My Lords, over the last five years the number of registered patients in England has increased, while the number of GPs has dropped by 5%. That has now resulted in a 12% increase in the number of patients per GP. No wonder there is pressure. I return to the original Question from the noble Lord, Lord Hunt: when will the Government provide proper workforce planning for GPs?

Lords ChamberMy Lords, the 3.7 million clinically extremely vulnerable people already follow government advice for them, regularly checking the daily Covid dashboard to see how many cases there are in their area. This will be even more important when others no longer have to self-isolate when they get Covid. They cannot do this when widespread testing and the daily dashboard stop. What advice would the Leader give these people on how to assess their own risk after 1 April?
The noble Baroness is absolutely right that throughout the crisis we have led the way on data reporting, and have ensured that data is always available to the public. UKHSA will keep the content and frequency of reporting on Covid—including the GOV.UK dashboard—under close review, to ensure that statistics are being produced of the appropriate quality and transparency, and that they remain useful and relevant in accordance with the code of practice for statistics. So we will continue to publish information.

My Lords, DfE guidance to schools, updated on 20 January, told heads that
“A director of public health might advise you that face coverings should temporarily be worn in communal areas or classrooms”.
What would the Minister say to the head who is asking all pupils to wear masks until further notice, as one of their pupils has leukaemia and is severely immuno- compromised? Why have the Government, whether the Department for Education or the department of health, not given advice to these pupils, their families and their schools?

My Lords, typically, private equity-backed providers spend about 16% of the bed fee on complex buy-out debt obligations. The accounts of Care UK show that it paid £4.1 million in rent in 2019 to Silver Sea Holdings—a company registered in low-tax Luxembourg—which is also owned by Care UK’s parent company, Bridgepoint. Given that the ONS says that 63% of care home residents are paid for from the public purse, does the Minister not think that private equity providers should be subject to a financial code of conduct?

Lords ChamberMy Lords, the Leader of the House just said that there is no advice for people who were formerly shielding, the clinically extremely vulnerable, but there is; the noble Lord, Lord Kamall, confirmed this to me last night. It says that this group should still consider meeting people only 14 days after they have been fully vaccinated, social distancing, asking friends and family to have rapid lateral flow antigen tests, asking any visitors to their homes to wear face coverings and not going into enclosed crowded spaces. Putting this guidance on a website is not the same as telling this group of people, or the wider public, especially their employers, directly that this group still need to take care. Will the Leader take this back and ensure that communications go to this vulnerable group of just under 4 million people?
I apologise to the noble Baroness and will certainly take that back to government colleagues. I recognise what she says.

My Lords, yesterday NHS England data showed that trolley waits of more than 12 hours in A&E rose in December to just under 11,000, which is three times higher than in December 2020. One hospital reported that it had a dozen patients waiting on a trolley for a bed for over 24 hours. The Minister has talked about extra money, but without staff and bed capacity in both hospitals and care homes, the crisis remains. Can he say what the Government are doing right now to help alleviate the current crisis?

My Lords, government data show that more children have been admitted to hospital with Covid in the last three weeks than in the whole of the nine months of the first wave. Many clinically extremely vulnerable children are still expected to go to school, even though they still await their vaccines and guidance on how to manage the risks they face, including RSV and influenza. Can the Minister say when these children and their families will get the vaccines and guidance they need? If he cannot, will he please write to me with the answer?

My Lords, the Minister has just had trouble responding to the Question from noble Lord, Lord Scriven, and from the noble Baroness, Lady Thornton, about the process used to review performance of the Government-approved travel PCR testing companies. Can I give him a specific example? TestnGo has a 76% “poor” rating on Trustpilot, with thousands of people not receiving their PCR tests and others not getting the results in two days. As the noble Lord, Lord Scriven, has said, many have reported this to test and trace, so why is it still on the Government’s list of approved suppliers?

My Lords, this week the Royal College of Emergency Medicine reports that 40 hospitals have cancelled at least 13,000 operations over the last two months because of the surge in demand, as well as the high number of Covid patients in hospitals. The Government winter plan says that there will be extra beds and staff to help, but there are no beds or spare staff right now, so what are the Government proposing to do before many of these patients end up back in A&E because of their delayed surgery?

Lords ChamberI declare my interest as a vice-president of the Local Government Association. My noble friend Lord Newby asked, but the noble Baroness did not answer, so I ask again whether she will give an undertaking from the Dispatch Box to guarantee that social care will receive the bulk of the revenue raised by the levy, in the medium term, regardless of whether the NHS has completed the backlog of waiting lists. Otherwise, this is no reform to social care at all, however delayed.
These proposals do nothing to help the current problems in the social care sector. The noble Baroness referred to the extra £1 billion to local government, but all the experts say a minimum of £4 billion a year is needed to help solve them. Can the Minister confirm that, given the current severe crisis in staffing and funding in our care homes, there will be funding on top of that £1 billion at least to start to remedy the problems in social care?
As I said in my opening remarks, we are committed to spending an additional £5.4 billion across the next three years. This will end the risk of unpredictable care costs and include at least £500 million to support the social care workforce.

Lords ChamberAs I set out in my response to the noble Baroness, there are a number of factors in why we believe that spring 2022 is the right time to start this inquiry. I gave them earlier. Of course the noble Lord is absolutely right that we need to tackle the worrying backlog of people needing care from the NHS, which is why we have committed billions of pounds to doing so, including £1 billion to tackle waiting lists by providing up to 1 million extra checks, scans and additional operations. We will continue to prioritise urgent and cancer care, as well as the recovery of non-urgent diagnostics and treatment so that patients receive the best healthcare as quickly as possible. That is an absolute priority.
The noble Baroness the Leader of the House did not respond to the point made by the noble Lord, Lord Newby, about the publication of interim reports from reviews and inquiries. The Hackitt review on the Grenfell fire and the Independent Inquiry into Child Sexual Abuse also produced interim reports in order to save lives and protect people. The Leader of the House has admitted that we know that the pandemic is by no means over. Surely an inquiry into the pandemic should also publish interim lessons learned to save lives and protect people. Can she make sure that that happens?
I am sure that, when a chair takes their place, views like that will certainly be put to them and it will be up to them to decide.

Lords ChamberI apologise for missing the noble Baroness, Lady Brinton, whom I now call.
My Lords, I return to my noble friend Lord Newby’s question about self-isolation. Australia and New Zealand give a straight- forward grant, set at minimum wage, for those self-isolating and quarantining, with no means testing. Their results have been outstanding, with a very high compliance level; people do not have to choose between putting food on their tables and isolating. Given our low levels of compliance, should not the Government move to a non-means-tested grant, as a tool to succeed in lifting lockdown, as a matter of urgency?
As I said, the £500 support payment has been extended, so parents of children who are isolating are now eligible for it. In addition to that, we are increasing, to £20 million a month, the funding available to local authorities to make discretionary payments, and that money is intended to support those who fall outside the scope of the main payment but still face hardship. As I have said, obviously we have the Budget next week, where there will be further detail in the round about the economic support we will provide going forward.

Lords ChamberYes, I can give the noble Lord that assurance. Work is already ongoing, but he is absolutely right: it is critical.
My Lords, this morning, the Health Service Journal has said that our already overburdened hospitals are trying to release Covid patients into care homes but that a major problem is stopping this. The National Care Forum reports that insurance for designated Covid settings is now almost impossible to get and that, without indemnity cover, they cannot take Covid patients. NHS Providers is begging the Treasury to help, as hospital beds must be freed up, but the Treasury is refusing. Can the Lord Privy Seal take this up with the Treasury as a matter of extreme urgency and help resolve this problem, not of care homes’ making, which is blocking beds in hospitals at a time of national crisis? Please will she keep me informed of progress?
I am very happy to raise those issues with both the Department of Health and the Treasury. I am sure that my noble friend Lord Bethell in particular will be able to keep the noble Baroness updated on discussions.
