Question to the Department of Health and Social Care:
To ask the Secretary of State for Health, how many (a) official and (b) official sensitive documents have been recorded as lost by his Department since 8 May 2015.

The Department has had two reported data-related incidents since 8 May 2015 that involved the loss of electronic or physical documents.
