Services are expected to work together to plan transition effectively and to meet the needs of young people, as well as their families and carers.

NHS England published its Model Specification for Transitions from Child and Adolescent Mental Health Services, in January 2015, to support better transition planning and delivery by local providers.

Transition from children and young people’s mental health services has also been included as one of 13 mandatory national indicators in the Commissioning for Quality and Innovation scheme (2017/19). This offers financial incentives to local areas to make improvements in support of the Five Year Forward View for Mental Health and National Health Service mandate, and will encourage improved transition planning and better experiences for children and young people.
