Commons ChamberIt is a pleasure to follow the hon. Member for Brighton, Pavilion (Caroline Lucas). While we do not always agree on everything, there are many things that we do agree on. I, too, would like to see the mass scale-up of renewables, but I also want to be pragmatic about where we are, some of things that we need to do, and what the Bill does.
The issue of energy is one that impacts every person in the UK, from the one-bed flat owner to the 200-bed hotel owner. The ability to secure safe and reliable energy is essential, and spiralling costs are having an effect on the day-to-day lives of people throughout the UK.
I think it is important that I make this point and perhaps explain where I am personally on the matter. I grew up, and others in the House are probably the same, in an age of throwing another jumper on. My children used to laugh at the stories I told them of how cold it had to be before the heating was turned on. For us today, it is no longer a laughing matter for many people. My researcher, who owns a modest home, had to put more than £250 of gas into her home in December. She was at home during term-time with the children. She tells me that the gas was not even running constantly; it was turned off whenever she went to visit her parents or her husband’s parents or went out. I thought of how much more a wee—I use that word often in the House as a descriptor—widowed pensioner would be paying in their home when they are there almost all day, every day. That figure is not one that their pension and a single winter fuel payment could cover. There are only so many jumpers that someone can put on, and a jumper does not help with a damp wall.
It is clear that the cost of energy dictates what steps we take to secure the current energy supply, while also striding for new alternative renewable energy sources. As many in this place will have heard me say on numerous occasions, tidal energy in Strangford lough and other such areas needs a great deal of funding. The pilot scheme worked, but it was at the wrong time because the cost of the energy that it produced was not financially feasible, but it would be now. If we can harness that power, which is as reliable as the sun rising in the morning, we are on to a winner. However, I understand that that is not the point of today’s debate and will leave it at that.
The Secretary of State for Energy Security and Net Zero is in her place, and I know the Government have taken giant steps to meet the net zero targets and have committed themselves to green energy. I wish to put on the record that I am committed to the same targets as well, but we need to strike a balance. The balance that I put forward to the House—and I wish to make a declaration—is from my point of view as a farmer. We own land. My neighbours are all dairy men or farmers of beef, cattle and sheep. They are willing to commit themselves to the net zero targets because they see that net zero is something that must be done. As part of that target, they must reduce the number of their animals. That is not possible to do while continuing to have a feasible and financially viable farm. There are other things that the farmers wish to do.
In the countryside where I live there are not enough electric vehicle points, so people do not buy an electric or hybrid car, probably because all the EV points are up in Newtownards. I drive a diesel vehicle—have done all my days and, if I am spared, will probably continue to do so because I believe that it is a choice. It is a choice we may not wish to make every time, but is one that we have to make because electric cars are just not feasible because the EV points are just not there.
Does the hon. Gentleman agree that the National Farmers Union has been positive on many of these issues? Does he agree with what it says about hydrogen being a source of sustainable power in the future, and that it is coming soon?
I thank my hon. Friend—I call him that because I have great respect for him—and agree wholeheartedly with what he has just said. The farming community, the NFU and the Ulster Farmers’ Union are clearly committed to the targets. They are committed to looking at the alternatives, but the alternatives have to be practical. The point I am trying to make is that it is about where things are practical.
There is no doubt that to meet not just our net zero target but, more importantly, our environmental obligations, we need to do a better job of accessing and using renewable energy fuels. However, the fact is we will simply not be there any time soon and, in the meantime, it is vital that we secure safety and security for our constituents. I support the aims of the Bill, which would enhance the procedures currently in place, and note that no financial hardship should be passed on through the Bill. That is vital as I know that households are struggling with the current pressures. No longer is it a matter simply for households in poverty, working families with decent wages are being affected.
It is a transition. It is about meeting our net zero targets and increasing green energy and renewables, which my hon. Friend the Member for Brighton, Pavilion (Caroline Lucas) referred to. At the same time, the Bill gives us the opportunity to progress those renewables in a way that is positive in the short term.
The Library briefing makes it clear that licensing rounds are run when the NSTA decides they are necessary. However, it should be highlighted that they have been held broadly on an annual basis up to the 32nd licensing round, which opened in 2019. The latest—33rd—licensing round was launched in October 2022, following the introduction of a climate compatibility checkpoint in September 2022. In October 2023, 27 new licences were awarded as part of that licensing round. That is not onerous, but it is necessary not just to safeguard our industry by enhancing investor and industry confidence, as the Government have highlighted, but to ensure that we do not see families scraping pennies together to afford heat.
My contribution to the debate is clearly for those who are in energy difficulties. Today, the papers referred to food bank referrals being up some 30%. The food bank in my constituency of Strangford in my major town of Newtownards saw a 30% increase in referrals over December and early January from people who are middle class who are finding it difficult to deal with energy prices.
I know of several young families who usually enjoy a few days away when the kids are off at Christmas, and they told me that they were just not able to do it this year. People may say, “For goodness sake, they can’t go on holiday…” I am not saying that because it is their right to have that break, but I am highlighting the knock-on effect for families of increased prices is that they cannot afford to sow into the local economy in the way they used to. That means the little 20-bed hotel they usually visit does not get their business. The knock-on effect is that they do not hire the cleaner for as many hours. Her income drops, and she cannot spend the way she usually does, so the knock-on effects continue.
We need the people who spend locally to do so, and for them to do that, energy bills need to be manageable. We are failing when it comes to energy provision. If the Bill helps safeguard our provision as we continue to find better ways to source reliable renewable energy, I support that. When the Minister or Secretary of State sums up, if they could give us that reassurance, I would be a whole lot happier about this debate. Of course, we need to explore tidal energy, but safeguarding domestic production can go hand in hand with that. Indeed, it must do so. I am committed to renewables, green energy possibilities and net zero targets because the farming community that I live in want to commit themselves to that as well.
I support our families, our vulnerable, ill and elderly, and those living in cold, damp homes because they cannot afford to do otherwise. Therefore, at this stage, I support the Bill on behalf of all those struggling to heat their homes and keep their families warm. We must commit ourselves to more renewables and ensure that the renewables percentage rises. If it rises, we can reduce gas and petroleum usage. By doing so, we can balance the process. That is what I am hoping for from the Minister’s reply; I hope we can deliver that.

As with so many issues, that barrier to trade is not logical, but then when did anything logical come out of the EU? I say that maybe a wee bit cynically, Ms Elliott. There may be a few others here who agree, and there may be some who would say, “No, that’s not entirely correct.”
If the hon. Gentleman intends to talk about Huddersfield, I am not sure, but I give way anyway.
We will be divided on membership of the EU and the wisdom of leaving it, but does the hon. Gentleman agree that what we are debating—air quality, the marine environment and support for species—is something that we have to work with the rest of Europe on?

I refer to my declaration in the Register of Members’ Financial Interests. It is a real pleasure to serve under your chairmanship, Mr Hollobone. Air pollution is one of the greatest public health challenges of our time. As we speak, in this place and beyond, people are being poisoned by filthy, unsafe air. Indeed, today the Mayor of London issued a high air pollution alert across the capital.
A diesel particulate filter, or DPF, captures and stores dangerous emissions. It can be found at the back of a diesel exhaust system and can reduce emissions from a vehicle by around 80%. In some instances, a faulty DPF is responsible for the same amount of pollution as a three-lane, 360-mile traffic jam. That is the distance between my constituency of Huddersfield and Land’s End in Cornwall. That truly terrifying fact must spur us on to identify and remove dangerous faulty filters. I emphasise that just one faulty filter in one car can spread that amount of poison.
I thank the hon. Gentleman for giving way. He has been quite insistent and persistent in highlighting this issue. He said that a faulty DPF on a single vehicle can cause the same amount of pollution as a 360-mile traffic jam. Does he agree that while we are putting fresh restrictions on business and manufacturing, there is a simple and effective way of cutting emissions? If so, will he put forward his ideas?
I thank the hon. Gentleman for his intervention and he is absolutely right; I will address his point during my remarks.
Increased particulate matter testing during the MOT would ensure that we identified faulty DPFs that are not picked in the current testing regime. I am pleased to see, from the Government’s open consultation on the MOT, which was published last week, that the Government want to adopt particulate number testing.
As chair of the Westminster Commission for Road Air Quality, I have been campaigning on this issue for a long time, and it looks as though we are making some progress at last. If the Government are looking for a legislative vehicle, my Motor Vehicle Tests (Diesel Particulate Filters) Bill is due for Second Reading on 24 March. I am very happy to share it, and all credit for it, with the Minister.
Before I speak more about changes that we can make to the MOT, it is worth dwelling on the life-changing harmful effects of air pollution, which my Bill would help to mitigate. It is estimated that up to 36,000 people die prematurely each year from the effects of air pollution in our country. The total cost to the NHS and social care will be £1.5 billion by 2025 and £5.1 billion by 2035.

I totally agree. I got into some trouble in certain quarters when I said that early on in the conflict; it was not something people wanted to hear at the time.
The Minister will know that I have consistently challenged the Government on this issue—I have form. Over the past 10 years, I have warned against cuts to the size of the Army. In 2013, I said that cuts to capacity would seriously restrict the ability of our country to defend itself. At the time, with the number of armed personnel at around 140,000, I felt as though we were retreating from being a significant player in the western alliance. In 2016, that number went down to 100,000, and there I was in the House, warning the Government that their course of action simply was not the right one. We now face the grim reality of soon having a limited capacity of 72,000 armed personnel. The fact of the matter is that those numbers are nowhere near good enough for a key player in NATO. As the Minister knows, I usually engage in constructive criticism, but it is crystal clear that the Government have their heads in the sand on this issue—or more specifically, the Prime Minister does.
I congratulate the hon. Gentleman on securing this debate. He is absolutely right: as of January 2021, the RAF figures were down by 6%, and the Army and Royal Navy figures by 5%, so we clearly have a problem. We also have increasing demands on our NATO commitments across Europe and elsewhere. We now have Sweden and Finland coming into NATO, which will strengthen it. Does the hon. Gentleman agree that one way of increasing the numbers might be through the Territorial Army and the reserves? I have always campaigned for extra soldiers to be set aside for Northern Ireland, where recruitment is high, but we have not seen those numbers yet.
My old friend nearly always intervenes on my speeches—on everyone’s speeches—in a very constructive way. I agree with much of what he says, and I will come to that specific point on recruitment in a moment.
The Prime Minister is a great fan of Churchill. In fact, I picked up his book on Churchill for £1 the other day—it was quite a bargain. Then, it was shown on television, and I was asked why I had it on my bookshelf. It is quite an interesting book actually. The Prime Minister is erecting a Churchillian façade, but the truth is that he has found himself at odds with NATO by reneging on his manifesto commitment to keep defence spending at 0.5% above inflation. He has given up on that, which is not acceptable and puts us all at risk.
The Minister knows that I find the Secretary of State to be a breath of fresh air. I like him, I work with him and I think his was a very good appointment. He is in the wrong party—never mind—but we agree on many fundamental issues, such as wanting to see the Prime Minister reform his approach to defence spending. The Secretary of State has rightly been calling for increases in personnel numbers. However, that raises the question: why was the defence Command Paper so quick to make those cuts in the first place?

Commons ChamberThe maiden speech from the hon. Member for Wantage (David Johnston) was inspirational. I liked what he said—it even made me want to come to Wantage and have a look round, rather than just changing trains there. Given what he said about social mobility, there are all kinds of cross-party groups that I immediately want him to join so that we can work on those issues together. I am sure that he will make a great contribution to the House in the years to come.
I made my maiden speech quite some time ago. I am the longest continuously serving Member on the Opposition Benches. I hate the term Father of the House; I am a senior Member. I dimly remember my maiden speech, and I want to return to the theme of that speech. We had had a bitterly divided general election. We do all the usual ritual things in the two parties—the Government and the Opposition—and we say pretty cruel things about each other, but we have a responsibility. The Government have a majority and a mandate for five years, so we must all get on with representing our constituents and working in the best way possible, which they would admire.
I notice when I knock on people’s doors that many of my constituents are fed up with the knockabout nature of this place. It is always them against us; “We are totally right, and you are totally wrong.” I have spent a lot of my career in Parliament—I served for 10 years as Chair of the Select Committee on Education—proving that by working cross-party we can make a difference. Working together we can come up with all sorts of good ideas, innovations and good policies that any party can adapt and adopt, and make a difference to our constituents.
We all want the very best education system in this country. We all want the best policy making possible. Many of the things that we have to do over the next five years will need cross-party participation, and I hope that we can secure that. There is a lot of ground for partnership in education in particular. I have been involved in a number of cross-party initiatives. I was asked to be involved in the new look at apprenticeships. There was a commission of inquiry, and we took evidence—I gave evidence too—and for the first time we secured an employers’ levy for training and apprenticeship. It was a big step forward, with cross-party support. Who would have thought that a Conservative Government would introduce that? I admire that initiative. There are some problems that have to be sorted out, because the levy is not working in quite the way we had intended, but it is right in purpose and we just have to get it right in the detail. Certain training providers are being squeezed out, and some of the levy is going to places we did not quite expect it to go to. We have to refine and improve all policies, and I will certainly work to improve that one, because the work that has been done on apprenticeships is inspirational and making a great difference to so many people.
I want to push one particular point today, and that is the fact that all of us, of whatever party, know that we do not have a bad education system. It goes back to 1880, when we first introduced compulsory education for children aged under 10. All those years later—140 years—a lot of children in our country are getting a pretty good education. I have three daughters, a son and 12 grandchildren, so I have a pretty good view of what is going on, from my seven-month-old granddaughter to my 18-year-old granddaughter and lots in between; I am pretty well connected with the market. A lot of people are getting a good education and standards are going up, which I applaud. The right hon. Member for Harlow (Robert Halfon), who is just leaving the Chamber, is an old friend of mine, and he and I agree absolutely that our standards have started to improve steadily, even with respect to the international comparisons.
I congratulate the hon. Gentleman on his contribution so far. He mentioned the international dimension. One of the great advantages of our education system in the United Kingdom of Great Britain and Northern Ireland is the offer to overseas students. Does he recognise the contribution that overseas students make to the education system across all regions of the United Kingdom, with partnerships, and the exchange of cultures and our educational standards? Does he also recognise that many countries across the world wish to send their students to the United Kingdom to get our education, because it is so good?
I absolutely take that point. When I chaired the Education Committee, we did an inquiry on the importance of overseas students, who provide roughly 10% of the income of universities. That is a massive contribution. Universities are so important in our communities. The University of Huddersfield makes a remarkable contribution and is the largest employer in my constituency, and those of us who have universities in our towns and cities know just how lucky we are. Thank goodness we have universities; long may they thrive and survive. The right hon. Member for Haltemprice and Howden (Mr Davis), a former Secretary of State for Exiting the European Union, mentioned the importance of research, and that point was absolutely right.
Let me return to the broader education system. We usually judge the success of our education through evidence-based policy. How do we drive it? How do we assess it? Is it evidence-based policy? Is it going in the right direction? Are we using public money in the right way? On the other hand, we obsessively look at other countries all the time to see if we are as good as them. The programme for international student assessment, and all that, is very important. There are deficiencies in the PISA methodology, but the fact is that those are the methods we use.
Generally, yes, our education system is improving, but a substantial proportion of children in this country are not getting the education or the opportunities. They are in particular constituencies, and we know which ones; we have all the bruises, cuts and everything else. [Interruption.] I am coming to the end of my speech. We know where those underperforming areas are and we have the methods—with passion and leadership—to sort them out. Our country is divided, and the Children’s Commissioner said only three months ago that 20% of children leave school with no qualifications at all. If that is the divide in our country, we have to understand that underperformance on a cross-party basis. We know the way to tackle it—and together, let’s do it.

It is a pleasure to serve under your chairmanship, Mr Robertson. I thank right hon. and hon. Members for attending the debate, particularly the Scottish National party spokesperson, the hon. Member for Linlithgow and East Falkirk (Martyn Day); the shadow Minister, the hon. Member for Great Grimsby (Melanie Onn); and the Minister. I look forward to positive contributions from all those able to participate.
I am grateful to have been allocated the debate. As chair of the all-party parliamentary group for healthy homes and buildings, I am delighted to have the opportunity to raise awareness and concerns about the cost of unhealthy homes to the NHS. I thank the background staff, who are in the Gallery, who have given information to us all, myself in particular, to help us in the debate.
Our APPG was created to shed light on the many problems caused to our nation’s health, wellbeing and economy as a result of people living and working in unhealthy homes and buildings. Given that most of us spend some 90% of our time indoors, it stands to reason that our homes need to contribute positively to our physical and mental health and wellbeing, not diminish it. This debate is so important because it pulls together the critical issues. It is perhaps not a normal Westminster Hall debate, but it pushes very much to the fore the effect of the homes that we live in on our health and, ultimately, on the NHS.
The APPG, following a weight of written and oral evidence received, launched a report, “Building our Future: Laying the Foundations for Healthy Homes and Buildings”, in October last year. It was a well addressed report, to which there were many contributions, and it brought together those with a deep interest in homes and those with a deep interest in health issues. It is good to have the report finished.
I have given the Minister a copy of the report, which contains a series of excellent recommendations that are helpful to the Government and will help us to move forward. I hope the debate will be a turning point, and that those recommendations will lay the foundations for change. The report sets out what needs to be done to ensure that new and existing homes do not cause or exacerbate health problems, because they often do. Many of us here, as elected representatives, will have people coming to us every week to complain about their home and, more often than not, the health problems related to that.
This debate is long overdue. It is time to raise awareness of the extent of the problem, and to recognise the human cost to the United Kingdom of Great Britain and Northern Ireland of doing nothing. It is clear from the White Paper that unhealthy homes cost the economy and our society each and every year. Living in or occupying unhealthy homes directly and negatively impacts on human health. Unhealthy homes that lack daylight, or are cold, damp, poorly insulated, energy inefficient, overcrowded, noisy, badly designed and generate poor indoor air quality can, in the extreme, lead to unnecessary deaths.
The hon. Gentleman is an old friend of mine. I think he knows that those of us who campaign on carbon monoxide poisoning really welcomed his excellent report. I have lost three constituents to carbon monoxide poisoning, which is one symptom of an unhealthy home. I assure him that we will work closely with him to ensure that no more people die of carbon monoxide poisoning.
The hon. Gentleman is absolutely right. My hon. Friend the Member for East Londonderry (Mr Campbell), who is sat to my left, also had constituents who passed away a few years ago due to carbon monoxide poisoning. That was in a holiday home, but it was none the less a problem. We in the APPG will take the comments of the hon. Member for Huddersfield (Mr Sheerman) on board, and we look forward to working with him.
Let me detail some of my concerns arising from the evidence that we heard. The effects of poor housing are estimated to cost the NHS £2.5 billion per annum; that rises when we consider all housing throughout the United Kingdom of Great Britain and Northern Ireland. The true cost lies in human misery and lives lost. Some of the figures are quite extreme, but they underline the issue. Some 43,900 excess winter deaths occurred in England and Wales in the winter of 2014-15, with cold homes causing one fifth of those. That is more than the number of deaths caused by road accidents, alcohol or drug abuse, which puts into perspective the need to make sure that homes are healthy. Children in cold homes are more than two times more likely to suffer from a respiratory problem. Cold homes increase the incidence of cold and flu, and worsen conditions such as arthritis and rheumatism. Again, we see that every day in our constituencies.
One in four adolescents living in a cold home is at risk of multiple mental health problems, so we are not always talking about physical issues; there can be emotional and mental issues as well. Those in poor-quality homes that lack effective ventilation suffer from indoor air pollution, which has been linked to allergies, asthma, lung cancer, chronic obstructive pulmonary disease, cardiovascular disease and, more recently, dementia.
If only we had that power! That is not to take away from the importance of the issue of social housing, which I will touch on later. Let us be honest: many people go into the housing that their pockets allow. As a result, they end up in housing that is not particularly in the right category, the right condition or the right shape. The hon. Gentleman is right that the benefit cap also dictates where someone can go. I will give the Minister plenty of time to get her thoughts together on that. However, that is an important point, and I will touch on it later.
Poor indoor air quality has an annual cost to the UK of more than 204,000 healthy life years. It causes thousands of deaths per year, and gives rise to health costs in the order of tens of millions of pounds. One third of people in the United Kingdom suffer from mould in their homes and are at increased risk of respiratory problems, infections, allergies and asthma. Just last week, I saw three constituents with mould growth issues in their houses—mould not caused by condensation, but ingrained in the walls. Sometimes ensuring that the housing associations or housing executive take those issues on board is quite a job.
There are more than half a million overcrowded households. The issue affects one in 10 children—something we cannot ignore. Overcrowding is linked to health and development issues, including meningitis, respiratory conditions, slow growth rates, accidents in the home, stress, anxiety, depression and poor adult health. Occupants of poor-quality housing are more likely to suffer from restricted daylight and noise pollution.
We cannot ignore noise pollution. In the news this morning someone put forward the idea of building houses and flats over railway lines. I am not sure if any hon. Member saw that. The first thing that came to my mind was the noise of the trains continually going underneath. How could those homes be adapted to mitigate that? We need to address noise pollution. Natural light helps to improve the recovery times of long-stay patients and reduces anxiety and the need for medication. Noise pollution can cause long-term health issues and increase stress and the risk of cardiovascular effects.
It is clear that there is a lack of public awareness of these problems, and limited knowledge of the facts. Too often, the homes we live in are, in so many ways, causing or aggravating health problems.
The hon. Gentleman is making a very good speech, and I am nervous about intervening again, but will he accept this point? He talks about the noise pollution from living over a railway, and we know that private rented accommodation is a real problem. On the other side of the equation, very modern and expensive housing that is totally hermetically sealed could be as dangerous, because it traps all the gases and pollutants within the home—not only carbon monoxide, but many other emissions.

It is a pleasure to be given the opportunity to lead a debate on this issue. I do not think that I have secured a Westminster Hall debate for two or three months. It is a great pleasure to secure one on this particular subject. I do not know whether anyone in Westminster Hall today thinks I am a latecomer to the subject, but when I was a very young lecturer at Swansea University I had the privilege of listening to a distinguished professor, Ernst Schumacher, just after he published a book called “Small is Beautiful”, and being introduced—as a traditionally trained LSE economist—to the notion of sustainable development and sustainable economies rooted in small, local communities. That started me on a lifetime of social enterprise and a lot of enterprises that were about the environment and sustainability.
Therefore, I am not a latecomer. I have not just read The Times 2 section, which, rather serendipitously, today is all about the plastic found in our marine environment, or just been influenced by that wonderful—what do we call him?—“saint of the environment”, David Attenborough; I saw “Blue Planet II” last night. He has become very much associated not only with such wonderful research but with wise advice, based on the research and good evidence about the dangers to the planet in general and to the marine environment in particular.
So I am not a Johnny-come-lately. Indeed, I was a founding member of Friends of the Earth in the United Kingdom. Many years ago, I started a group called the Socialist Environment and Resources Association, or SERA, a left-wing environmental organisation. On environmental urban mines, we believed that, instead of digging holes in the Earth’s crust and taking out virgin material, we should recycle and reuse material, including the waste that flows from towns and cities. I say that just to prove that I have some interest in and a record over the years of involvement in these subjects, and the desire, as a social entrepreneur, to do something about those issues in communities, both national and local.
The fact is that our marine environment is at risk, in a way that we have not previously thought it was at risk. I woke up to the issue a couple of years ago. I suppose I always knew how bad the marine environment was becoming. We had all heard of these vast islands of floating, semi-submerged plastic, which nobody knew how to deal with or tackle and which were getting bigger. The Environmental Audit Committee has done excellent work recently on microbeads. So I was conscious of the impact of that. I was also interested in recycling, what we did with waste and where waste ended up, as well as sustainability. All that came together, I suppose, when I reread an old favourite of mine by the man who created the term “the dismal science” for economics. Thomas Malthus predicted that, eventually, the population would outgrow the food supply and that we would all perish, unless two wonderful things happened—war or pestilence. That was Malthus’s way of suggesting that there would be a natural ability of the economy and society to renew themselves as we ran out of food.
The old counter-argument to Malthus was that human beings were clever, innovative and creative. They would discover new forms of science, applied science and engineering. Agriculture would become highly sophisticated in how it treated the land and grew crops, and we would become so much more productive.
The critics of Malthus were absolutely right, but the fact is that, although humans are creative, clever and innovative, they are also greedy, careless and exploitative. That is the truth. I said to one of my staff yesterday, “It’s not the sort of thing you run round your constituency saying to your constituents”. I do not pick on the British people particularly, but humans are clever, careless and exploitative, and they are in danger—one species—of destroying this planet through climate change and global warming and what we are doing to the oceans of the world, let alone what we have done to the poor species that we have shot, eaten, killed and driven into extinction.
I thank the hon. Gentleman for giving way; I had asked him before the debate if he would take an intervention. Does he agree that it is not only essential that we preserve and protect our marine environment but that fishermen are not prevented from sustainable fishing in areas that they know? Does he agree that science has proven to be fully capable of handling sensible fishing, as was done through the common fisheries policy? Does he realise that there are many who can sustain a business and that fishing is one of them, and that the environment will not be harmed by it?
The hon. Gentleman is absolutely right to bring up the issue of fishing. With your permission, Mr Owen, I will come back to fishing a little later, including that specific point. Of course there are better ways and worse ways of fishing. When we have Department of Environment, Food and Rural Affairs questions, if anyone mentions bottom-scraping, everyone giggles, but the fact is that there are ships that do scrape the bottom of the ocean, taking everything. That is a savage and unacceptable form of fishing.
The hon. Gentleman is absolutely right; I agree with every word he says. I will also put that issue on the backburner for a moment, because I want to talk about how we move forward. Anyone who saw the last David Attenborough film knows that it offered one little chink of light. Viewers get to the stage where they are feeling quite suicidal about the future of the marine environment, and then suddenly David Attenborough mentions that, actually, there is some possibility of the oceans renewing themselves in some areas, although not as well as we might hope.
Let me talk about the purpose of this debate. There has been a slow awakening to the peril the marine environment is in, but now is the time that we must act. David Attenborough says that we have 50 years to save ourselves, but I think that he is being generous. I think that we have to act much more quickly and decisively, and have the right kind of organisations. I am afraid that the only political things that I will be saying today are about what I believe to be the real strengths of the European Union over a number of years in helping us to co-operate across nations to tackle some of the great problems of the environment.
I remember meeting Surfers Against Sewage in my early days in the House. Mr Owen, you will remember what the seas around Swansea were like a few years ago. They were full of sewage—dreadful conditions. So many of our coastal towns used to pump sewage, in a pipe, out into the sea and, of course, back it would come. There has been a remarkable change because of European regulation on discharge to the sea. We rapidly cleaned up our seas and beaches, and also those right across Europe, so that when holidaying there we would know how clean the environment was; there is a standard and a system of flags.
I also remember the tiny amount of recycling that was done in our country in my early days in the House. Local authorities were at 14% recycling. The rates across the country have since zoomed up. Why? Because we took on board European regulations that meant the payment of a levy on any waste that was put in a hole in the ground. What a society we used to be, not long ago, putting all our waste product in holes in the ground. It is still there—a great treachery, a misspent youth. For 150 years, going back to the Victorians, we threw everything we had finished with into holes in the ground. That was a disgrace, and it was only European regulation and landfill tax that turned it around. We now have a much better—but not perfect—situation. Funnily enough, only recently I asked how much each local authority in Britain pays in landfill tax. I have not yet had a reply; the Government are very reluctant to give me the information, saying, “It is so difficult to collect. Inland Revenue cannot provide it”. It is, however, a very good indicator of how effective the authorities are in their recycling.
The hon. Gentleman is very gracious in giving way a second time. It is the new generation of young people who are very much into recycling; the older generation must learn to get into it. Does he agree that, when it comes to educating and thinking ahead, it needs to be at primary and secondary school levels, so that the next generation coming through can continue what has been and even do it better?
I hope we are not going to agree on everything here, but again, the hon. Gentleman is absolutely right. He will know that I was the Chairman of what is now the Education Committee for years and I am chairman of the John Clare Trust, which, in the name of our great English poet, who lived between 1793 and 1864—probably our greatest poet of the environment, in my opinion—has a centre where we specialise in getting young people to come to the countryside to learn about the rural environment, and so to love it. If young people in our towns and cities do not visit the countryside, we will not get them to love it at all.
We have expanded that work into my constituency. My hon. Friend the Member for Halifax (Holly Lynch) will know that we have a charity called Greenstreams, through which we try to clean up the rivers and streams in our part of Yorkshire. In the industrial revolution, the rivers were terribly polluted and the fish were killed; the colours of the dyestuffs would flow into the rivers and make them red, blue, whatever—very patriotic—killing everything. Now the water is clean again and we take children down there to show them that if they lift a stone they will see wiggly things that the trout eat, which are then eaten by the kingfishers—the cycle of life. The hon. Member for Strangford (Jim Shannon) is very much on the ball. We must start in schools, and the earlier the better.
I want to cover four things: plastics, overfishing, oil and petrol, and then come back to the big picture of climate change. We are sometimes too polite, aren’t we? If we look back over 400 years, we in Britain, as the earliest industrialised nation, with the greatest sea power, have not been good at keeping the global environment clean. I think we chopped down most of our trees to build warships. The biggest problem today is that as China is the most polluted country, followed by India, and then the United States, if we do not work with those large countries, everything we do in the United Kingdom will be of much less value. We need international co-operation, but not in a colonial way, pitching up in any country—even in Russia, which is a great polluter—and saying, “You should do what we do”. They would point to us and say, “Well you don’t have a very good record. You’re a late convert”. We are late converts, but we know a great deal now about how to change the environment in which we live and make it more sustainable.
Let us quickly look at one of the inspirations of recent years: the United Nations sustainable development goals. Goal 14 is about conserving oceans and protecting them from the adverse impacts of climate change, overfishing, acidification, pollution and eutrophication. At United Nations level, it is very important that every country sign up to the goals and make them happen.
My other interest as a Back Bencher is transport safety. Many years ago I introduced seatbelt legislation and my first private Member’s Bill was on children in cars. I have just recently been elected chairman of the Global Network for Road Safety Legislators. That relates to a different United Nations sustainable development goal, but that package of measures, globally driven by the United Nations, is, at the end of the day, what we must look to—international co-operation.
I was on a ship recently, and its environmental officer explained to me just how tight the fleet’s regulations were, and how stringent its rules were, on recycling, including dropping off metal at one port and plastic at another. Her fiancé, however, worked for a commercial firm in Alaska, where they basically threw everything overboard—no rules, no regulation and, it seems, no conscience.

I congratulate the hon. Member for Mid Dorset and North Poole (Michael Tomlinson) on delivering such an excellent and comprehensive speech. Like he, I am a glass-half-full person, who looks optimistically towards the future, and the facts, as I hope to illustrate, will indicate that things are very bright. If we try to be positive and look forward we can find solutions and see the good things. It is not always good to dwell upon the negative things; it is good to be aware of them but not to dwell on them.
Northern Ireland, as such a small country, is somewhat reliant on foreign investment and it is clear that we have the expertise and knowledge to attract investment from all over the world. I want to give a Northern Ireland perspective, because the hon. Member for Mid Dorset and North Poole referred to the United Kingdom of Great Britain and Northern Ireland. I say that often; I have said it every day since I came into this House back in 2010, to emphasise that we are very much part of the United Kingdom and that we are all together—Scotland, Wales, Northern Ireland and England.
Northern Ireland secured 33 foreign direct investment projects during 2015-16—as the Minister knows, we have some good stories—which created more than 2,000 jobs. Despite the usual Brexit warnings that scream that FDI will be affected by our leaving the EU zone, I read a piece by an economist that states:
“From a Northern Ireland perspective, the type of inward investment we have been winning has been back office functions, which may not need the access to the single market, if its function is to serve a parent company. Investor motives for investing here would back that up, with a strong majority of investors citing the skilled workforce as the main reason for investing here.”
Backing that up, figures in yesterday’s press indicate that the region of the United Kingdom with the strongest growth rate is Northern Ireland. So there is an indication that we are growing and that Northern Ireland is, for once, leading the way.
Before the hon. Gentleman gets carried away with all these good feelings about the future, he would surely agree that our productivity is absolutely awful, looking carefully at UK performance. Many people who would like to invest in this country are worried about low productivity rates. Productivity is much higher in Yorkshire, which has a much bigger population than Northern Ireland and some of the places he has mentioned. We are doing very well and manufacturing is still alive and well. People would be much better off investing in manufacturing in Yorkshire than coming to Northern Ireland.
I am always happy to receive an intervention from the hon. Gentleman. He is definitely an optimist as he is a Huddersfield Town supporter, and that is an indication of optimism at its highest. I wish him well, although on Saturday I hope Leicester beat them. I digress slightly, but there are ways of doing better and we need to address productivity.

I am being barracked: the comment “Not that educated” is coming from behind me.
I have been involved in the issue of out-of-school learning for a very long time. I had a very good run as Chair of the variously named Education Committee—it had a number of names, including the Children, Schools and Families Committee. Indeed, the Minister who will answer this short debate was a brilliant member of the Select Committee. We had great fun working together on a lively Committee.
I became somewhat obsessed with out-of-school learning, for two reasons. When one is Chair of a Select Committee of that kind, it is one’s job to visit as many schools as possible, in all parts of the country and at every level. In those days, we covered topics ranging from pre-school learning and nurseries right through to further education, apprenticeships and higher education, so it was a wide-ranging brief. However, when I got to schools, particularly in the primary and secondary sectors, I found that those schools that had the ability to take children outside the classroom transformed young people’s lives. All the research that has now been done on the issue shows that. A report that we did goes back 10 years. We did not have so much research evidence, but since that report came out 10 years ago from the Education Committee, we have been able to conduct research to show just how much young people are stimulated by getting outside the classroom and particularly into the countryside, and I became passionate about getting children out of the classroom and giving them an experience.
One of the wonderful things about getting a class of 30 kids out of the classroom is that we can do wonderful things for them and with them. Let me describe what all the research showed when we did our first inquiry. We have a treasury in London and in Northern Ireland, Scotland and Wales because we have free museums—what a wonderful treasury, what a wonderful learning experience. But tragically, when we delved into who among our children goes to those museums, we found the following. We found that more middle-class children went to them with their parents regularly. Going to them is a wonderful experience. Now, in London and Leeds, there are all-night stays in museums. That is an incredibly innovative and fun thing to do—sleep-ins at the museum, sleeping with dinosaurs. What a wonderful experience. However, all the research showed that more ordinary kids, from more ordinary, less affluent homes, did not go to the free museums—not even the free museums.
If the people from a less privileged background did go to the free museums, they went with their school. All the research showed what we needed to do if we wanted to reach out to all the children in this country, not just the more privileged—and I do not mean 5% or 10%, but something more like 60%. A very high percentage of kids living in this country, in our towns and cities and in the countryside, do not visit those wonderful museums unless their school takes them out of school to do that. They do not do it, or certainly they do it in lesser numbers and on fewer occasions, so I became dedicated to the view that it should happen.
Then I mixed up one passion with another. I do not know whether I should be indiscreet, when we are getting close to the general election, about falling in love with someone—it might get in the popular press—but I fell in love with John Clare, the English poet. He has been dead a long time: he lived from 1793 to 1864. When I went to school, I had the privilege of having a wonderful teacher who loved John Clare and imparted some of that enthusiasm to me, and I became dedicated to John Clare and giving him a wider audience.
When John Clare was alive, he had only 100 poems in print. The special thing about John Clare is that he was not a posh vicar or a Member of the House of Lords, as many poets were. He was an ordinary working man; he was called the Northamptonshire peasant poet. He was a day labourer, a farm labourer, and his father was a farm labourer; they threshed together in the village of Helpston. However, John Clare went to a dame school and learned to read and write, and after he left school at 12, he never stopped reading and writing. He briefly became popular in the Victorian period, when rustic poetry was popular, and 100 of his poems were in print when he died.
Then, in the 1960s, a treasury of wonderful poetry by John Clare was found. We think that his mental challenge was that he was bipolar. He could have been treated easily these days, but he was bipolar. The well-wishing people who looked after him put him in the Northampton general asylum. He was there for many years—he lived until his early 70s—and we discovered in the 1960s that he had been writing and writing and writing, even better poetry than the poetry that he had already published. One thousand of his poems are now in print, and more are being published.
Then I had the strange fortune of my eldest daughter marrying an academic who happened to be a John Clare scholar, from Cambridge University. He is now senior tutor at Fitzwilliam College and he has written a book about John Clare. I do not know how this happened, but I became the chairman of the John Clare Trust; I bought John Clare’s house; and we raised £3 million to turn John Clare’s cottage into a centre for children to visit. It is for everyone to visit, but we have a particular campaign called Every Child’s Right to the Countryside. I have seen the work that we and other people in the same field have done transform the lives of children; their lives are transformed by going to the countryside. One does not have to love poetry, art, music or what I often called—all my daughters studied English at famous universities—arty-farty people. They do not like that, but you know what I mean, Mr Paisley. I am a social scientist, trained at the London School of Economics in economics, so I can sometimes be disparaging about some of the more literary pursuits.
However, I know that if we take a child into the countryside and use technology, innovation, science or any subject under the sun, we can transform the experience of that child in that environment. Of course, John Clare writes about the woods and hedgerows and the plants and animals of our country, many of which are now very challenged in terms of their very existence. What we found in our work, which we did in partnership with others, was that if we want our country to have a countryside and our people to love it, they must visit it. Our secret—but not very secret—mission is to get people in this country, especially new generations of young people, to come to the countryside to learn and to really find their spark. I come across so many people in this country, even my own constituents in Huddersfield, who would benefit from that. I am sure that other hon. Members feel the same.
It is always nice to serve under your chairmanship, Mr Paisley—I think this may be the second or third time.
The hon. Gentleman raised those who wish young people to see and be involved with the countryside. I am very aware that in Northern Ireland we have under-achievement by Protestant males because they are not academically inclined, but their disposition might be towards the countryside. One organisation that has enabled those people at least to achieve something from a physical point of view is the Prince’s Trust. Has he had any opportunity to work with the Prince’s Trust to enable people who are not academically inclined to look towards the countryside, because they might find a job and perhaps a realisation of what they could do there?
The hon. Gentleman reminds me that we are not talking about an exclusive society of brethren. There are a lot of us, including the Scouts, the Prince’s Trust and lots of other wonderful organisations. The wonderful chief executive of the National Trust came to visit John Clare’s cottage in Helpston only a month ago. We need to work with the National Trust and all the organisations that can offer wonderful destinations to more and more schools. I would be wrong not to mention the Institute for Outdoor Learning, whose chief executive Andy Robinson was very helpful as soon as he heard that I had secured this debate. There are a lot of organisations out there.
All the research shows that it is good for children to come to the countryside. It shows the real improvement in academic subjects, as well as in achievement across the board, from getting children out for a day in the countryside, a museum or somewhere they can get a different perspective on their learning.

It is a pleasure to follow the hon. Member for Colne Valley (Jason McCartney) and my hon. Friend the Member for Dewsbury (Paula Sherriff), who have eloquently made the case and saved me from spending an awful lot of time going into the detail. However, I must repeat some of the narrative. Mr Speaker often, I think, verges a little on ageism when he points out how long I have been in the House of Commons, but it does mean that I have a long memory and I know the narrative of what has happened in health provision in my part of the world. That is always difficult for Ministers.
I noticed that this Minister, when asked whether he had visited Huddersfield, looked down at his papers rather intently. I do not blame him for that—there are parts of England that I have yet to visit—but Huddersfield is an absolute gem of a place. It nestles in the Pennines. I once had an American student who said, “I’ve found out the difference between Lancashire and Yorkshire—you’ve got the Pyrenees between you.” I said, “A lot of people in Yorkshire wish it was the Pyrenees; actually, it’s the Pennines.” That is a slightly humorous remark, but the fact is that it is a very hilly area; conditions can be very difficult. We see the special signs up in bad weather. Can we go over the tops? Often the conditions are such that we cannot. Very close to us, it is very hilly, with very difficult road networks. There is not much flat land. We were looking for industrial investment. You and I, Mr Pritchard, care very much about the manufacturing sector, and when people are trying to attract new businesses, they are all the time looking for flat land. We do not have any flat land; that is the truth. It is very difficult to find a flat space in our part of the world. It is difficult terrain.
What is nice about this debate is that from both sides of the Chamber we are making it clear that we do not want to beggar our neighbour. We want good health provision throughout our area. Good health provision is what motivates all of us. We want the highest-quality health provision. However, we do want accountable delivery of health provision. Many of us feel that the old system had its imperfections and the new system has its imperfections. Both the hon. Member for Colne Valley and my hon. Friend the Member for Dewsbury talked about the PFI. I have a long knowledge of PFIs. When I was chairing the Select Committee on Education, PFIs were used, as you know, Mr Pritchard, for much school building. I learnt over many years of controversy over PFIs that one cannot dislike PFIs on principle, but one can be against bad PFIs and in favour of good PFIs. I think that that is the truth of the matter.
There is a lot of evidence that some of the health PFIs were entered into with a rather amateur group of people representing the health trusts. That is the only explanation if we are to be kind to those people who made the arrangements. They were dealing with some pretty clever people—leading consultancies and people who really knew their stuff from the City of London. A senior professor said to me that some of the people sitting on the other side of the table were not as sharp as they could have been. They may have been local accountants and solicitors or the local management team, and perhaps they did not see quite how much the PFI was going to cost them over the number of years for which it was to run. That is the context.
A particularly worrying PFI was agreed for the Calderdale hospital in Halifax. There were two trusts in those days: the Halifax trust and the Huddersfield trust. The Huddersfield trust was always very well managed and had plenty of reserves, but when Halifax and Calderdale ran into trouble, we were pushed by the then Department to merge with the trust that was limping rather. People may remember this. We did merge, because we did believe in a good health service for all the people in our part of Kirklees and in Calderdale. That is the history; now we have to bring ourselves up to date.
There is a new dilemma, and I do not want to make it party political, but the urgent question on national health service finances yesterday did point to the fact that up and down the country a number of trusts are in serious financial trouble. Until comparatively recently, our health trust was in pretty good shape. Only comparatively recently did we suddenly have some real financial challenges. The Minister will be very familiar with this dilemma. On the one hand, we are being asked to make savings, efficiencies—4% every year—in order to maintain a good record with all the organisations that look at our health provision. On the one hand, there is that pressure for greater efficiency and saving money, but at the same time on our patch we have this PFI that is a great drain. On the other hand, we have what is a pretty old hospital in modern terms. I was once with Harold Wilson in the hospital when I was a very young MP. He had come up, and we were waiting for the top brass to come down and guide us. He said, “Barry, I don’t think I’ve ever been here before,” and behind him was a great marble stone that said, “Opened by Harold Wilson in 1965”.
The hospital is a classic early 1960s building. Some of us love some of the 1960s buildings. There are some that we cherish, such as the Barbican. Many people hate the hospital; I quite like it. There is a kind of brutalism that one likes. However, a lot of 1960s building was a little bit below par. We have on the one hand a hospital PFI that is very expensive and on the other a local hospital that is getting old. It has been invested in over the years. A great deal of investment has gone in, but I am told that a conservative estimate is that at least £200 million would be needed really to get it back on track. That is a great pressure on local health provision.
All of us across the parties in our area—local councillors have also been very active in the campaign—understand that we want the best possible healthcare for all the people on our patch. I know that the Minister is not so familiar with our part of the world. Not only is it hilly but it has a very mixed population. A lot of wealthy people live on our patch. There are a lot of middle-class people and a lot of people who are more challenged in terms of their income. It is a very mixed area, and that is the beauty of it. It is not boring; it is in every sense a vibrant area. I recently challenged the Secretary of State for Business, Innovation and Skills to come to Huddersfield and have a decent suit made of fine Huddersfield worsted; we still make the finest worsted in the world. Indeed, Mr Speaker is now also coming to Huddersfield to have a fine worsted suit made. I see you looking interested, Mr Pritchard—the invitation could be extended.
The fact is that, were there not so much contest between the smaller towns, the area might have had the name “Greater Huddersfield”. It is a city—one of the biggest urban conglomerations in the country—but people, especially outsiders, do not realise that because we have broken it up into different names. Kirklees is vast, which means that there are great healthcare challenges. Put that together with our difficult geography and an interesting history, and we face real challenges. We want the Minister to be open-minded and to enter into a discussion to find a way to get the very best result for the people of our area.
I shall be quite blunt about my resistance to CCGs. I wanted to be independent in assessing PFIs, and I said that there had been good PFIs and poor PFIs. There are also good CCGs and not so good CCGs, and I am not impressed by the quality and leadership of my local CCG. Although I have some resistance to CCGs, the general model is not a difficult one. I chair the all-party parliamentary group on management, so I am keen on good management in the health service and outside. Sometimes I see doctors managing CCGs; management is not part of any medical course I know of. We would not expect it to be. We train doctors to be good clinicians and good GPs, not to be managers. Some CCGs have real difficulties because they lack quality management.
There has been a failure of management in our local CCG when it comes to a proper, rational assessment of where we are now and how we can get the best possible healthcare in our area, taking into account all the difficult pieces of information that I have mentioned, including an ageing hospital that needs investment, a newish hospital that was built under a PFI, and difficult communications. I ask the Minister to look very carefully at what has been going on in our locality and to get the whole situation appraised carefully, independently and objectively.
I understand that this is an issue for the A&E in Huddersfield, but the hon. Gentleman mentioned getting other advice. In Northern Ireland, the Minister has set up a new panel to look at the whole health service and how best to take it forward in an area of financial restraint. Does he agree—I suspect that he does—that it is time to share those ideas across the whole United Kingdom of Great Britain and Northern Ireland? Thereby, we can all learn together.
I very much welcome that information, which relates to the point made by my hon. Friend the Member for Batley and Spen (Jo Cox). She said that there was no clear, strategic plan for the broader area of West Yorkshire. West Yorkshire is very close to Barnsley on one boundary. On another, it goes a long way right up the valley to where a very large number of people live in places such as Todmorden, where a bridge was recently affected by floods. Those places are in strong Manchester commuting territory. The area is vast and complex, and I cannot remember a proper evaluation across the piece, rather than an assessment that just carved out one bit of territory and looked into that very carefully.
I do not want to go through how many people are enraged, but they include—I read in the Huddersfield Examiner—Sir Patrick Stewart. Until recently, he was the chancellor of Huddersfield University, which was university of the year last year. He sends, from Hollywood, his solidarity with the people of Huddersfield on the issue of keeping the A&E department open.
On 11 March this year, we celebrate the centenary of the birth of Harold Wilson—a great man and a great Prime Minister—who was born in Huddersfield. When I used to drive him around Huddersfield, we would pass the old further education college, which was the old, old Huddersfield hospital, and he always said, “My appendix is in there.” The area has a great history. Please, in this special year, let us listen to the voices of the people of Huddersfield and Halifax, and get this right. At the moment, the suggestion of closing A&E in Huddersfield is not right, nor is the suggestion that Halifax is the only alternative. Personally, I think that there is a scheme by which we could keep both A&E departments open. My request to the Minister is: get that rigorous, independent, thoughtful appraisal of what the hell is going on, and get it right.

I hope that colleagues from Yorkshire who have been listening to the debate on trans-Pennine rail would like to stay for this important debate on atrial fibrillation, but in truth I know that many would ask what on earth atrial fibrillation is. Part of the purpose of the debate, therefore, is to open up our understanding and knowledge of the condition. I am sure that some colleagues at least will know that atrial fibrillation is a disease of the heart—[Interruption.] Excuse me, Mr Turner; I thought that that was the Division bell. Atrial fibrillation is a disease that causes an irregular heartbeat. It can often lead to a stroke and therefore to either a disabling long-term disability or death.
I called for this debate because 750,000 people in this country have atrial fibrillation. We know that because they have been tested for it. They may or may not be receiving treatment, but we know about them. It is estimated that another 750,000 suffer from atrial fibrillation but are undiagnosed, so we do not know where they are, although we have an idea because we can do the stats. If we know the average number of AF sufferers in a given area, we can extrapolate the numbers across the country. Where we see very low diagnosis numbers, we know that the local GPs and health service are not getting to grips with discovering who has AF, checking them out and treating them.
I would like to tell a little fairy story. There is a fantasy world in which a dreadful heart condition affects more than a million people in our country. A great deal of research is carried out, because no one really knows how to tackle it, and then there is an enormous breakthrough: we discover a new class of drugs that can not only help people to avoid having a stroke in the first place, but allow them to live a good, full and healthy life. However, the wicked godmother arrives and says, “Thou shalt not have any of these wonderful drugs, even though the National Institute for Health and Care Excellence says that they are good and should be available.”
I remind Members that the NHS constitution states that patients have a right to
“treatments that have been recommended by NICE”
when they are deemed “clinically appropriate”. That is the right of your constituents, Mr Turner, and mine. However, the fact is that most people with AF in this country are not getting the opportunity to receive such drugs. There is a real problem: we have a new generation of drugs that are recommended by NICE, but they are not available. What is the barrier? The barrier is the clinical commissioning groups and GPs.
If someone is a sufferer of AF—my wife is, which is why I know something about the condition—they are traditionally treated with warfarin, which is a very popular drug in this country. You will not be surprised to hear, Mr Turner, that even on the Isle of Wight GPs know—because they are highly skilled and knowledgeable —that warfarin is very cheap indeed. It is one of the cheapest drugs that can be prescribed.
I thank the hon. Gentleman for bringing such an important matter to the attention of Westminster Hall and of the Minister. Warfarin can be used to treat hyperthyroidism, as well as many other conditions. The issue that comes to my attention is that of GPs and their training, and their ability to administer to atrial fibrillation as required. Does the hon. Gentleman feel that GPs can do more in their own surgeries?
The hon. Gentleman is absolutely right. I was going to come on to that issue and I am grateful to him for making that point. The fact of the matter is that a very high percentage of people who are diagnosed with atrial fibrillation are currently either not treated with anti-coagulants, or they are given aspirin. Everyone knows that aspirin is very cheap but not effective as an anti-coagulant.
People with AF may also be given warfarin, which is a good treatment. I can say that from the heart. I have watched a close member of my family—it does not get any closer than one’s wife—undergo treatment that must be evaluated day in, day out. It is quite complicated to ensure that the dosage is right. If someone does not have a home testing kit, they will have to go regularly to the hospital for their blood to be tested and their dosage evaluated. If they have a full-time job or family responsibilities, that is an onerous requirement. As a result, many of the people who are taking warfarin are not taking it in the right dosage and so are not getting the proper, balanced treatment.
It is a scandal that 8.5% of atrial fibrillation patients are not receiving treatment, 35% are receiving aspirin, and only 56.9% are receiving oral anti-coagulation treatment. What is more, we now have three drugs that could be prescribed. We should be saying, “Isn’t it wonderful? We’ve had a breakthrough!” I hope that I can pronounce them properly—they are dabigatran, rivaroxaban and apixaban. However, compared to the 3p that it costs for a dose of warfarin, they are more expensive—I have seen an estimate that treatment would cost around £800 a year.
That might be considered an excessive cost compared to the tiny amount that warfarin costs, but strokes cost this country £2.5 billion a year. If we really want to wreck the national health service, we should not treat people with AF properly. They will have a stroke and end up in long-term care, making great use of hospital beds and highly qualified medical staff. Such a burden on the health service could be avoided.
I have been a member of the all-party group on atrial fibrillation for some time now, and I know that its chair, the hon. Member for Montgomeryshire (Glyn Davies), is going to speak after me. We served together on the Education and Skills Committee, Mr Turner—do you remember when we had a very good Clerk working for us? I think he is sitting on your left-hand side. When we were on that Select Committee, you will have heard me articulate many times the watchwords, “I like evidence-based policy.” The atrial fibrillation campaign is the one, against all others, for which the evidence shows that if we have a drug that can sort out the condition, it should be used.
NICE says that we should use it, and it is clear that it is the right of patients to have it. The people getting in the way are GPs—not because they are malign, but because the cost means that they are leant on by their practices about prescribing it. Also, a very substantial population of GPs do not understand the treatments and their effectiveness—which treatments work and which do not. It is a scandal that people suffering from AF are prescribed either nothing or aspirin by their doctors. That is a very serious problem for the profession, and we have been taking it up with the Royal College of General Practitioners.
There is a second barrier, which is that even with the cheapest of the drugs—warfarin—the sophistication of the treatment and the monitoring are very difficult for very large numbers of our population to deal with. My constituents and your constituents, Mr Turner, find it very difficult to get the right dosage and to maintain the quality of treatment.
The third barrier is the clinical commissioning groups. There is no doubt that the clinical commissioning groups are a barrier to this spending. These are relatively new drugs. They were approved by NICE about 18 months ago—I am looking at the chair of the all-party group in case that is not correct—and NICE said that by now it would expect about 20% of AF sufferers to be on the new anti-coagulants, but only 3.4% of sufferers are on them. Even NICE, projecting forward, thought that the figure would already be 20%.
I do not want to talk for too long, because other hon. Members want to speak, but it is a national scandal that people are dying today, are dying every day and are having incapacitating strokes, and that that is costing the national health service an enormous amount of money and requiring the use of an enormous amount of expertise. It is a burden on the national health service that should not be there.
There is an easy resolution. It is based on science, based on research, based on evidence. It is about time that the ministerial team took the lead on this matter, that GPs woke up, and that clinical commissioning groups heard the hard words that we will not allow our constituents unnecessarily to die or suffer long-term disability just because of the inactivity of the system. We are seeing this short-term saving, this mean-minded pettiness of saving a bit of money on the balance sheet of a CCG today, when the real cost to the health service is a generic one right across our country.
This is the beginning of a campaign. We have been campaigning for a long time, but it is at a new level. We are not going to let this issue go away. This is not party political. We will chase the Minister, chase the Secretary of State and chase the Prime Minister, because this issue is important and we cannot allow this injustice to continue any longer.
