Southampton City Primary Care Hub, the local Wave 2 Prime Minister’s GP Access Fund scheme in Southampton, supports 276,338 patients (registered population at April 2016) registered across 33 general practitioner practices, and works from five hubs. There were 4,873 referrals to the scheme between 22 June 2015 to 13 March 2016 and the scheme is still operating.
