I look forward with great enthusiasm to my monthly updates to the House on this important initiative. We are moving as quickly as the machinery of government allows us to. Taking along all the nations is an important aspect, but, quite fairly, it requires consultation with and the engagement of the devolved assemblies, which is why we have written to them and are engaging with them accordingly. I am also pleased to share with the noble Baroness that we are actively engaged with Defra, which is undertaking a wider review of bread and flour regulations. We will be aligning its fortification plans with this measure in due course.

My Lords, I congratulate the Government on the progress made on a well-financed vaccination programme that continues to win the war against the worst effects of the virus. Will my noble friend the Minister and his colleagues now ensure that as much effort and commitment will be put into a lead role for the new office for health promotion in his department, to co-ordinate government policy and ensure that we place national well-being, physical and mental health at the centre of policy priorities for all age groups as we emerge from Covid?

I am very grateful to my noble friend for raising the office for health protection, because it is an office that I am extremely hopeful for. It will be giving clinical leadership from the CMO. It will bring together all the enormous resources of data that we have brought together in the pandemic response. It will, I hope, capture the national mood around healthy living, including, as my noble friend rightly points out, eating habits and physical activity habits. It will work through local authorities, it will not be a large organisation like UKHSA is, but I hope it will have an enormous impact. I look forward very much indeed to discussing it more in this Chamber.

My Lords, I am grateful for the noble Lord’s comments. Yes, we are engaged with behavioural scientists, but I reassure him that lotteries for vaccines are not on the cards. Taking vaccines into communities has proved an extremely effective measure. I led a call with council leaders in the north-west—from Lancashire and Greater Manchester—and there I heard about the effective use of small mobile units and tents to bring vaccination teams into either religious or community settings to make it easier to get a vaccine. That simple measure appears to be a really winning formula, and one that we are investing in in a very big way.

My Lords, I echo my noble friend Lord Balfe’s figures on Covid-19 hospitalisations: of the 114 people in hospital, just three had received both doses of vaccine. Does my noble friend the Minister agree that the best approach the Government can follow is to continue with an urgent and comprehensive vaccination programme—with the further easing of restrictions secondary to the goal of a successful national vaccination campaign—using, not least in local communities, positive influences in communities wherever possible? Will he also accept the thanks of the Olympic and Paralympic athletes for the positive approach the Government and the International Olympic Committee have taken to ensure that athletes and their support staff will be vaccinated before leaving for the Olympic and Paralympic Games in Tokyo?

I am extremely grateful for my noble friend’s comments on the Olympics, and we wish our Olympic champions all the best luck. We keep our fingers crossed for Tokyo, under very difficult circumstances. On the vaccination programme, he is entirely right: positive influences are key. It has been interesting that the positive influences we think have made the biggest impact are not necessarily only the celebrities—they are community influencers who work in clinical settings and are present at a grass-roots level in communities. That is why a large volume of videos, endorsements, community meetings and answering quite reasonable, but sometimes very sensitive, questions from the public have been the essence of our vaccination communications programme. It seems to be extremely successful: the younger age groups seem to be stepping up for the vaccine in proportions that we could not have believed possible some months ago, and we hope very much that this will continue.

My Lords, we are redoubling our efforts on obesity, mental health, well-being and other aspects of prevention. That is why the Office for Health Promotion will operate from within the Department of Health and Social Care. It will be directed by the professional leadership of the CMO and it will drive forward our prevention strategy. It will focus on reducing health inequalities and its approach will be proactive, predictive and personalised interventions.

This welcome government initiative has the potential to lead on vital cross-departmental work to improve physical and mental health, well-being and active lifestyle measures for all ages. Does my noble friend agree that this agenda deserves a well-being statement and budget from the Treasury, as in New Zealand? Given the critical importance of the nation’s health and the widespread interest shown in this Question, will my noble friend accept a request for an online meeting involving interested Peers and officials appointed to the OHP?

My Lords, I absolutely endorse my noble friend’s points. This is an extremely important agenda which we are getting behind in a very big way. I also completely acknowledge and recognise the enormous amount of interest from noble Lords in this agenda; we have taken a number of Questions on this and I celebrate the fact that noble Lords have as much interest as they do. I would be very glad to arrange such a session as my noble friend suggests.

As the noble Baroness, Lady Jolly, just said, does the Minister agree that it is high time that we followed Australia, which mandated the addition of folic acid to wheat flour for making bread? As long ago as 1988, folic acid fortification of all enriched grain product flour was fully implemented in the United States and Canada. It is time to say yes to the long-running campaign of the noble Lord, Lord Rooker; it should be a departmental priority on 7 May.

I reassure the noble Lord and others who have pressed this point that it is a departmental priority. There has of course been a pandemic and that has slowed things down. I cannot avoid that fact, but we are very much returning to the prevention agenda in the round and the issue of folic acid in particular.

My Lords, I warmly congratulate the Government on the management of the vaccine programme. As we emerge from the epidemic, will the Government commit to a major health policy initiative to ensure that all young people engage in a more active lifestyle, participate in sport and recreation, gain affordable access to gyms, swimming pools, leisure facilities and dual-use school facilities used by local communities, and to tackle what is the least fit generation of young people in over 100 years? Does my noble friend agree that affordability and access are the critical components in this context?

My Lords, the policy on sports is best left to colleagues at the Department for Digital, Culture, Media and Sport, but on a personal level I emote complete sympathy with my noble friend’s sentiments. I may be naive in this matter but I cannot help hoping that this pandemic will have led to a feeling across the country that the health of the nation has to change—it has to change emphatically, not only through diet but the amount of activity taken. This nation has an opportunity to embrace a lifestyle with more outdoor activity and exercise and a greater commitment to healthy living. That is a reasonable ambition, not just for my noble friend but for the whole country, and I support it entirely.

The noble Lord is, as ever, cutting to the chase. The role of hotel staff, transport to and from airports and the flight crew themselves is incredibly important. As the noble Lord probably knows, the outbreak in Melbourne that hit Australia hard was caused in part by the bus drivers from the airport to hotels becoming vectors of infection. That created an unfortunate outbreak, which was hit extremely hard with a long lockdown to squeeze out the outbreak. We are putting in all the right, responsible measures to segregate crew, keep them apart from the rest of the population and ensure that they are, wherever possible, vaccinated against the virus so that they cannot be vectors of transmission.

My Lords, having flagged up the quarantine restrictions, tens of thousands of people from around the world have been returning to the UK in advance of Monday’s deadline to avoid unaffordable costs in hotel bills. This is resulting in many connecting flights across the UK being unexpectedly packed. I understand that this is leading to large numbers of passengers, on arrival at major UK satellite airports, waiting shoulder to shoulder around baggage carousels for their luggage, without airline or baggage-handling staff in evidence, no social distancing and a serious risk of exhausted passengers with low resistance being prone to Covid infection. Will my noble friend the Minister consult colleagues and ensure rigorous application of all social distancing restrictions around all domestic baggage carousels over the remaining three days and beyond?

My Lords, I am grateful for the question but my information is slightly different from that articulated by my noble friend. Passengers overseas have heard the message loud and clear, and there has not been, as far as I understand, the kind of rush that he describes. In fact, there has been a lot of sensible behaviour by passengers. We are grateful to London Heathrow, London Gatwick, London City, Birmingham and Farnborough, which to date are the authorised red list airports and have put in place exactly the kind of social distancing measures around transit from the aircraft to the PCP, from the PCP to the baggage hall, and from the baggage hall to the transport to the hotel. A huge amount of thought has been put into the personnel, signage and arrangements to ensure that that is done in a way that applies the best possible hygiene measures.

The noble Baroness is right. Those involved in domiciliary care, particularly part-time, make an extremely important contribution. We are naturally concerned about how they are contracted and their educational needs supported. We would like to think more about how part-time domiciliary care staff in particular, who make such a valuable contribution, can be further supported.

My Lords, does my noble friend the Minister recognise that nurses remain at the heart of the world’s response to the Covid-19 pandemic? As key to the restoration of health systems that have been neglected during the crisis, will the Government support calls by the International Council of Nurses fundamentally to reset preparedness and response systems and work towards the global requirement for an additional 10 million nurses by 2030?

My Lords, we massively value the contribution of nurses from all areas. In fact, that recognition has manifested itself in practical terms; we are growing the nursing workforce and are committed to delivering 50,000 more nurses, putting the NHS on a trajectory for sustainable long-term supply in the future. That journey includes giving eligible nursing students an additional £5,000 of funding per academic year. I cannot say more clearly or loudly how much the contribution of nurses to our healthcare system is appreciated. We will do everything we can to ensure that it is recognised.

Those who are not registered with GPs and would like to take the vaccine need to register with GPs. We have put in place provisions to allow easier registration processes, we have updated our data arrangements and we are expecting a large number of people to seek out registration. That will be one of the benefits of the vaccination programme: clearer, better records of those in this country who are part of the NHS family.

My Lords, last month, my noble friend the Minister was hoping to receive advice from the World Anti-Doping Agency for our Olympic and Paralympic sports stars preparing for the 2021 Tokyo Games on the specific point of whether MRNA vaccines were prohibited under the WADA code. I understand that the vaccines have been deemed safe and acceptable for use within the guidelines, protocols and rules of the WADA prohibited list. If this is the case, will my noble friend ask his department: to publish WADA’s detailed advice; what testing capability our national anti-doping agency—UKAD—has for synthetic messenger RNAs; and for information on when all elite sportspeople can expect to be vaccinated so that they can train and compete safely?

I am grateful to my noble friend for his championship of this important point. It is not necessarily the role of the department to rule on this matter, but I note that UK Anti-Doping welcomed the World Anti-Doping Agency’s publication of its view on the vaccine. We welcome that moment and I very much hope that it provides the reassurance that athletes are looking for.

My Lords, I congratulate my noble friend the Minister on the importance that he attaches to the safe return of fans to designated sports events, the reopening of many gyms and sporting activities across England, and the financial package announced last week for key sports in need of urgent financial assistance. Does he agree that we now need to help all clubs, particularly grass-roots clubs, to fulfil the critical role of ensuring that the population is as active and healthy as possible, to provide a strong counter to the Covid virus? Delivering and funding that objective must remain a key priority for the Government.

I am grateful to my noble friend for the opportunity to mention sports and to thank him for his advocacy of sports, both national and grass roots, during the epidemic. He is entirely right: if there has been one striking aspect of the pandemic, it has been the way that the British public have missed the elite sports that they follow and support avidly, and have increasingly participated in their personal fitness and sport. I cannot help thinking that this pandemic will be something of an inflection point in the British people’s attitude to sport. I support the department’s campaign to encourage people to do more sport. We are doing an enormous amount to open stadia. I am pleased that we have new guidelines that will enable large stadia to open, albeit with social distancing. But my noble friend is entirely right that we must do more to support the grass roots, and I know that colleagues in the DCMS are doing as much as they can to help.

My Lords, there is growing concern that the unique Pfizer vaccine, with an mRNA genetic molecule that cellular machinery reads in order to build proteins, could be incompatible with the World Anti-Doping Code for all sports men and women, including Premier League footballers and Olympic athletes. Since the World Anti-Doping Agency already bans the use of agents designed to impact genome sequences if they have the potential to enhance sport performance or provide unfair advantage, and pursuant to my noble friend’s very helpful answer yesterday, will the Government undertake to work with the World Anti-Doping Agency and offer comprehensive advice to the sporting world before any vaccine programme begins?

I am extremely grateful to my noble friend for flagging up this important concern, which I acknowledge is a serious worry for those in the athletic and sporting arena. Colleagues at DCMS are aware of this concern. It is extremely early days and we do not know what the impact of the vaccine will be on the kind of protocols analysed by the World Anti-Doping Agency, but we have sought advice from the WPA on this matter and I will be happy to convey it as soon as it arrives.

The noble Baroness is right that we face a challenge. While there will be millions of people who will come forward emphatically to have the vaccine, there will be some who are either disengaged with the British Government or actively hostile to the thought of a vaccine, and we take seriously the disruption caused by those who seek to profit either financially or politically from the confusion and distress caused by anti-vax campaigns. It is not appropriate for me to discuss at the Dispatch Box the detailed measures we are putting in place to deal with the anti-vax message, but I can reassure the noble Baroness that they are focused, energetic and proving to be effective.

We also take seriously our efforts to reach hard-to-reach communities—those who might not have confidence in the Government or we might not have the right connection with. Those communities are exactly the ones we need to vaccinate, and we are making them an enormous priority in our efforts.

Will my noble friend the Minister ensure that among the priority groups for vaccination will be the sportsmen, sportswomen and their entourages who are seeking to represent GB in international events during 2021, in particular the Olympic and Paralympic athletes who aim to qualify for and participate in the Olympic Games in Tokyo? Will the Government urgently consult with the World Anti-Doping Agency to ensure that all approved vaccines are exempt from any possible breach of the current regulations on doping?

I am grateful to my noble friend for that question, which is incredibly important because we all take our preparation for the Olympics extremely seriously. I am also grateful to him for giving me advance notice of it. There is a desire for all the UK population to be vaccinated, including those who represent the UK. Prioritisation decisions will be based on vaccine availability and scientific clinical evidence on the safety and efficacy within different population groups. The JVCI is the independent expert on this and will make the decision he refers to. The World Anti-Doping Agency is actively responding to the coronavirus outbreak as it relates to the global anti-doping programme and the regulations are evolving rapidly.

Over 90% of the population will be unlikely to have suffered from Covid-19 when the restrictions are lifted. Will the Government therefore initiate and support a nationwide fitness and recreation campaign, given that physical exercise can help ensure that immune systems become stronger and less susceptible to infections and their most severe consequences and lead to a greater ability to recover from infections?

My noble friend is entirely right. Fitness and the Covid epidemic are closely connected. The Secretary of State for Health has made it clear that a daily walk should be part of everyone’s lockdown arrangements and we are ambitious to make fitness and diet part of the agenda as we leave the Covid lockdown.
