Question to the Department of Health and Social Care:

To ask the Secretary of State for Health and Social Care, how many serious incidents have been reported in each of the last 10 years.

The attached table, which has been provided by NHS England and NHS Improvement, shows the number of serious incidents that were reported on the Strategic Executive Information System for each year from January 2011 to August 2020. The data for the most recent 12 month period may change as a result of investigation findings indicating an incident initially reported as a serious incident does not meet the definition of a serious incident under the Serious Incident Framework.
