I do not have a problem with having a full, fair and transparent register of lobbying activities. I do not believe that charities will feel themselves constrained from lobbying. I am concerned that the Bill’s loopholes, which do not catch personal, behind-the-scenes and subtle lobbying, could lead to more lobbying being driven underground by the craft’s practitioners. The charities have nothing to fear by being transparent about making powerful cases. My concern is about decisions that have been influenced subtly and policies that have been driven by a particular narrative behind the scenes that we as Members of Parliament find hard to track down.

Does my hon. Friend agree that when the barnacles are scraped off the boat—including our entire public health policy—what people want to know is what organisations are represented by those who are in a position to make powerful cases in rooms to which the rest of us do not have access?

My hon. Friend makes a valuable point. Many Members will have short careers in this place and I am sure that my career is as open as anyone else’s to the vagaries of public decision making. Many former Members go on to exert the subtle forms of lobbying that we are all decrying, because they get powerful positions in and links to industries and bodies and know, as has been said, which buttons to press and which mobile phone numbers to call. That is what I want to address. Other Members have the same concerns and the Select Committee has raised them, too. Today gives us an opportunity to ensure a level playing field and to bring a degree of clarity to the domain of lobbying and the role of a lobbyist.

As I said in a speech last week, I am unhappy that discussions about a strategic rail freight interchange in my constituency were held over a private lunch. That would not be captured by the Bill. The gentleman involved is a professional lobbyist, but he is also a personal friend of the then Transport Minister. I do not understand the volte-face, but it would help if I knew who met whom. The e-mail the gentleman sent asked whether there was

“anything your department can do”.

That is how a lobbyist works: once they get an ear and access, the chain reaction—the butterfly effect—that they so desire occurs and, without transparency or a register, it is very hard for people to know where meetings have taken place.

Private lunches would be captured by my proposed new clause, which covers any activity for the purpose of “influencing government” or

“advising others how to influence government.”

Any one of us could sit at a table at a private lunch or a fundraising function and end up being lobbied firmly. If such lobbying were to continue, I would feel an obligation to declare it under my proposed new clause. I could listen to what was being said, but if I did anything about it I would regard myself as having been successfully lobbied.

Parliament has done its job today: it has applied the brakes to a headlong rush into unilateral western military action.

The problem with the motion, which is undeniably full of a series of truths, is that it draws one into agreement. However, there is a sting in the tail, which is that it asks us to agree that unilateral western military action is legally justified. I do not agree with that statement. For that reason, I am sorry that I will not support the Government motion tonight.

The country is almost unanimously opposed to unilateral western military intervention. That is not because we are a nation of appeasers and apologists; it is because the nation rightly has weighed up the risks of such action exploding into a wider military conflict with hundreds of thousands more deaths.

Does my hon. Friend agree that one of the reasons why many of our constituents are so opposed to this debate taking place is that they believe we are about to vote on military action? Of course, that is not the case, as the Prime Minister made clear today.

I will not at this stage.

The Arab League has supported the principle of UN-backed intervention, but it has stated today that it does not back unilateral western military intervention. That is right. It undermines the Arab League when the west constantly steps in and makes decisions instead of allowing it to develop a regional solution that could lead to lasting peace.

We cannot destroy Assad’s arsenal of weapons. That has been made clear. The best that we can do is to deliver a warning. Are we seriously suggesting that no nation in the Arab League is capable of delivering that warning? Is Saudi Arabia not capable of that? If not, what on earth are we doing arming all these nations to the teeth? It is time for the Arab League to step up to the plate and for western countries to recognise that we cannot continue to impose solutions, because those solutions fuel resentment and harden attitudes; they raise the question about the double standards of the west across the middle east.

Where was the world’s policeman in 1985 when Iran was under sustained attack from chemical weapons? It suited the west to support Iraq in that situation. Why did we allow the world’s policeman to weaponise white phosphorus? When white phosphorus contacts the skin and burns as it oxidises, it burns right down to the bone. If that is not a chemical weapon, what is? Why is the world’s policeman allowing the USA to sell cluster bombs to Saudi Arabia? The point about cluster bombs is that they continue to kill and maim children long after the combatants have left the field. We need to be very clear. Why is the world’s policeman not calling a coup a coup in Egypt? These are the kinds of issues that cause burning resentment across the middle east, with good reason. It is time that we let the Arab League come to a regional solution, if we are going to achieve lasting peace.

To be wary of war is not to stand idly by, but a realistic appraisal of the risks and learning from past experience. The British people are not standing idly by; they are delivering humanitarian aid, but they do not feel that humanitarian aid from the west is best delivered in the form of a cruise missile.

It is a pleasure to follow the hon. Member for Sefton Central (Bill Esterson) and my hon. Friend the Member for Lichfield (Michael Fabricant). I agree with my hon. Friend’s reservations about High Speed 2, and if just a fraction—even the tiniest fraction—of the investment in HS2 were invested in cycling infrastructure, it could transform the lives of millions of people across the UK. I am a south-west MP and in that region we feel that we could have benefited from a small percentage of that investment in HS2 being invested in electrification of the line down to the south-west, preventing it from being completely cut off every time that it rains heavily.

Yes, indeed. They say in the south-west that if one can see across the valley, it is about to rain, and if one cannot see across the valley, it is raining already.

Having expressed those reservations about HS2, I welcome almost everything else within the Gracious Speech. My principal point is about the draft care and support Bill. To see that Bill finally being introduced in this Session of Parliament is very welcome. I remember well the shock and horror of many of my former patients when they realised that if they had assets above the threshold of £23,250, they would receive absolutely no support with their care needs. We know that one in 10 families face losing more than £100,000 of their income just to care for a relative, and that very many people end up having to sell their homes to pay for their care needs. So, such a massive increase in the asset threshold and a cap on lifetime costs is very welcome, particularly because those measures will encourage people to come forward at an earlier stage to seek the help that they need. In turn, that will help to reduce unnecessary admissions as well as helping people to remain as independent as possible for as long as possible.

Of course, the Bill will introduce support and proper assessments for carers, not only for adult and elderly carers but for child carers, who suffer and are robbed of so much of their youth as a result of their caring responsibilities. I am looking forward to seeing the detail in the Bill, and I very much enjoyed being part of the Joint Committee on the draft Bill that made recommendations to the Government; I hope that many of those recommendations will be included in the Bill when it is placed before Parliament.

I also particularly welcome the fact that there will be compensation for the victims of mesothelioma who cannot trace an employer and for those whose employer has gone out of business, or who do not have any insurance. It is particularly cruel that they receive no access to any compensation, despite mesothelioma being almost entirely attributable to asbestos exposure. But, and this is a big but, how ironic that while providing fairness and support for one type of lung cancer we are failing in this Queen’s Speech to address preventing a far more common type of lung cancer—failing to address how we are going to stop the next generation of smokers coming on stream. We should bear in mind that every year 200,000 children take up smoking. Those children will be at risk of going on to face a lifetime of problems. We know that 100,000 people a year at least are dying as a result of smoking-related problems. The failure to take forward plain packaging is a huge missed opportunity.

I want to clarify one thing: there is nothing plain about so-called plain packaging. I would encourage everyone to google what plain packaging looks like. Plain packaging sets out very clearly what is involved. It sets out the disease and suffering that people will face if they do not address their smoking. My experience as a doctor was not so much that people feared the thought of death, but that they most feared the process of dying. The process of dying from many smoking-related illnesses is hideous. We are not just talking about lung cancer. We are talking about, for many people, the years spent in a kind of living death, tied to an oxygen cylinder, suffering from end-stage chronic obstructive pulmonary disease and many other conditions; or the suffering that comes from needlessly losing a leg from arterial disease. Smoking is a leading cause of blindness. There are many effects of smoking—all entirely preventable. So-called plain packs spell that out graphically, and to anyone who hands around such a pack, it is quite beyond a simple public health message. It is a very graphic message.

I welcome what the hon. Lady says about plain packaging. In the Committee stage of the Children and Families Bill, I and a number of Members tabled an amendment about banning smoking in cars with children present. I wonder whether she would agree that we hope that the Government will bring back their own version of that amendment in good time.

I thank the hon. Gentleman for his intervention. Indeed, this is about protecting children, and that is what we should focus on. It is not about introducing a nanny state. The so-called plain packs would not necessarily change the habits of a committed lifetime smoker, but they are aimed predominantly at deterring the next generation. I feel this is a missed opportunity, and I very much hope that as further evidence emerges from Australia, the Government will reconsider their position and send a very sensible public health message.

Many Members have commented that the Gracious Speech is not just about setting out what legislation will be introduced; it is about sending a message on the direction of travel. My very clear view is that Government’s core business does include public health. Members know that I feel strongly about minimum pricing for alcohol. I am not trying to be the nation’s supernanny here— I enjoy a drink myself. This is about trying to get rid of ultra-cheap alcohol.

In my part of the country, we have shops that sell white cider with a maximum price. They are not allowed to sell it for over 23p a unit. I am afraid that is causing carnage. We have recently seen deaths of rough sleepers in my community, and rough sleeping is very closely associated with dependency. We know that as people start to lose control of their drinking, they start to target cheaper and cheaper alcohol. We know that the heaviest drinkers spend 40% less per unit on their alcohol. Just as with the smoking issue, this is not necessarily about saying that it is always possible to save everyone who has become a dependent drinker. We know that 40% of dependent drinkers will, whatever happens, be unable to control their drinking and will lose their life as a result of their dependency. It is about trying to help those who are starting to lose control of their drinking. It is about helping those who are right at the beginning of the journey, who may have developed a harmful pattern of binge-drinking.

The argument goes beyond the public health message and towards what alcohol dependency is doing to our communities. We know, for example, that there are 705,000 children in this country living with a dependent drinker—not just a hazardous drinker or a harmful drinker, but somebody who is dependent on alcohol. We also know that in 40% of child protection cases, alcohol is a key part of the problem. We know that nearly half of all violent crime is partly attributable to alcohol, as I know from my experience of seeing victims of crime and domestic violence. We know that a huge number of those who are victims of domestic violence report that alcohol directly caused or significantly worsened that violence.

We know that about a third of people feel that their town centres have become no-go areas to them at the weekend, and we know that all of us are paying for that. It costs us a staggering amount—about £21 billion a year just within our health service. I welcome the suggestion from the Secretary of State for Health that members of the Front-Bench team should spend time on work experience, and I suggest that suitable work experience for all members of the Front-Bench team would be a Friday night in casualty. If they really want to see what is causing delays in casualty departments at the weekend, they need look no further. Perhaps they would like to go out with the special constables in my area, who tell me that all their time at the weekend is spent dealing with alcohol-related crime and violence.

The final point that I would like to make about the subject is that it is an important cause of health inequality. To all those who say that minimum pricing penalises the poor, I would say that it is the poor who are suffering the most as a result of ultra-cheap alcohol. There are many reasons why we need to address the problem. If alcohol harmed only the individual who was drinking, that would be purely a matter of personal choice, but the wider harm is caused by the ripples that spread out from the individual who is losing control of their drinking, affecting those closest to them, their wider family and their community. So there are good reasons for saying that this is fundamental and core Government business.

I feel very disappointed that such a well-evidenced measure has been dropped from the agenda. It is not good enough to say, “We have not made a decision.” Continually kicking a ball down the road can, in effect, be the same thing as dropping it altogether. I hope that alcohol-related measures come back as “any other business” within the legislative programme.

I call on the Government particularly to look at the emerging evidence from Canada. Apart from the myth from the alcohol industry that such measures would make alcohol unaffordable, which is not the case, other myths are perpetuated. We need to challenge those. What we have seen clearly from states in Canada that have introduced a floor price is that following a 10% rise in the floor price there is a 32% reduction in deaths directly caused by alcohol. That is important evidence. There has also been a decrease in alcohol-related hospital admissions. Let us look at the evidence and have evidence-based policy, rather than listening to the power of lobbyists. It is vital that we look at the power of the alcohol lobby and the way that that operates at the heart of Government. I would like to see a register of lobbyists. I would like to see transparency about who is calling the shots when it comes to forming policy.

I sometimes get a little flack for using social media—surely not, Members might think—but if we look at the Chamber now, which of course is where Members of Parliament should be, we might consider that Twitter can reach parts that other tools cannot. In particular, if we look at the tools that others use, and at the power of the lobbying industry, we will see that MPs need to use every tool at their disposal to fight for the causes they believe in. Public health is fundamental to why I applied to be in this place in the first place, and it is fundamentally Government business. We should look at the evidence.

That is a very good summary. We want the best and freest press in the world, but we do not want a press that is marred and tarred by being seen to cosy up to the Government of the day, and compromised, and parties compromised. We do not want a press that does dubious deals with the police behind closed doors, not in the interests of members of the public. We want people to be able to be supported by the press, not trashed by the press. My hon. Friend is quite right.

No, please, many others want to speak.

For 20 or 30 years, colleagues of mine in both Houses have said that we needed an inquiry such as this—Lord McNally, Matthew Taylor and many others—and we have now had one. Whenever people ask me what I stand for, I do not invent my own words; I look at the little card that my party issues, which quotes from the preamble to our constitution. I hope that we have all done what one sentence of that preamble says, and that we have all today taken action

“to build and safeguard a free, fair and open society, in which we seek to balance the fundamental values of liberty, equality and community”.

Today is about getting the balance right. I think that we have corrected the balance and it is a tribute to all who got us here.

This morning was quite exciting. Last week’s papal conclave lasted only two days, but a conclave that had lasted five days, or even 10 weeks, brought forth white smoke this morning. I hope that I do not have to say this too often, but I commend the Prime Minister. We now agree on two issues—press regulation and same-sex marriage. It is getting to be a habit, so let us hope that he joins us on the bedroom tax and a few other things.

Does the hon. Gentleman agree that it was more white flag than white smoke? I agree with my hon. Friend the Member for Broxbourne (Mr Walker) that this is a sad day. This is more than just a toenail in the door of regulation of the press and we may live to regret it.

I am afraid that I am going to take the Prime Minister’s side on this. I think that the proposal is well crafted, necessary and sufficient and that more might have been harmful in the way suggested by the hon. Lady. Incidentally, I am not particularly in favour of Popes, so the white smoke analogy is almost irrelevant.

I also commend the leader and deputy leader of my own party, because they have driven resolutely towards a sane and sensible conclusion, which is what we are discussing today.

We have to read up to schedule 4 of the charter before it sets out to whom it applies. It states that a

“‘relevant publisher’ means a person…who publishes in the United Kingdom…a newspaper or magazine…or…a website containing news-related material”.

That is why it is so unrealistic, because websites can be set up anywhere in the world. My hon. Friend the Member for South Swindon (Mr Buckland) said that the code was voluntary, but if the stick being carried is sufficiently big, the code becomes coercive.

What will happen when our press become more bland and we see the people who read newspapers deserting them for the internet? Will the code apply to Twitter? There is a risk that we will abandon the printed press for the online news media, and what will happen then to our particularly vulnerable regional and local press? Who will be there to report from the courts? What will happen if our national press go into further decline and can no longer afford to fund world-class investigative journalism?

There has rightly been much talk today about the victims of the press, but we forget at our peril the victims of big pharma, of big corporations and of big state. I would far rather have our two-fingers-to-the-establishment, slightly out of control press than a nervous press, a bankrupt press or a bland press. Although we can all be commentators through the internet, we cannot all be investigative journalists. We owe a huge debt to investigative journalism and we should be very mindful of any threat to it. I hope that when he responds to the debate, the Prime Minister will feel at least that a free press deserves a free vote.

The hon. Gentleman makes an important point. Clearly, a health service facing growing demands requires growing resources. We are growing those resources, but they are limited, so at the same time we must meet the challenge of increasing productivity and cutting waste in our NHS, which we are doing. I do not believe that that should impact on patient care. Every public sector body has to look at how it can become more productive and efficient, but that must not be at the expense of patient care, and that is important for the future.

I join the Prime Minister in paying tribute to all those NHS staff who go to work with great care, compassion and vocation, but will he look in particular at one staff group—health care assistants, who deliver much of the day-to-day personal care in the NHS, yet have relatively poor access to training and development? They have no regulatory body, so if individuals are not acting with care and compassion, they can move on to another institution, and perhaps work unprofessionally there too.

My hon. Friend speaks with great knowledge about the NHS, with her long years of experience as a GP. On health care assistants, the Government have said that Robert Francis’s idea of proper training standards needs to be looked at. I tend to agree with that. The issue of registration is more complicated and potentially more bureaucratic. We will certainly look at it, but I think that needs some close examination.

We did not discuss international development at this Council, because it was largely about the eurozone, but we did discuss briefly the effect of European aid, particularly in Syria, where Britain is playing a key role, as is the EU, with its aid budget, which is making sure that we ease the scale of the humanitarian crisis, and that is good and important work.

Better tantric than premature—we have all seen where that got the Leader of the Opposition on Leveson. We are waiting breathlessly for my right hon. Friend’s speech in mid-January. Will he include in it a statement on the impact on employment in this country of an influx of Bulgarian and Romanian workers?

I do not think our rebate would last long with the Labour party. Tony Blair—the last Labour Government—gave away the rebate, in return for which they thought they had secured a promise for reform of the CAP, but they got absolutely nothing in return. It was a terrible piece of negotiation, and one, I am afraid, for which we are still paying the price.

Far from being isolated in Europe, the Prime Minister has plenty of allies. Does he feel it was at all helpful to be able to go to Europe and demonstrate the strength of feeling of this House? Will he set out when this House—and more importantly the British people—will be able to see his proposals for a new settlement on our relationship with Europe and when the British people will be able to give their consent?

I do not think that anybody in the EU doubts the very strong views of this House of Commons and of the British public about our relationship with Europe and the fact that we should not be having big increases in the EU budget. That is well understood and this Government reflect that very clearly, unlike the last Government, who endlessly gave away our money. I have explained that I will be saying more this year about the new settlement that we are seeking in Europe.

Today’s debate sends a very powerful message, and I congratulate the hon. Member for Croydon Central (Gavin Barwell) on choosing to present such an important Bill. It sends a message about recovery: that if a person has experienced a mental health problem in the past, that does not make the person unpredictable or dangerous, and that there should be no barrier to his or her full participation in public life or, indeed, any part of life. However, I think we also need to issue a call to arms. If we are to make people more confident about coming forward and seeking help at an early stage—confident that they will not be discriminated against later in life—we must ensure that primary care in particular is ready for them: that people who present their doctors with a mental health problem are taken seriously, and are given the support and help they need.

This has been a big week for mental health in Parliament. We have seen the launch of the Government’s strategy on suicide prevention as part of their overall mental health strategy, and we should pay tribute to that very effective strategy, which, again, sends a positive message. We should remind GPs, when thinking about suicide and depression, to ask their patients, “Are you depressed?”, to take the issue seriously and ask patients specifically whether they have had any suicidal thoughts, and then to deliver an effective treatment for them.

To anyone following this debate, I would say the same as my hon. Friend the Member for Broxbourne (Mr Walker) and the hon. Member for North Durham (Mr Jones): “You will find that people will feel positive about you and will encourage you, so do have the confidence to seek help and tell your friends; and when you have recovered, make sure that you tell other people that you have recovered, and support them so that they too can seek help.”

I recognise the subject that the hon. Gentleman raises, which he also mentioned earlier. I do not think that today is the right time for me to go into that matter in detail; the cross-party consensus might be affected if I did. We do need to get that process right, as we need to get right many processes of administration and welfare, in its broadest sense, across government. I am sure that my colleagues at the Department for Work and Pensions and the Department of Health will have heard his plea. As a member of the Government, I certainly want us to get that process right, and I will work with colleagues to achieve that.

To build on the point made by my hon. Friend the Member for Broxbourne, we need to work with partners on the mental health strategy. It is not possible for the Government to say, “It shall be so.” We need to ensure that the right outcomes are delivered locally and are driven by good evidence. We must cast the net wide to do that. The challenges are enormous and extremely difficult. That can be seen, on an individual level, when we have conversations on this matter in our constituency surgeries. However, the rewards of getting this right are vast. That is part of the point of today’s debate.

It is often quoted that at least one in four of us will experience mental health problems at some point in our life. What is less often quoted is that about half of people with a lifetime mental health problem experience their first symptoms by the age of 14. That is a startling statistic among the sea of statistics in this debate. By promoting good mental health across society and by intervening early, particularly in the crucial childhood and teenage years, we can help to prevent mental illness from developing and mitigate its effects when it does. Only a sustained approach across the course of life will equip us to meet the enormous social, economic and environmental challenges, and to deliver the benefits to the people who need them, which is why we are here today.

When mental health services work well, they work well with the public sector, the private sector and the voluntary sector, and they help people to overcome disadvantage and to fulfil their potential. Any action on mental health, from a Government or otherwise, is not only a mental health strategy but a social justice strategy. I know that that is what all of us here today stand for.

Does my hon. Friend agree that this matter is the responsibility not just of Government, but of non-governmental organisations? Only this week, the Health Committee saw the disappointing example of how the chair of the Care Quality Commission and that organisation sought to stigmatise a member of their own board with regard to what they alleged to be a past episode of mental illness.

My hon. Friend is right that we need to encourage cultural change across a number of organisations. I hope she will forgive me for not responding to that example, because I was not there to see it, but we need to ensure that such stigmatisation is not possible, is not the done thing and is frowned on whenever it is seen or experienced. We need to stand up and speak up for that view, and whenever we can we need to frown on that stigma from a front row seat. One of the six objectives in the mental health strategy was exactly that—that fewer people will experience stigma and discrimination. As the House will know, and as my hon. Friend the Member for Croydon Central set out, the strategy has the full backing and endorsement of the whole Government. My right hon. Friend the Deputy Prime Minister has a long record of calling for the reform of mental health policy.

For negative attitudes and behaviour towards people with mental health issues to decrease, we need to improve public understanding of those issues and gain more sympathetic treatment of them in our mainstream media. Again, I refer to the excellent work that my hon. Friend the Member for Broxbourne did in June, has done since and will do in future.

I also pay tribute to my predecessor, my hon. Friend the Member for Forest of Dean (Mr Harper), for the work that he has done. He has spoken passionately and often on these proposals, and he said in an interview with politics.co.uk in June 2009:

“Mental ill-health is still very much a taboo subject in Parliament as well as the work place and this must change. Mental ill-health affects as many as one in four of the working age population and it is crucial that Parliament leads the way in promoting a better understanding of mental health.”

That is still pertinent today, notwithstanding the steps that we are taking to ensure that the mental health taboo is well and truly broken. Momentum has been building behind the measures in the Bill for some time, as many hon. Members have shown in their comments today and their actions over time both inside and outside the House.

I welcome the hon. Lady’s support, hoarsely delivered as it was—she has a cough. It is crucial to wait to hear from the official Opposition what their attitude is to the programme motion. Will they not accept any form of programme motion, or do they have suggestions of their own on the number of days required to deal with the legislation? The Government have been very generous already.

The Deputy Prime Minister spoke earlier of the need to reform the other place to make it fit for the 21st century. Does he accept that science and technology are very much part of our future? Will he accept an amendment that would mean greater recognition of expert Cross-Bench expertise in engineering, science, technology, maths and medicine? In addition, I am very happy to explain the correct use of the term “lobotomy”.

We can have precisely that debate and a multitude of others on the detail of the Bill as long as we make progress on Second Reading and the programme motion this week. As the hon. Lady may well know, the appointments commission envisaged in the Bill will be statutorily required to ensure proper diversity and representation of expertise in the 20% of non-elected peers in a reformed House of Lords.
