Question to the Department of Health and Social Care:

To ask the Secretary of State for Health and Social Care, how many unexpected deaths have occurred while an individual has been in the care of (a) the Tees, Esk and Wear Valleys Foundation NHS Trust, (b) West Lane Hospital and (c) Roseberry Park Adult Service (i) as an inpatient, (ii) while receiving care in the community and (iii) within three months of discharge from care in each year since 2016.

The information is not held in the format requested as there is no definition of ‘unexpected death’. However, NHS England and NHS Improvement publish patient safety incidents reported by trusts, including those reported with a degree of harm of death, which is available at the following links:
