Commons ChamberWe have record numbers of staff in the NHS in England. We are on track to hit our target of 50,000 more nurses since 2019. Speaking of 50,000, there are more than 50,000 more people working in social care since 2016. We are boosting the home-grown workforce, recruiting from the EU and welcoming health and social care workers from all around the world.
Of course the hon. Lady would like to make everything about either Brexit or the Union—or, most often, both—but the fact is that a doctor, nurse or care worker from the EU who wants to work here can do so. We are also welcoming people from the rest of the world, and there are 12,000 more staff in the NHS in England from EU and European economic area countries since the referendum.
In the Baillieston area of my constituency, one of the biggest issues people raise with me is the difficulty in accessing GPs. Part of the problem is that we do not have enough GPs. The Royal College of General Practitioners shows that 49% of EU-trained medics experienced issues with their visas, which led 17% of them to think of leaving the UK altogether. When will the Minister grasp this issue, speak to the Home Office and get rid of the hostile environment that means many of my constituents do not get access to the GPs they need?
If the hon. Gentleman had been listening to earlier questions, he would have heard about the increased number of GPs in England, with more than 2,000 more GPs now working in England. Coming to the question of the NHS in Scotland, which is of course run by the SNP-led Scottish Government, I have heard that NHS Scotland is “haemorrhaging” staff, in the words of the chair of the British Medical Association in Scotland.

Lady well knows, having followed this debate and recent conversations about the Budget, that constituencies and Members on the Conservative Benches and on the Opposition Benches have received funding from the levelling-up fund, including Members of the Opposition Front Bench. I encourage the hon. Lady’s constituency to bid in future rounds of the levelling-up fund. So far—[Interruption.] If she would please listen to what I am saying rather than continuing to shout at me. She asked me a question and I am responding. It is important to her constituency that she listens. Her constituency is clearly seeking levelling-up funds. I would be delighted if it received them. It will have the opportunity to bid for further funding in future rounds, along with other Members and constituencies that have expressed an interest in doing so.
Across the UK, we are creating the conditions for businesses to invest and flourish, because businesses create jobs and drive the growth that will see people’s living standards going up. This was an argument that was made skilfully by my hon. Friend the Member for Bosworth (Dr Evans) among others. Hence, we have a £1.4 billion global Britain investment fund and £1.6 billion additional funding for the British Business Bank’s regional funds.
As we pursue levelling up, we are empowering local leaders to shape and drive the transformation in their communities—local leaders who know best what their communities need. That was a point made by my hon. Friend the Member for Henley (John Howell) among others and exemplified by the £5.7 billion five-year consolidated transport settlements for the eight city regions.
During today’s debate, I heard several colleagues make comments such as “Don’t forget about the south”, from my hon. Friend the Member for Southampton, Itchen (Royston Smith); “Don’t forget about London”, from my hon. Friend the Member for Wimbledon (Stephen Hammond); and “Don’t forget Suffolk and Norfolk”, from my hon. Friend the Member for Waveney (Peter Aldous). I also heard, “There’s not enough money for the north.” I assure hon. Members across the House that levelling up will happen across the UK, in all regions and nations of the United Kingdom.
One wonders why the Government need to level up after 11 years in power, but that is perhaps a damning indictment of their record in office. The Minister talks so much about levelling up and a level playing field, so why is it that English sparkling wine gets a tax cut in the Budget, but Scotch whisky only gets a duty freeze?
I am delighted to pick up on the hon. Member’s comments. I am surprised that he did not welcome the level of funding—the highest ever block grant—that Scotland is receiving as part of the Budget and spending review. We are making much-needed reforms to the alcohol duty system, which has been recognised by many commentators over the years as dysfunctional and in the interests of neither public health, nor our economy. We are now moving to a fairer system that taxes more alcoholic drinks at a higher level. This is also fair to whisky. The hon. Member for Glenrothes (Peter Grant) called for whisky and wine to have the same duty rate by unit of alcohol. That is exactly what we are proposing in the reforms that will be introduced in 2023, because we recognise the importance of many parts of the sector—whether it is beer, cider, English and Welsh sparkling wine, or whisky—to the UK economy.
We are ensuring that regions that have historically received less investment are no longer overlooked. For example, some £500 million of the £1.7 billion of the first round of the levelling up fund will indeed go to the north of England, but there are examples of levelling up fund investment all around the country.

It is a pleasure to serve under your chairmanship, Mr Hosie. I thank the hon. Member for Liverpool, Wavertree (Paula Barker) for securing the debate on this important issue and giving me the opportunity to talk about our NHS staff. I come from a family of doctors and nurses, and before I became an MP I spent years working in hospitals and other NHS organisations, particularly to make the NHS a better place to work, because NHS staff are our health system’s greatest asset. In normal times, they go the extra mile, but in the pandemic they have done so time and again.
I welcome many of the points made by hon. Members this morning. Many have talked about the huge sacrifices that staff have made during the pandemic—not only them, but their families. Some, for instance, moved out of their family home to ensure they did not bring coronavirus home with them.
Colleagues have talked about not only nurses but other members of the healthcare workforce, including healthcare assistants, who are often overlooked in these conversations but are a vital part of our health service.
Some Members have talked about vacancies in the NHS workforce, of which I am well aware, and I will provide some reassurance on that in my remarks. Hon. Members have talked about PPE, but I do not see it as a choice between paying the NHS workforce and providing PPE—we must do both. PPE is not a choice; it is essential to protect those working in the NHS, and to me, it is non-negotiable. The shadow Minister, the hon. Member for Ellesmere Port and Neston (Justin Madders), spoke about the need to invest in our workforce. I wholeheartedly agree with that, and the Government are doing so.
As I said, NHS staff are the NHS’s greatest asset. In fact, the NHS is, in essence, its people. The Government are determined to ensure that the NHS can retain and recruit the staff it needs. Over the last few years, the have worked with trade unions to deliver multi-year pay and contract-reform deals for more than a million of our NHS staff. Over the last three years, nurses specifically have seen their starting salaries increase by more than 12%. We have increased the lowest starting salary in the NHS by more than 60%.
We all know that our NHS staff work around the clock to provide care. That is why, on top of the basic salary, NHS staff earn premium rates of pay for working at night and over the weekend, and for agreed overtime. That increases individual pay by around £4,000 on average. On top of that, the NHS reward rightly includes benefits that go beyond the statutory minimum on holidays, sick pay and pensions.
We have also invested in our junior doctors, recognising the huge contribution that they make to the NHS. The deal that we have agreed with the British Medical Association improves junior doctors’ working lives, protecting rest requirements and reducing the number of consecutive shifts worked. By the end of that deal, junior doctor pay scales will have increased by at least 8.2%, and around one in eight junior doctors will receive more as they reach a new higher pay point to reflect their level of responsibility. For our nurses and non-medical staff, this is the final year of the multi-year “Agenda for Change” deal. We have asked the independent pay review bodies to make recommendations on the pay of our NHS staff for 2021-22.
As the Government have set out, the coronavirus pandemic has placed a huge strain on public finances, and the economic outlook remains uncertain. The Government’s written evidence to the independent pay review bodies set out that, in settling the Department of Health and Social Care and NHS budgets, the Government anticipated a headline pay award of 1% for NHS staff. That compares with the pay freeze for the wider public sector and, as we all know, with the context of many people facing unemployment and pay cuts in many parts of our economy.
The Minister is setting out the bleak fiscal picture for the Government and the tough financial choices that have to be made. Will she explain, then, why they have seen fit to invest in more nuclear warheads but not in pay for NHS staff?
The hon. Gentleman will recognise that although the Government have to make some difficult decisions, various things are non-negotiable. One of those things is ensuring that the NHS is there for all our constituents who need it, and another thing is ensuring that we have the defence that we need to protect people from threats from overseas.
Let me return to the matter in hand and set out a bit more about the process that we are going through on NHS pay. As I mentioned, the evidence that we recently submitted to the NHS pay review covered a wide range of data that was relevant to the decisions that that pay review body will make. The pay review bodies themselves are independent advisory bodies made up of industry experts. Their recommendations are based on a comprehensive assessment of evidence from a range of stakeholders, including trade unions. The wide range of factors that they will consider includes the cost of living, recruitment and retention in the NHS, affordability and value for money for the taxpayer, and comparisons with wider public and private sector earnings.
As the pay review bodies are independent, I cannot, and would not wish to, pre-empt their recommendations. We have asked the NHS pay review body, and the review body for doctors and dentists, to report later in the spring, and we will carefully consider their recommendations when we receive them.

Further to that point of order, Mr Speaker. On that very theme, this is a picture of my daughter who will be born in the autumn, so I am particularly keen to see this measure put in place as soon as possible.
On a serious point, where is the Leader of the House? The Government have just unilaterally changed the business of the House this afternoon. I am the acting Whip for the Scottish National party today, and I found out by a rumour that this had happened. It is a gross discourtesy to the House, and the Leader of the House should have come to the Dispatch Box and made an announcement.

The hon. Lady makes a good point; I very much agree.
The latest quarterly release on appeals of work capability assessments shows that 59% of decisions are overturned at appeal. To be blunt, that means that six in every 10 decisions are wrong. That is incredibly alarming.
There is, of course, a wider point about the undertaking of work capability assessments by a private sector provider, which I oppose on ideological grounds—I agree with the hon. Member for Coventry South (Mr Cunningham) on that point. I doubt, therefore, that it will come as much surprise that I very much welcome the commitment by the Scottish Government to ban private firms from carrying out benefit assessments. I wholly concur with the Scottish Social Security Minister Jeane Freeman, that
“profit should never be a motive nor play any part in assessing or making decisions on people’s health and eligibility for benefits.”
Over and above my ideological objection to private sector provision, I am sure that all hon. Members will be concerned to note that, according to the DWP’s own data released only last week, the ESA assessment provider has consistently failed to meet the contractual expectation for the quality of assessment reports.
One thing I have been calling for, for some time, is standard recording of all work capability assessments. Often there is one story from one side and another story from the other, and recording would not only provide some evidence about what really happened, but improve people’s experience of the assessments. It has been piloted, so does the hon. Gentleman agree that it would be good to push forward with the recording of assessments as one way of improving the experience for our constituents?
I am grateful to the hon. Lady and will come back to how the assessments are conducted, because that forms a major part of my speech.
At this juncture, I want to commend to the House the excellent report by Rethink Mental Illness entitled “It’s broken her”. It was published just last week and makes truly harrowing reading. The report lays bare the full extent of the challenges for people with mental illness when facing assessments for both ESA and PIP. Drawing on findings from a series of interviews and focus group-style discussions, the report finds that the assessment can be “traumatising and anxiety-inducing” for the following reasons: there are numerous issues with the paper forms that claimants must submit, including their complexity and length, and the inflexible nature of the questions they ask; claimants must collect their own medical evidence, which is extremely burdensome, often expensive and time-consuming; the staff who perform face-to-face assessments frequently have a poor understanding of mental illness; and, finally, delays in mandatory reconsideration and appeals to the tribunal mean that claimants may have to wait many months for the correct result.
The report concludes that the current PIP and ESA assessment procedure
“inherently discriminates against people with mental illnesses”.
It sets out a number of policy recommendations to
“dramatically improve the benefits system for people with mental illnesses, as well as saving the Government the vast costs that are currently incurred due to persistent incorrect decisions made early in the process.”
