family. Sarah is a 29-year-old constituent who lives with cystic fibrosis. Thanks to the approval of the drug Kaftrio, Sarah and thousands of others living with cystic fibrosis can look ahead with an improved degree of certainty and a greater quality of life. However, that wonder drug does not solve all the difficulties of living with that disease; Sarah needs a liver transplant.
Over the course of the last two years, I have met Sarah’s mother Cathy and sister Jessica to hear first hand about the ailments from which Sarah suffers and some of the problems that they have identified within our transplant system and the wider regional disparity when it comes to healthcare services. I have already highlighted some of the concerns around organ donation, but I would like to add a few words about healthcare infrastructure. The organ utilisation group, chaired by Professor Stephen Powis, was established by the then Health Secretary, my right hon. Friend the Member for West Suffolk (Matt Hancock), to provide recommendations that would deliver improvements in the number of organs accepted and successfully transplanted; to optimise the use of existing skilled workforce investment in infrastructure; to support innovation in the field of organ transplantation; to standardise practices across the country—a point made by the hon. Member for Strangford (Jim Shannon); and to provide equity of access and patient outcomes. I look forward to seeing the recommendations, the report when it is published—in March, I believe—and the Government’s response.
Will the Minister come before the House when the report is published to take questions from Members interested in this topic? Although the south-west can boast a high consent rate—it is only slightly off the Government’s 80% target—we are at something of a disadvantage when it comes to liver transplant units across the region. There are just eight such units in the UK, including one in a children’s hospital. The liver transplant centres nearest my constituency of Totnes in south Devon are in either London or Birmingham.
I have heard anecdotal and first-hand accounts about ill patients who have been asked to make the journey to London from south Devon for a transplant, only to arrive and discover that the organ they were expecting has deteriorated and is no longer suitable for transplantation. One can only imagine how awful that journey is in both directions in that situation. I understand that there is a new national programme to expand the number of living transplant centres across the UK and that the north-west and south-west are two priority areas due to a lack of existing transplant infrastructure. Will the Minister reassure me and all those across the south-west who are hoping for an improved service that this new programme will be rolled out at pace? It is clear that many cannot wait.
I hesitate to intervene on the hon. Gentleman because he is making an excellent speech, but I wanted to take the opportunity to warmly congratulate him on securing this debate and bringing this important matter to the House’s attention. I should declare an interest as part of the team of people who took the legislation through the House—that was a genuinely outstanding cross-party effort and I am really delighted that we are proceeding now in that same vein.
The hon. Gentleman rightly raised concerns about the number of people currently on the waiting list; we are now at a five-year high and the pandemic and lockdowns have not been helpful. Does he agree that although legislation was, of course, very important, it is not enough in itself? What we need is continued public education and additional capacity within the NHS so that we can continue the important process of saving lives. I congratulate him again on securing this important debate.
I thank the hon. and gallant—and mayoral—Member for his intervention. He has been an extraordinary champion on this matter; a significant amount of my research has been on the back of his words in this place to help to get that legislation to where it needs to be. On his point about education, I think he is absolutely correct. We need a combination of education and funding across all our hospitals, GP surgeries and other available forums to promote this issue so that we can bring down that five-year high and help to get as many people as possible off the transplant list, as quickly as possible.

Commons ChamberI very much agree with my right hon. Friend; as the whole House knows, he speaks with considerable experience. We need to do things differently, especially as a result of the pandemic and the challenges that it has created. That requires reform, and we will set out further reforms in due course. He is absolutely right about targets: they can play an important role, but they can also lead to poor outcomes for patients, and all targets need to be properly reviewed.
Sheffield’s Weston Park Cancer Centre is one of just four specialist cancer facilities in the country, but it desperately needs a £50 million upgrade, as the Secretary of State will know because I raised the matter with his predecessor and wrote to the Secretary of State in October and again just last week. Will he urgently respond to the proposal, which is vital for cancer outcomes in South Yorkshire?
We will endeavour to respond swiftly, but if the hon. Gentleman would like to meet me about capital funding for those sorts of projects, I am always happy to meet him.

Commons ChamberI refer the hon. Gentleman to the answer I gave earlier to my hon. Friend the Member for High Peak (Robert Largan). We will be considering everything in the round. As I have said, I have spoken to many of the stakeholders involved and as we move forward we will be ensuring that we take all trusted individuals with us to build confidence in the system.
The organ donation opt-out system has increased the number of organs available for transplant and is saving hundreds of lives. Since the law changed last year, 296 people in England have donated their organs under the opt-out system. These donations account for 29% of the 1,021 donations that took place last year.
I am grateful to the Minister for that response. She will know that 20 May marked one year since Max and Keira’s law came into effect—a change that will give hope and save lives—but despite the tireless efforts of NHS staff, covid has had a devastating impact on patients in need of life-saving operations. Can the Minister outline how she is going to get organ transplant services back to pre-pandemic levels and tell us what additional resources will be committed in order to support an increase in organ availability?
I would first like to thank the hon. Member for the part he played in campaigning for this life-saving change to organ donation and bringing about the increase that I mentioned earlier. The current services are now running at pre-covid levels and NHS Blood and Transplant is working with the wider healthcare system to enable as many transplants as possible. The new Organ Donation and Transplantation 2030: Meeting the Need strategy, which was launched last Tuesday, sets out the steps we are taking to increase organ availability further.

Commons ChamberI highly, 100%, wholeheartedly agree with my right hon. Friend. The model that we established in the vaccines taskforce—of the holy trinity of business, academia and civil service colleagues working together as one team—was learned from reforms from his time in government. It has worked incredibly well, and I think will do as well in the future.
We also apply that approach to therapeutics, and one of the reasons, alongside the NHS, why we are the only country in the world that has participated in the discovery of all the current therapeutics is our use of that approach. Obviously we are driving it further, and I would be very happy to discuss it with my right hon. Friend.
One of the gravest threats posed by covid is its potential to create a cancer epidemic. I recently co-ordinated a cross-party letter to the Secretary of State in support of an urgently needed £50 million refurbishment plan for Sheffield’s Weston Park cancer centre. I urge the Secretary of State to give serious consideration to the proposal, which would ensure that patients across South Yorkshire and Bassetlaw received the best possible treatment.
I have seen the letter and I am looking into what we can do. Because of the crisis the need to tackle cancer is more urgent but there is also more hope, and the hope is that the underlying technology behind some of the vaccine—the so-called mRNA technology—also has the potential to improve cancer-fighting technology radically. So we propose and plan to support very significantly work on that, in order to find further breakthroughs in cancer treatment. I would be very happy to talk to the hon. Gentleman—and indeed you, Madam Deputy Speaker—about the proposal for South Yorkshire.

Commons ChamberYes, absolutely. Stepping Hill Hospital is obviously vital to my hon. Friend’s constituents. The urgent care centres are important too, especially in ensuring that people can have access to treatment closer to home for smaller, yet urgent problems. We have also introduced 111 First and people should call 111 before going to an urgent treatment centre or an A&E to let them know they are coming and to check that that is the right setting for them. That is an important part of our wider considerations, which the measures in the White Paper will help.
Shamefully, the poorer people are, the younger they will die. That link between economic deprivation and health outcomes means that Barnsley is suffering one of the highest covid mortality rates in England. Health inequality is an incredibly complex problem, but it is avoidable. Does the Secretary of State agree that a key test of the reforms is whether they will tackle that injustice?
I do. Tackling health inequalities is incredibly important and is a vital part of our levelling-up agenda. The hon. Gentleman is right to point out the gaps in life expectancy across the country. I hope that a move to a population health approach, whereby the focus of the whole local system is on improving the health of the population, not just those who ship up needing support, can help us as a society to tackle health inequalities.
