It is a pleasure to serve under your chairmanship, Sir George. We all agree that we need more doctors and I think we all welcome what the Government have done to increase the number of places and of medical schools. We had 2,671 trainees start in 2014 and we have had 4,000 start in the most recent year. That is all welcome. We know it takes time and costs money, in the region of £250,000 per person, but it is clear the Government want to get a grip on the problem.
The Government need to do that. I have had a huge population growth in my constituency and have seen a number of GP surgeries close their books. I have seen a surgery in Wallingford close its books, as have all the surgeries in Didcot. In some parts of the constituency we have helped a building expand to ease the problem, but here, without more doctors, it becomes difficult to serve the growing population.
Will the Minister comment on one thing that concerns me about the people we are training? I had an interesting conversation with one of my GP practices just a few weeks ago and I was told that a lot of the trainees now want a portfolio career. Of the cohort from which they have a trainee at the moment, only one intends to be a salaried GP. No one wants to be a partner; it is seen as the drudgery or boring part of the profession. People want to do some days as a locum in urgent care, specialist clinics and so on. I do not know the extent to which Government are looking at that and at how the profession is marketed. It seems to me that a salaried GP is a key pillar of the community, but, much like other people of their age, trainees are looking to do a range of different things, rather than the thing I believe we most need them to do at the moment.
My hon. Friend the Member for South Cambridgeshire (Anthony Browne) set out the challenges of training in superb detail and I am grateful to him for securing the debate. As my hon. Friend the Member for Bolsover (Mark Fletcher) commented, one key issue for me is about the make-up of the profession. Medicine is the most socially exclusive profession in the country. Only 6% of doctors come from a working-class background and someone is 24 times more likely to become a doctor if their parent is a doctor. If anyone wants to intervene and say that that reflects the country’s talent, they are welcome, but I simply do not believe it.
Medicine outstrips every other profession that we think has a problem, such as politics, journalism and law. In all the work I did on social mobility with young people on free school meals, a high proportion of whom are from ethnic minorities, inner-city areas, coastal towns and so on, it was the most popular profession. As others have said, this is not an issue of medicine not being popular or people not applying or not meeting the grades, as the grades have to be met to be able to apply. Applying is a complex process that involves all sorts of things, from personal statements to interviews and work experience. People get work experience very easily if they are related to a consultant but they do not get it without those connections, yet it is essential to getting into the profession. To make the most of the country’s talent, the profession needs to look at that very closely.

It is worth outlining where we see the future of our immigration system. As I touched on in my statement, EEA nationals already make fully online applications, for things such as student and skilled worker visas. When we rolled out the British nationals overseas route last year, we included a fully digital application system, which the vast majority of applicants have used. Our future work is to move away from people having to go to a VAC every time they want to apply for particular types of visas, including visit, student and skilled worker visas, and for a range of products that people apply for. For example, we will be moving to more of a system where we re-use biometrics or are able to extract biometrics via passports. Our future vision for the UK immigration system looks towards a time when a lot fewer people will be going to a VAC than are doing so today, and that technology will be used. We have seen that move in the Ukraine schemes. For example, about 90% of those who have now been granted visas under the Homes for Ukraine scheme have done this via the biometric bypass: they have not had to attend a VAC. We are also looking to roll out next month the system that will allow those who have come to this country with six months on a permission to travel letter to then be able to apply for the full visa from home, as would those looking to travel after that. So we are looking to reduce significantly the number of people who need to use a VAC.
That said, for those who do not have valid international passports the VACs perform a role of carrying out safeguarding checks, particularly in relation to children. For those of a younger age, we are not looking at the same security checks as we would do for an adult. For children, we are ensuring that key safeguarding checks are done. As we have said, our feedback at the moment is that there is wide availability of appointments, and that a large number of visas have been issued and people have arrived in the UK, having been through that process, in relatively significant numbers. We continue to work with our provider to improve the service on offer, but, as I say, our long-term vision is moving strongly away from VACs and things such as the issuing of vignettes, and instead looking towards e-visa permissions, which will mean that people do not need to go to collect something physical in their passport to allow them to travel to the UK. That is where the vision is going, but the changes we have made to the two systems, allowing the biometric bypass, means that the vast majority of people now making applications need to go nowhere near a VAC.
A member of my staff spends a significant proportion of each day working on visas for Ukrainians, and I thank her and the hub team here, who have been supporting us all with this. Does my hon. Friend realise that one common problem is that the visa is approved but the person in question does not get the email giving them permission to travel for quite some time afterwards? Is he aware of that? Is he working on it? Will he set out what is being done about it?
We have been aware of an issue with the system in terms of the current process of the decision being made and then the visa dispatched. We have a particular team working on ensuring dispatch. The changes we will make in respect of the fully online system next month will mean that a lot of it becomes automated, which will resolve that particular issue. We have been aware of some instances and have a specific team that makes sure that decisions are dispatched.
I very much appreciate my hon. Friend’s comments about the hub, which has been assisting Members and ensuring that people’s visas get dispatched. As I say, we have now seen nearly 90,000 visas issued and significant numbers of people arriving here in the UK having used the biometric bypass route or been to a visa application centre. That indicates to us that the system is now working effectively.

As the right hon. Lady will know, thanks to this Government there are now significant penalties under the modern slavery legislation for those who traffic individuals. However, I hope she will forgive me if I do not necessarily reveal what is in the review. I hope that she will see that, whatever the circumstances of that particular offence, once the work starts—the work has started, but once we get going on the work that sits behind the rape review—we will see perpetrators of all kinds of these offences in court, where justice can be dispensed.
Last month, a constituent of mine sent me a very powerful account of how her case has taken nearly three years to reach court. During that time, she has been told not to have therapy; that she could have therapy as long as the notes were shared with the defence; that she should not claim compensation; that she should not speak about it; and, at one point, that she would not be able to watch the trial. Will my hon. Friend assure me that the review will look both at how we can get cases to court more quickly, but at how victims can feel more supported, rather than feeling as my constituent has felt—inadvertently silenced?
I am very distressed to hear the experience of my hon. Friend’s constituent; it sounds like a dreadful case. On the therapy issue, the guidelines in place say that pre-trial therapy is absolutely allowed and appropriate, and nobody should be steered away from it. I would be more than happy to meet my hon. Friend to discuss that particular case, because it sounds like one from which we can learn some lessons.

Commons ChamberWhen it comes to police training, I think it is important to reflect on a lot of the work that is already under way across all police forces, not just the Metropolitan police force. The College of Policing has extensive work taking place in this area, which is also subject to a lot of the work that takes place at the National Crime Agency Board.
I also extend my deepest condolences to Sarah Everard’s friends and family. Does my right hon. Friend agree that preventing violence against women is partly about what we do with boys? That means teaching them that what is often depicted on TV, online and in video games is not acceptable behaviour, as well as simply restricting what they see through the forthcoming online harms Bill.
My hon. Friend is absolutely right, and that is another Bill that will come to the House in due course. This is about cultural aspects and the behaviours we inculcate in our children, including how our boys grow up and the things they are exposed to. This will be subject to much discussion and we welcome the views of my hon. Friend and others in the consultation we have just reopened.

Commons ChamberThe lead times that the hon. Member is describing are, thankfully, very rare exceptions. As I said, for the most serious cases, where the defendant is remanded in custody, a clear majority of those that had a first hearing in November will have their substantive trial by July this year. We are taking action, we have been taking action, and we will continue to take action to look after victims of the most serious offences— the most distressing ones, such as rape and domestic violence—by making sure that they are supported. I have mentioned the £32 million of additional money that will be spent next year on the victim service, ISVAs and all those things that support victims and witnesses. I have also mentioned the use of section 28 evidence, which means they can give their evidence by video at a very early stage, rather than having to wait a long time. All those things are concrete, tangible actions that will help in the area that she raises today.
I thank my hon. Friend for his statement on recent progress in clearing the covid backlog. There will be victims out there who will look at these headlines and wonder whether they should pursue their cases, or perhaps recent victims of crime wondering whether they should come forward at all, given the delays that the papers have suggested. What is my hon. Friend’s message to victims of crime at this time?
I say to victims: we are there to support you, to hear you and to seek justice for you. As my hon. Friend knows, we are hiring 20,000 extra police officers to keep victims safe. We are keeping the court system running in these difficult circumstances. We are getting back to a period in which magistrates courts are clearing the backlog and, I believe, the Crown court shortly will do so. So I say to victims: justice will be done. Your voice will be heard. Come forward. We are here for you. Do not hesitate—we want to hear your story. We will listen to it, we will act and we will make sure justice is done.

Lady is absolutely right to highlight the enormous work that schools are doing during lockdown restrictions as well as in the period that we had last year between lockdown restrictions. We have invested some £8 million to help teachers spot some of the signs of abuse and distress in children. It may take some time for those signs to show when children return to the classroom, but the money is intended to help to address that. The hon. Lady will, I hope, have heard about the investment we have made in the “See, Hear, Respond” scheme. We have also provided £7.6 million to children’s charities that have suffered financial hardship due to covid-19.
We continue to fund Childline, as well as the professional helpline for adults who are concerned about a child. In addition, we are funding a helpline set up by Operation Encompass in the middle of the first national lockdown last year. It is a helpline for teachers worried about the families of children in their school, particularly in relation to domestic abuse, and want to know what they can do to help. The Operation Encompass helpline exists very much to help the teachers who help to look after our children, and I recommend it to teachers in the hon. Lady’s constituency and elsewhere.
I warmly welcome the launch of the new codeword scheme today. I have a constituent whose abusive partner is the subject of a non-molestation order, but because the place where she lives is the place where they had a joint business, he has managed to get the zonal aspect of the order lifted, on the ground that he needs to be in the location for business. The abuse has restarted, and during lockdown it is difficult for my constituent’s friends and family to support her in the right way. Will my hon. Friend meet me to discuss how to give better support to victims of abuse in these circumstances?
I most certainly will meet my hon. Friend, and I thank him for raising a case that demonstrates the complexity of many people’s experiences of abuse, even after a relationship has ended. The Bill contains a number of measures to address that sort of abusive behaviour, including the introduction of domestic abuse protection orders that will put positive requirements on offenders, as well as negative ones. I am happy to meet him, and I hope that, through his work and support from services, his constituent is able to find a solution to the terrible situation he has described.

Commons ChamberThroughout the covid-19 pandemic, we have provided unprecedented additional funding to domestic abuse service providers to bolster their capacity to meet the demand for support. We announced further funding last month and relaunched the #YouAreNotAlone campaign to ensure that victims of abuse and those worried about them know how to access help and advice. In addition, the police continue to target perpetrators of abuse proactively because there is no excuse for abuse.
I am grateful to the hon. Gentleman for raising a constructive point. I hope that he knows that we were very keen through the pandemic to help at a local, regional and national level. Indeed, I was very careful to ensure that police and crime commissioners, who are responsible for distributing the local level of funding, do so not just to the services that are commissioned, but also to non-commissioned services, because there is a wealth of expertise across the country. On the point about funding, I will of course take that away. It is something that I have been discussing with charities and I know their concerns; we are dealing with that issue during the spending review allocation process.
Reducing the Risk supports victims of domestic abuse in my constituency and across Oxfordshire, and it was grateful to receive some of the funding that my hon. Friend refers to and which has helped it to support 50% more people. One of the things it focuses on is prevention. Does my hon. Friend agree that although we have to deal with the cases that we have seen spike during this year, we must not lose sight of the importance of prevention so that they do not get to that stage in the first place?
I very much recognise that. Indeed, part of the programme of work that sits within the Bill and outside the Bill is about tackling those who perpetrate domestic abuse. We need to stop these cycles of abuse; sadly, in some cases, perpetrators go from relationship to relationship, abusing and hurting people in their wake. One of the things that I am very interested in—I know that this is also an interest of my hon. Friend—is looking at what more we can do to understand the work of academics, particularly in interesting areas such as the use of artificial intelligence, to see whether we can do better by way of risk assessing domestic abuse perpetrators and the terrible impacts that they can have on their victims.

Commons ChamberThe hon. Gentleman asks important questions about surveillance techniques. There are always lessons learned about surveillance techniques and how people are monitored. As I mentioned in my statement, the intelligence and security services have to make calculated judgments based on the threat and the information that they have. They will continue to do that, and they are constantly reviewing many of their own techniques.
May I extend my deepest sympathies to the family and friends of those who lost someone, and commend Thames Valley police and the other emergency services? My constituency is in the Thames valley and I know how good they are. Does my right hon. Friend think that when the police do have the person they think committed this crime, the media attention should focus on the victims and not on the beliefs and backstory of the people who perpetrate such attacks, thereby denying them the attention they crave?
There is merit in my hon. Friend’s point. The spotlight should not be on the individual, the organisation or those who promote terror, hate and fear. We must always be respectful to and mindful of the victims. Of course, at times like this, we have to give the police the time and space they need to get on with their investigations.

Commons ChamberIt is a pleasure to speak in this debate. During the election campaign, I was on a street stall in Wantage when a woman from Zambia came up to me wanting to talk about Brexit. Wantage and Didcot was 54% remain, so this conversation could have gone either way, but she was in favour of Brexit. She told me that she had been working in the NHS for nine years, but that she could not get settled status, yet if she had come from Europe and been here for five years she would have been able to do so.
It is right that we have a settled status scheme for those in the EU. The fact that 3.5 million people have already applied for it suggests that it is working very well. That lady’s question to me was, “What about the Commonwealth countries? What about Britain’s relationship with those?” I agree with her and think it is right that we now have an immigration system based on what we need rather than on whether someone is from Europe.
There has been quite a bit of discussion about the NHS and social care this afternoon, which is completely understandable. I welcome the Government’s commitment to a fast-track visa for doctors and nurses, and their extension of the health worker visa by a year if it is to expire before 1 October. I also think it is absolutely clear to all of us now, if it were not so before, what a vital role those from other countries have been playing in our social care system. We knew that for decades, but it has been highlighted in recent months. Where I part company from some Opposition speakers is in their thinking that because of that, we should continue to import our care workers from overseas. The answer to social care is in a cross-party solution wherein we properly fund and structure it and it is seen as a well-regarded profession; it is not to keep on doing what we have done for decades, because if we do, we will only put off into the future the solution that is really needed. It is worth saying that those from the EU who are currently working in our care system have probably already applied for settled status and are certainly entitled to do so.
I wish to make a similar but different point about higher education. I welcome our being a magnet for global talent. It is right that we continue to attract international students and that we have committed to a two-year work visa so that they can find work after they graduate, but I have watched with increasing despair as certain universities have chased a higher and higher proportion of international students, whom they can charge higher fees for low contact time, while those universities often neglect to widen access to their institution to young people who are under-represented in this country. Why are they so reliant on the international fee income and the international market? That is the fundamental question and it cannot be solved by changing the Bill.
I support the Government having lowered the income threshold from £30,000 to £25,600, and it is right that it is lower still for those occupations where we have a shortage of people. It is of great value that we are going to have a seasonal worker visa, which will be particularly important for a constituency like Wantage and Didcot, which has a lot of farming. It is completely correct that the House should continue to debate whether the income thresholds and occupation lists are right and whether we get the point system right, but the most important thing about the legislation is that these things will now be within our control. We will be able to adjust those income thresholds, occupation lists and points.
If one talks to the vast majority of British people, one will hear that they support immigration—they welcome it and can see the contribution that it has made to every aspect of our life—but they expect that the people they democratically elect should be able to control the flow and to increase or decrease it. Importantly, they expect that those people they elect will properly plan the infrastructure that needs to accompany immigration—that we will have the school places, GP appointments and houses we need. That is why they have supported the ending of freedom of movement and the move to the new system that we are going to have, and that is why I, too, fully support this change.
