Commons ChamberI agree. That is why we are increasing fourfold the number of inspections, so that water companies are not marking their own homework. It is why we have the plan for water, introduced by the former Secretary of State, my right hon. Friend the Member for Suffolk Coastal (Dr Coffey), and significant additional investment. It is why we are taking tougher enforcement action, with the biggest ever criminal prosecution of water firms by the Environment Agency. It is also why we are taking action on things such as bonuses for companies that commit serious wrongdoing.
Food security is more important than ever, which is why we need to back British farmers to keep putting food on our tables, while protecting the environment. We are supporting farming with £2.4 billion of annual spending, an average boost of 10% for the sustainable farming incentive payment scheme, and new rules to ensure that farmers get a fair price for their products.
Obviously farmers in low-lying areas of Lincolnshire are suffering from flooding, so any update on the support that they can be given would be very helpful. I have a specific question for the Secretary of State: will the Government update us on when they will announce the long-term funding solution that they promised for internal drainage boards, to address the pressure on local authorities through special levies?
My right hon. Friend is right to focus on the importance of drainage boards, particularly in Lincolnshire. He knows that I have a particular constituency interest in the adjacent area. We have announced £65 million of funding, and the Minister for water, my hon. Friend the Member for Keighley (Robbie Moore), will make further announcements on that shortly. We are looking more widely at the huge pressure on farming from the wet weather, particularly in areas such as Lincolnshire. There has been a 60% increase in rainfall—these have been our second wettest six months—and we are looking at a series of easements, particularly with regard to SFI, to ensure that farmers get their payments.

Commons ChamberAccording to the King’s Fund, the proportion of GDP taken by the NHS has increased in the past 50 years from 3.4% to 8.2%. On the same trajectory, in 50 years’ time, it will take a fifth of all our GDP. That is totally unsustainable, especially as someone’s only right, despite the fact they are paying ever increasing amounts of tax, is to join the back of the queue. I ask again: will the Secretary of State launch a study—and, if necessary, appoint a royal commission—on fundamental reform of the whole nature and funding of our health system, so that we can learn from every other developed country, such as Australia, France, Italy and Germany, where they unleash private sector investment into healthcare and give people rights to their healthcare, while ensuring that those who need it get free healthcare at the point of delivery?
I hope my right hon. Friend is pleased to see the measures we are taking with the Lord O’Shaughnessy review on clinical research trials to make it easier and faster to do research in the NHS. That in turn attracts private investment to the NHS. He will have seen the announcement I made on Tuesday of £96 million for 93 different research projects, such as at Great Ormond Street Hospital, where we have allocated £3.5 million for research into rare conditions in children. That translates into research that is then deployed, usually in adults. We are investing there, and we are screening 100,000 children through Genomics England. We have got a deal with Moderna and BioNTech so that we can have bespoke cancer vaccines. On Monday, we rolled out national lung cancer screening. Previously, in our most deprived communities we were detecting lung cancer late—80% were diagnosed late—but in those pilots we turned that on its head with 76% detected earlier.
I know that my right hon. Friend, as a former Chair of the Public Accounts Committee, will agree that by detecting earlier, not only are patient outcomes far better but treatment is far cheaper, whether that is for lung cancer or through our innovation on HIV screening in emergency departments picking up HIV in people who do not realise that they have it. When we treat it early, the patient outcomes are better, and it is fiscally much more sustainable. That is how we will address some of his concerns.

Commons ChamberClearly, an increase in population in a specific area will have an impact on the health needs there. I recognise the concern that my right hon. Friend raises, and I will ask the Minister for Primary Care and Public Health to follow up with him on this important point. While the NHS is well equipped to deal with short-term pressures, this issue highlights the importance of the Prime Minister’s commitment to stop the boats and the Government’s overall strategy on illegal migration.

Commons ChamberAs the record shows, numbers have been increasing. There are 37,000 more doctors and 52,000 more nurses within primary care than in 2010. We have already reached our manifesto commitment on additional roles in primary care to deliver more appointments. We have repeatedly said that we will bring forward a workforce plan and we are committed to doing so shortly.
I congratulate my right hon. Friend on promoting more choice than in the socialist republic of Wales, which is a pretty low bar, but can we go further and promote real choice by adopting two previous Conservative policies? Both would be wildly popular. The first is a patient passport, by which a patient could get a free operation on the NHS, or take the same cost to a private or charitable hospital, which would promote choice and accountability. The second is tax relief for private health insurance, which is a matter for the Chancellor, but the Secretary of State could have a quiet word with him.
My right hon. Friend has lobbied me on this issue a number of times, including outside the Chamber. As such a senior parliamentarian, he well knows that tax is a matter for the Chancellor, who I am sure will have noted his wider point.

Commons ChamberThe very purpose of today’s announcement—I have made it on the first day that Parliament is back—is to give that urgent uplift in funding to local authorities and ICBs so that they can act now, knowing that that funding is available. They have the additional £500 million, which is ramping up as well. That is part of a wider package of measures—NHS England putting in community support with 7,000 more beds—but the purpose is to recognise the very real immediate pressure the frontline has been under. It also needs to be viewed as something that other healthcare systems across the globe have faced: a very sudden and very significant spike in flu seven times higher than last month and 100 times what it was last year.
Yes, but they also have covid and flu in France, Germany, Italy, Sweden and Holland. Winter after winter, they cope far better because they have much more integrated social insurance systems. Some people like me have been banging on about this for years, but now the former Health Secretary, my right hon. Friend the Member for Bromsgrove (Sajid Javid), is suggesting a social insurance system, as is newspaper editorial after newspaper editorial. What is our long-term plan? We cannot leave the Labour party to have a long-term plan while we do not. How are we going to reform this centrally controlled construct? People of my age have paid taxes all their life and their only right is to enjoy the back of a two-year queue! What is the Secretary of State’s plan?
First, integrating health and social care through the integrated care boards. That is what we put in place from July, recognising that actually the pressures on the NHS are often as much about pressure on social care as they are about pressures in the NHS itself. In particular, if we look at ambulances, we see that often it is the delay in domiciliary care that is driving the blockage on the wards, which in turn applies there. Secondly, it is recognising that there are workforce pressures, which is why NHS England has been working on the workforce plan that has been set up.
Thirdly, we have already set out our elective recovery plan. Over the summer, the longest waits—those of over two years—were largely cleared. [Interruption.] Opposition Front Benchers chunter, “How’s it going?” Let us look at how it is going, compared with the Labour Government’s two-year clearance in Wales. Before Christmas, there were about 60,000 people in Wales who had been waiting for more than two years; in England there were fewer than 2,000. We are making progress on the longest waits through the work of Jim Mackey, Professor Tim Briggs and Getting It Right First Time. We are innovating with the surgical hubs and the community diagnostic centres. That, in turn, gives greater resilience to the electives that used to be cancelled when there was winter pressure. With hot and cold sites, they are much more resilient.
Finally, I must take issue with what my right hon. Friend says. In France, Germany, Canada and many other countries, the massive spike in flu and covid pressure, combined with pressures from the pandemic, has placed similar strains on healthcare systems. It is simply not the case that the issue affects England alone.

Gentleman skirts over the fact that it is this Government who have opened five new medical schools, who have significantly boosted medical undergraduate training, who are investing more in our NHS as per the long-term plan, who have invested a further £36 billion as part of our covid response, and who are investing in technology and the skills of the workforce as a whole. We are boosting the number of medical trainees and—I touched on this question earlier—we are also boosting the number of doctors in training to be GPs.
My right hon. Friend raises an important point about face-to-face care, which matters to many constituents, but I gently remind him that in the Lincolnshire integrated care system, 71.9% of appointments were face to face in September 2022. It is not the case that every patient wants a face-to-face appointment; there can be instances in which an online service is better. For patients facing domestic violence, for example, it can often provide a much more convenient service.

Commons ChamberFirst, the approach of the United Kingdom Government to these Scottish payments is exactly the same as applied recently in Wales. To further reinforce the point that I made a moment ago, while decisions on whether to exempt these payments are reserved, the Scottish Government will keep all the income tax receipts from these payments, so if they wish NHS and care workers to receive £500 net of tax, which is what they say is their wish, they can simply increase the value of the payments going to them. That is the point of substance. That is the point they do not want to engage on.

As the Minister responsible for Government spending on behalf of the Chancellor, I would want to look closely at why the school in the hon. Gentleman’s constituency is saying that that very significant uplift in funding for schools last year does not appear to be reaching the frontline. The education funding settlement in the 2019 spending round should more than cover the cleaning costs. I will happily look at that, but if he looks at the funding settlement allocated in SR19, I think he will accept that it was a very generous one.
It is not surprising that more and more Members are calling for more Government support, because the Government are forcing more and more businesses, particularly in the hospitality sector, out of business. The Chief Secretary says that his priority is to help business. The best way to help businesses is to let them get on and do business. We are going bankrupt as a nation—there will not be the money to pay for the NHS or pensions. What is the Treasury doing to row back against other parts of the Government and insist that we must allow British business to operate? He did not answer the question from the Chairman of the Select Committee, my right hon. Friend the Member for Central Devon (Mel Stride)—what is the scientific evidence for pubs closing at 10 o’clock? Is he leading the fight to help Britain to stay in business?
With respect, I did answer it. I pointed to the projection given by the chief medical officer and chief scientific adviser at that time, the SAGE guidance and the fact that the package of measures put in place by the Prime Minister has resulted in a lower infection risk. The CMO and others would recognise that this is a range of measures. My right hon. Friend says that the Government have gone too far and that there is no evidence for the curfew. The tenor of most of the questions one gets is that we have not moved far enough and should be taking more drastic actions. That speaks to the fact that this is a balanced judgment. One needs to look at the range of measures we are taking, and that is what I would refer him to.

We have provided significant funds to Northern Ireland—an additional £2.2 billion—to cope with the pressures of the pandemic, and that has enabled 300,000 jobs in Northern Ireland to be protected through the furlough scheme, along with an additional 78,000 jobs through the self-employed income support scheme. Indeed, the package of measures that my right hon. Friend the Chancellor announced in his winter plan applies in terms of additional support for Northern Ireland as it shapes its response to the pandemic.
Does the Chief Secretary agree that the best way to help business is to let business get on with the job, as free as possible from Government control? Will he note that when the Treasury argues against further lockdowns for business, scores of Tory MPs and tens of thousands of businesses cheer it on? After all, to quote the Chancellor, is it not our “sacred” duty to “balance the books”? What is the point of solving this problem by borrowing money? Is that not the socialist way? What would be the point of a Conservative Government if we did that?
As a former Chair of the Public Accounts Committee, my right hon. Friend is quite right to focus on the importance of value for money and protecting the interests of the taxpayer. He knows me well enough to know that I share that sentiment. On our wider response, it is important that we get the right balance between responding to the virus and doing so in a way that is supportive to the economy. It is a false choice to see this as a choice between health and economics; they are clearly intertwined and we need to work together in shaping our response.

May I first welcome the constructive tone that the right hon. Gentleman has struck? His offer to come to the Treasury might contravene some of the recent social distancing requirements, but I appreciate the spirit in which it was made. He is right that we need to move at pace and to work together. That is why my right hon. Friend the Chancellor was involved in further meetings this morning, as he will be later today, as we work through how to take this forward.
The Chancellor was drawing attention to the complexity of the target population. I think that a number of Members would have concerns, not least as we look to the future, if we were subsidising some very wealthy self-employed people. I take the point that they are not the ones getting in touch with the right hon. Gentleman, but it is important that our approach is mindful of the target population.
The right hon. Gentleman raised the issue of reassurance, which is a legitimate concern, and one shared across the House. I draw the attention of his constituents, and those of colleagues across the House, to the Chancellor’s comments this morning. We are working at pace on this and we recognise the issue being raised. I hope that provides reassurance, certainly in terms of an announcement, although the operation of any solution may take further time, as the Chancellor set out.
Considerable work is being done, but the population is complex. We are looking at the burdens of different delivery mechanisms, whether on the Department for Work and Pensions or local authorities, which have their own staffing pressures because of the number who are ill. That is why we are exercising flexibility in lots of other areas in order to reprioritise resources, but it is important that the scheme is deliverable and mindful of the other challenges we are dealing with.
May I make a point from a public accounts point of view by urging the Chief Secretary to the Treasury to consider a system that is humane, rapid and, above all, simple? The Treasury and the Department for Work and Pensions have traditionally been obsessed with complexity, targeting and clawback, but what we need is a simple system. About 40 years ago I suggested having a universal basic income to Mrs Thatcher, and I got an earful for my pains. But we need something like that, which could be rolled out very simply and claimed by taxi drivers, cleaners and those sorts of people, because rich people would not bother with it. So just get on with it, make it simple and do it now.
My right hon. Friend wants us to get on with it. I refer him to the meetings and the considerable work being done to allow us to get on with it. As a former Chair of the Public Accounts Committee, he well knows that many policy ideas start with the simple but then the devil is in the detail of delivery. I recall many an interview that he has given to draw attention to simple schemes that were then less simple in their delivery. It is worth bearing in mind that a small number of self-employed people—a very small proportion—might be doing quite well in the current climate, while many others are suffering, but that is not what we are focusing on now. The question that we are seeking to address is how we target our measures at those who are most deserving, which is what the attention of the House is focused on, and we need to ensure that the scheme that is brought forward does likewise.
